« Every gynaecologist knows how much imaging depends<br />
on the skill of “his/her” radiologist »<br />

Mammograms are an integral but very special part of a gynaecologist’s everyday work.<br />
They are usually recommended to women every 18 to 24 months from<br />
the age of 40 to 50 onwards, depending on a woman’s preferences and<br />
individual condition.<br />
Mammography is of primordial importance in early diagnosis of abnormalities,<br />
which are then confirmed or disproved by histological examination.<br />
Breast cancer (44,000 new cases every year) is a source of great concern<br />
to women and anxiety among their doctors.<br />
We cannot “miss a diagnosis” – we would be subjected to much greater<br />
criticism in this case than we would for other cancers if we failed to carry<br />
out effective screening because, as we know, late diagnosis is highly prejudicial<br />
to the health of our patients.<br />
In short, failing to make a diagnosis when it is possible to do so would have<br />
an extremely negative impact on the patient’s perception and expectation<br />
of our medical expertise and on our own perception of our skills.<br />
The breast, however, is a very difficult organ to x-ray. Not only does it consist<br />
of glands, connective and adipose tissue and skin, but also, in certain<br />
women, “densities” that may make it very difficult to “read”. We will all have<br />
fallen prey at some time to the traps inherent to x-rays!<br />
Every gynaecologist knows how much images depend on the work of “his/<br />
her radiologist” and the extent to which, when he’s found a “good one”, he’ll<br />
often continue to work with him, just like our patients who also continue to<br />
consult “their” gynaecologist once they’ve found one they feel happy with. A<br />
“good radiologist” combines considerable professional experience built up<br />
over years of work and increasingly effective radiological technology.<br />
<strong>Dr</strong>. Pierre-Alain <strong>Goumot</strong> describes the great radiologists that we have all<br />
been delighted to have come across at some time or other.<br />
Now at the pinnacle of his career with a high level of expertise, extensive<br />

Les gynécologues entretiennent avec la mammographie des rapports très intimes.<br />
Cet examen occupe à vrai dire une place très particulière dans notre<br />
exercice quotidien…<br />
Selon ses préférences et l’individualité de chaque femme, cet examen<br />
radiologique est demandé tous les 18 à 24 mois à partir de 40-50 ans.<br />
Car c’est de lui que dépend le diagnostic précoce d’anomalies, anomalies<br />
à confirmer ou infirmer ensuite par l’histologie<br />
Le cancer du sein - 44 000 nouveaux cas par an – terrorise de plus en<br />
plus les femmes et préoccupe fort leurs médecins.<br />
Il est interdit ici de « rater un diagnostic »: on nous en voudra beaucoup<br />
plus que pour d’autres cancers de ne pas avoir su mettre en place le<br />
dépistage efficace. Et de fait, on le sait bien, ce retard au diagnostic est<br />
réellement très préjudiciable à la santé de nos patientes<br />
Bref, ne pas avoir su faire un diagnostic alors qu’il était possible est vraiment<br />
catastrophique au regard des compétences qu’elles attendent de<br />
nous et de l’idée aussi que l’on se fait de sa propre compétence.<br />
Or, le sein est un organe vraiment difficile à radiographier. Fait de glandes,<br />
de tissus conjonctifs et adipeux, de peau, il présente de plus selon les<br />
femmes des « densités » qui le rendent parfois fort difficile à « lire ». À<br />
combien de pièges d’imagerie n’avons-nous pas tous été confrontés !<br />
Tout gynécologue sait à quel point les images dépendent du travail de<br />
« son radiologue » à tel point que lorsqu’il en a trouvé « un bon », il lui<br />
reste souvent très fidèle, à l’instar de nos patientes qui savent, elles aussi,<br />
rester fidèles à « leur gynécologue, une fois qu’elles l’ont trouvé(e) ». Le<br />
« bon radiologue » est celui ou celle qui sait associer une expérience professionnelle<br />
importante, une expertise sans cesse nourrie par les années<br />
de pratiques, à une technologie radiologique toujours plus performante.<br />
Le Docteur Pierre-Alain <strong>Goumot</strong> fait partie de ces grands radiologues<br />
qu’on est heureux d’avoir rencontrés un jour.<br />
Au fait extrême de son art et de son savoir, l’œil exercé et perspicace,<br />

Cancer research has shown that it is possible to significantly reduce the mortality caused by the complications<br />
of breast cancer if it is diagnosed early.<br />
This result depends on a number of factors e.g. the equipment, the radiographer<br />
and the radiologist. Together, they must operate as an experienced<br />
team.<br />
The mammography must be of excellent quality and must be interpreted<br />
in accordance with a protocol that includes clinical and ultrasound data.<br />
The decrease in doses of radiation, the ongoing improvement to quality<br />
thanks to excellent resolution and the use of digitisation to improve image<br />
quality still further (detail, contrast, pattern recognition and back-up) have<br />
relegated conventional techniques to the “Middle Ages”.<br />
Twenty years ago, the challenge was to improve the quality of mammograms<br />
– that has now been achieved.<br />
Other technologies currently on the horizon include 3D imaging, holograms<br />
and pattern recognition.<br />
For radiologists, this data will require a period of learning, but learning is a<br />
lifelong process and part of his/her everyday responsibility.<br />
This should not prevent a radiologist from listening to his patient and providing<br />
her with the best possible service. She is a unique human being to<br />
whom he should deliver the results face to face, explaining his diagnostic<br />
approach.<br />
The qualities required for a diagnostic approach to breast cancer (attention<br />
and vigilance) are fundamental and they have only one aim - to avoid<br />
missing the signs of breast cancer visible on a mammogram.<br />
Modesty is another quality that results from this approach.<br />

Les recherches sur le cancer ont révélé qu’il est possible de<br />
réduire de façon significative la mortalité due aux<br />
complications du cancer du sein si on le découvre<br />
précocement.<br />
Ce résultat dépend de nombreux facteurs : de<br />
l’appareil, de la technicienne et des médecins<br />
radiologues qui forment une équipe entraînée.<br />
La mammographie doit être d’excellente qualité et<br />
interprétée avec une heuristique associant les données<br />
de la clinique et de l’échographie…<br />
La diminution des doses d’irradiation, l’amélioration<br />
continue de la qualité, une bonne résolution, l’apport<br />
de la numérisation accentuant sans cesse la qualité de<br />
l’image : finesse, contraste, reconnaissance de formes,<br />
archivage ont relégué au « Moyen Âge » les techniques<br />
classiques…<br />
Le défi, il y a 20 ans, consistait à améliorer la qualité de<br />
la mammographie, nous y sommes.<br />
D’autres voies nous attendent : la 3D, l’holographie,<br />
la reconnaissance des formes.<br />
Ces données impliquent pour le médecin radiodiagnosticien<br />
un apprentissage, une formation continue<br />
qui engagent sa responsabilité au quotidien.<br />
Cela ne doit pas empêcher le radiologue d’être à<br />
l’écoute et au service de sa patiente, un être humain<br />
unique à qui il se doit de délivrer oralement un<br />
résultat en expliquant sa démarche diagnostique.<br />
Les qualités requises pour l’approche diagnostique<br />
du cancer du sein, attention et vigilance, sont<br />
fondamentales avec, comme seul but, de ne pas passer<br />
à côté des signes mammographiques d’un cancer du<br />
sein.<br />
Une autre qualité en découle : la modestie.

Radiologues spécialisés en sénologie, nous<br />
avons utilisé quotidiennement ces dix dernières<br />
années le système FUJI FCR 5 000 de<br />
résolution 50 µm, utilisant les écrans radio<br />
luminescents à mémoire avec des cassettes ou<br />
des écrans plaques.<br />
Il nous a ensuite été proposé de tester la dernière<br />
technologie développée par Fujifilm, à<br />
savoir un capteur plan direct 50 µm, à conversion<br />
directe avec double couche de sélénium<br />
amorphe, dont le principe est expliqué à la fin<br />
de cet ouvrage.<br />
Nous disposons ainsi depuis plus d’une année,<br />
au sein de notre centre, de deux mammographes<br />
AMULET complets intégrant des fonctionnalités<br />
d’utilité pratique.<br />
Dès les premiers essais, nous avons pu immédiatement<br />
constater combien nous avions ainsi<br />
gagné en rapidité d’accès aux images ainsi qu’en<br />
amélioration de la résolution et du contraste.<br />
En effet, grâce à des logiciels performants, la<br />
console « AXON » permet une lecture par un<br />
traitement d’images « sur mesure ». Le système<br />
d’archivage SYNAPSE offre quant à lui la<br />
possibilité de lire et de comparer directement<br />
en salle d’examen des images d’actualités différentes.<br />
De plus, le système CAD peut fournir<br />
une aide au diagnostic utile.<br />
Devant la qualité voire la beauté des images<br />
obtenues, nous avons alors pensé vous présenter<br />
ces clichés de manière esthétique et<br />
ludique à la fois sous forme d’un quizz.<br />
Nous avons ainsi sélectionné ces images parmi<br />
plusieurs milliers.<br />
À titre d’information, la classification ACR en<br />
mammographie a été appliquée, en sachant<br />
qu’il existe des nuances très faibles selon les<br />
équipes. Pour les scores ACR 4 et 5, nous avons<br />
donné le plus souvent le résultat histologique<br />
(sur prélèvement biopsique et chirurgical).<br />
Toutefois, malgré les avancées qu’apporte la<br />
technologie, il convient de rappeler que nos<br />
maîtres nous ont appris à ne jamais interpréter<br />
une image sans avoir personnellement interrogé<br />
et examiné la patiente. Aussi, dans la pratique,<br />
nous continuons d’effectuer un interrogatoire<br />
ciblé, une inspection attentive (patiente<br />
assise, les yeux du médecin à la hauteur du<br />
sillon sous-mammaire) et une écho-palpation<br />
facilitée par la visualisation des images mammographiques<br />
en salle d’examen.<br />
Rappelons aussi qu’il ne faut pas faire d’histologie<br />
sur une image de mammographie… en<br />
effet, souvenez-vous que l’image n’est pas l’objet,<br />
comme le précisait René Magritte.<br />
Et n’oublions pas qu’il existe des seins normaux,<br />
mais cela est une autre histoire…<br />

As radiologists specialising in breast-related diseases, we<br />
have been using the FUJI FCR 5000 system with 50<br />
µm resolution for the past ten years. We also use LCD<br />
x-ray screens with a memory stored on cassettes or<br />
imaging plates.<br />
We were then asked to test the latest technology developed<br />
by Fujifilm i.e. a 50 µm pixel pitch, direct conversion<br />
sensor with a double layer of amorphous selenium.<br />
The principle behind the equipment is explained at the<br />
end of this work.<br />
For the past year or more, our centre has therefore<br />
been equipped with two complete Amulet mammography<br />
machines that have practical, integrated functionalities.<br />
As soon as we carried out the first tests, we immediately<br />
noted how much time we had saved in accessing<br />
the images and how much better the resolution and<br />
contrast.<br />
Thanks to very efficient software, the “Axon” console<br />
allows for read-outs based on “customised” image<br />
processing. The Synapse storage system makes it possible<br />
to read and compare images from different dates<br />
directly in the consultation room. Moreover, the CAD<br />
system can provide useful assistance with diagnosis.<br />
Given the quality or even the beauty of the resultant<br />
images, we decided to share the plates with you in an<br />
aesthetically pleasing and entertaining way, in the form<br />
of a quiz.<br />
We selected these images from among several thousand.<br />
For your information, the ACR mammography classification<br />
was applied since we knew that there<br />
are very slight differences from one team to another.<br />
For ACR scores of 4 and 5, we usually give<br />
the histological results (from biopsy and surgical<br />
sampling).<br />
However, despite the advances provided by technology,<br />
we should remember that our teachers<br />
taught us never to read an image without having<br />
personally questioned and examined the patient.<br />
Thus, in everyday practice, we must continue to<br />
pose targeted questions, carry out a very careful<br />
examination (patient seated, with the doctor’s eyes<br />
on a level with the sub-mammary ridge) and perform<br />
an ultrasound-palpation which is made easier<br />
thanks to the screening of the mammography images<br />
in the consulting room.<br />
It is worth remembering that histology should not<br />
be requested solely on the basis of an x-ray plate.<br />
To quote René Magritte, “Remember that an image<br />
is not the object itself”.<br />
Remember, too, that there are normal breasts –<br />
but that’s a whole other story!<br />

Les techniques d’imagerie dans le domaine de<br />

The new type of flat panel detectors developed by<br />

millions de pixels, le système Fujifilm permet<br />
d’obtenir une image très nette du sein et particulièrement<br />

resolution available at the present time in digital<br />
mammography, with a 50 µm pixel pitch. This accuracy<br />
in sensor components detects even the smallest<br />
micro-calcifications and studies their morphology<br />
with a high level of precision. With almost 29<br />

clear picture of the breast and contains a particularly<br />
large amount of clinical data.
La classification ACR<br />

This radiological classification, first described<br />
by American radiologists (American College of<br />
Radiologists), is now used in France.<br />
It provides a means of describing breast<br />
imaging (mammography, ultrasound and MRI)<br />
by classifying abnormalities on the basis of<br />
appearance as shown below:<br />
ACR 0: reporting requires additional imaging.<br />
At present it is incomplete. This is especially<br />
common during screening. It may require<br />
additional, localised images, enlargement,<br />
ultrasound or MRI.<br />
Cette classification radiologique, décrite d’abord<br />
par les radiologues américains (American College of<br />
Radiologists), a été adoptée en France.<br />
Elle permet de décrire l’imagerie du sein<br />
(mammographie, mais aussi échographie et IRM) en<br />
classant les anomalies en fonction de leur aspect, selon<br />
le classement ci-dessous :<br />

ACR 1: normal mammography. NTR.<br />
ACR 2: negative mammography but the<br />
radiologist describes a benign malformation<br />
e.g. hamartoma, calcified fibroadenoma,<br />
intramammary lymph node, lipoma etc.<br />
ACR 3: a probably benign malformation evident<br />
on the mammogram. Short-term monitoring is<br />
recommended to confirm the stability of the<br />
image(s). The required interval is not specified<br />
and may be of the order of 4 to 6 months.<br />
ACR 4: suspect malformation. A sample should<br />
be considered (biopsy). High probability of<br />
malignancy.<br />
ACR 5: resemblance to breast cancer and high<br />
probability of malignancy.<br />
Remember that:<br />
- mammography does not detect all breast<br />
cancers;<br />
- a negative screening test is not absolute proof;<br />
- Any change in the breast not linked to the<br />
menstrual cycle requires further examination.<br />
ACR 1 : examen mammographique normal. RAS.<br />
ACR 2 : examen mammographique négatif mais<br />
le radiologue décrit une anomalie de type bénin.<br />
Exemple : hamartome, fibroadénome calcifié, ganglion<br />
intramammaire, lipome…<br />
ACR 3 : anomalie mammographique probablement<br />
bénigne. On propose une surveillance à court terme<br />
afin d’affirmer la stabilité de/ou de images observées.<br />
L’intervalle requis n’est pas spécifié et peut être de 4 à<br />
6 mois.<br />
ACR 4 : anomalie suspecte. Un prélèvement doit être<br />
envisagé (biopsie). Probabilité importante de malignité.<br />
ACR 5 : aspect de cancer du sein et haute probabilité<br />
de malignité.<br />
Il faut bien se rappeler que :<br />
- la mammographie ne détecte pas tous les cancers du<br />
sein ;<br />
- qu’un examen de dépistage négatif n’est pas<br />
déterminant ;<br />
- que toute modification du sein non liée au cycle doit<br />
attirer l’attention.<br />
