Outre les deux causes précédentes, une ophtalmoplégie externe<br />
progressive peut correspondre à une dystrophie oculopharyngée, à<br />
une maladie mitochondriale (syndrome de Kearns-Sayre ou<br />
délétions <strong>multiples</strong> [55] ) ou à un ensemble disparate d’affections dont<br />
l’ophtalmoplégie n’est qu’un aspect sémiologique particulier [16] :<br />
dystrophie myotonique de Steinert, myopathie centronucléaire,<br />
abêtalipoprotéinémie, maladie de Refsum, syndrome<br />
polyneuropathie-ophtalmologie-leucoencéphalopathie-pseudoobstruction<br />
intestinale (POLIP) [86] , syndrome de Moebius (paralysie<br />
congénitale bilatérale du VI et du VII), maladie de Steele-Richardson<br />
(où les signes sont en fait d’origine supranucléaire).<br />
PARALYSIES FAMILIALES DES NERFS CRÂNIENS [13]<br />
Ce groupe comprend <strong>des</strong> cas de paralysies faciales héréditaires, le<br />
plus souvent récidivantes. Une étroitesse congénitale de l’aqueduc<br />
de Fallope a été proposée comme explication. Il existe en outre<br />

: savoir déceler <strong>des</strong> signes centraux associés ; atteinte extraaxiale<br />
: importance majeure de l’imagerie et de l’étude du LCR ; toute<br />
paralysie unilatérale extra-axiale multiple <strong>des</strong> <strong>nerfs</strong> <strong>crâniens</strong> doit être<br />
considérée comme tumorale jusqu’à preuve du contraire ; rechercher la<br />
lésion en dehors du crâne si l’imagerie intra-crânienne est négative ;<br />
penser aux pathologies curables ou urgentes : dissections, anévrismes,<br />
myasthénie, ophtalmopathie thyroïdienne, encéphalopathie carentielle.<br />
