9. TITRE ET RESUME EN ANGLAIS<br />

Core decopression and iliac crest cancellous bone autografting in treatement of<br />

avascular necrosis of the femoral head : a retrospective and continued study of 66 cases.<br />

The goal of this study is to evaluate the effectiveness of core decompression and iliac<br />

crest cancellous bone autografting as preserving treatement of osteonecrosis of the femoral<br />

head. At 2 years following grafting, results are 58% of successes in global study with an<br />

overall survivorship of 29 months. One in addition notices a clear difference according to the<br />

stage with 64% of succes for the ARCO stage II (70% for stage IIA and IIB ) and only 45% for<br />

stage III. Two years after treatement, half of the initial radiological lesions remained stable and<br />

the follow-up shows a good perpetuation of the results beyond this period.<br />

It seems to bee a good indication for ARCO stage IIA/IIB. In the event of failure, this<br />

technic does not disturb the installation of a prothesis or resurfacing.<br />

In addition the role of tobacco has a long time been underestimated by authors in<br />

ethiopathology of this desease. If the high proportion smokers in our serie does not certainly<br />

make possible to conclude at the imputability of tobacco, one cannot exlude it for as much.<br />
