SDRI ET TRAITEMENT PAR SURFACTANT EXOGENE 3. La maladie des membranes hyalines ou syndrome de détresse respiratoire idiopathique 3.1. Définition Elle résulte d’une insuffisance de surfactant, et doit être suspectée lorsqu’une détresse respiratoire survient au cours des premières heures de la vie chez un prématuré surtout lorsque l’âge gestationnel est inférieur à 34 SA. Il s’agit d’une cause majeure de mortalité et morbidité néonatales. La maladie s’aggrave habituellement dans les 4-5 premiers jours de vie puis s’atténue. 3.2. On note aussi une polypnée*, une compliance pulmonaire* basse. L’hypoxémie est marquée avec une acidose mixte. Le déficit quantitatif en surfactant peut être noté par une analyse des sécrétions trachéales. Le collapsus* avec diminution du nombre d’espaces intercostaux est révélé par le cliché thoracique.
