Recomendaciones para el reemplazo percutáneo<br />

de la válvula aórtica<br />

Clase I<br />

– El implante valvular percutáneo se halla indicado en<br />

pacientes con estenosis aórtica grave sintomática,<br />

que no son considerados candidatos para tratamiento<br />

quirúrgico por el equipo cardiológico, y que tienen<br />

posibilidad de mejorar su calidad y expectativa de<br />

vida en más de un año a pesar de la presencia de<br />

comorbilidades (Nivel de evidencia B).<br />

Clase IIa<br />

– Puede considerarse el implante valvular percutáneo<br />

en aquellos pacientes con estenosis aórtica grave<br />

sintomáticos, de riesgo quirúrgico alto estadificado<br />

mediante el puntaje de la Sociedad Americana de<br />

Cirugía Torácica, el EuroSCORE o el ArgenSCORE,<br />

considerados candidatos pasibles de cirugía, pero en<br />

quienes el equipo cardiológico considere indicar este<br />

tratamiento en base a la relación riesgo-beneficio<br />

(Nivel de evidencia B).<br />

Clase III<br />

– Pacientes con estenosis aórtica grave, sintomáticos,<br />

con riesgo quirúrgico moderado y bajo y sin criterios<br />

de inoperabilidad (Nivel de evidencia B).<br />

– Pacientes con estenosis aórtica grave, sintomáticos,<br />

con comorbilidades que generen una expectativa de<br />

vida menor a 1 año (Nivel de evidencia C).<br />
