Einleitung<br />
Nach Mollison [1] sind die Phänotypen des Lutheran-Systems unter der kaukasischen<br />
Bevölkerung wie folgt verteilt:<br />

In der Literatur wird Anti-Lu b für geringfügige bis mittelgradige Transfusionsreaktionen<br />
und als Ursache eines geringfügigen MHN verantwortlich gemacht.<br />
Die Antigen-Antikörper Reaktion zeigt eine typische Mischfeldagglutination, die aus losen<br />
Agglutinaten und nicht-agglutinierten Erythrozyten besteht [3].<br />
Antikörper gegen Antigene im Lutheran-System (Symbol nach der ISBT-Terminologie:<br />
LU, Nummer: 005) werden nicht häufig angetroffen, können aber nach Schwangerschaft<br />
oder Transfusion auftreten. Die Antigene sind bei Geburt nur unvollständig ausgebildet,<br />
so dass Anti-Lu a nicht als Ursache einer fetalen Erythroblastose (Morbus haemolyticus<br />
neonatorum, MHN) und nicht als klinisch signifikant eingestuft wird.<br />

Anti-Lu b has been reported to be implicated in mild to moderate transfusion reactions<br />
and as the cause of mild HDN.<br />
The antigen-antibody reaction produces small and loose agglutinates, interspersed<br />
among unagglutinated red cells, causing characteristic mixed-field appearance [3].<br />
Antibodies against antigens in the Lutheran system (ISBT terminology symbol LU,<br />
number 005) are not often encountered. Anti-Lu a and anti-Lu b are produced in response<br />
to pregnancy or transfusion. The antigens are poorly developed at birth, consequently,<br />
anti-Lu a has not been found to be the cause of haemolytic disease of the newborn (HDN)<br />
and is not considered to be of clinical significance.<br />

Les anticorps anti-Lu b sont connus pour être responsables de réactions transfusionnelles<br />
modérées et de MHNN mineures.<br />
La réaction antigène-anticorps produit de petits agglutinats fragiles, dispersés parmi des<br />
hématies non agglutinées, provoquant l’image caractéristique de double population [3].<br />
Les anticorps contre les antigènes du système Luthéran (terminologie ISBT: symbole LU,<br />
numéro 005) sont rares; ils sont secondaires à une transfusion ou une grossesse. Les<br />
antigènes étant faiblement développés à la naissance, les anticorps anti-Lu a n’ont pas été<br />
décrits lors de maladie hémolytique du nouveau-né (MHNN) et ne sont pas considérés<br />
comme cliniquement importants.<br />

Achtung: Die Ausgangsmaterialien, aus denen diese Produkte hergestellt wurden,<br />
haben sich bei der Prüfung mit zugelassenen Reagenzien als nicht reaktiv für HBsAg,<br />
HCV und HIV (1 und 2) erwiesen. Allerdings kann kein verfügbares Testverfahren das<br />
Nichtvorhandensein infektiöser Substanzen garantieren. Aus Humanblut gewonnene<br />
Produkte sollten immer als potentiell infektiös angesehen werden.<br />

Probenmaterial<br />
Für verlässliche Resultate sollte die Bestimmung mit frisch abgenommenen Proben<br />
durchgeführt werden oder in Übereinstimmung mit lokalen Laborvorschriften für die<br />
Akzeptanz von Probenmaterial erfolgen. Vorzugsweise sollte die Probengewinnung in den<br />
Antikoagulantien Citrat, EDTA oder CPD-A erfolgen. Native Proben (kein Antikoagulanz)<br />
können auch verwendet werden.<br />
Sample material<br />
For optimal results, the determination should be performed using a freshly drawn<br />
sample, or in accordance with local laboratory procedures for sample acceptance criteria.<br />
Preferably, blood samples should be drawn into citrate, EDTA or CPD-A anticoagulant.<br />
Samples drawn into plain tubes (no anticoagulant) may also be used.<br />
Echantillons<br />
Afin d’obtenir des résultats fiables, la détermination devrait se faire sur du matériel<br />
fraîchement prélevé ou conforme aux exigences du laboratoire auquel la demande<br />
d’analyses est adressée. L’échantillon devrait être prélevé de préférence sur anticoagulant<br />
citrate, EDTA ou CPD-A. Du sang prélevé sans anticoagulant (natif) peut également être<br />
utilisé.<br />

Eine 5%ige Erythrozytensuspension in <strong>ID</strong>-Diluent 1 wie folgt herstellen:<br />

1. 0,5 mL <strong>ID</strong>-Diluent 1 in ein sauberes Röhrchen pipettieren.<br />
2. 50 µL Vollblut oder 25 µL Erythrozytenkonzentrat zugeben, leicht mischen.<br />
3. Die Erythrozytensuspension 10 Minuten bei Raumtemperatur (18-25 °C) inkubieren.<br />
1. Dispense 0,5 mL of <strong>ID</strong>-Diluent 1 into a clean tube.<br />
2. Add 50 µL of whole blood or 25 µL of packed cells, mix gently.<br />
3. Incubate the red cell suspension 10 minutes at room temperature (18-25 °C).<br />

Méthode<br />
Ne pas utiliser les cartes <strong>ID</strong> présentant des signes de desséchement, des bulles d‘air dans<br />
le gel, un système de fermeture endommagé, des goutelettes de gel ou de surnageant<br />
sur les paroies supérieures des micotubes ou sur la face interne de la languette<br />
d‘aluminium.<br />

Do not use <strong>ID</strong>-<strong>Cards</strong> which show signs of drying, have bubbles, damaged seals, drops of<br />
gel or supernatant in the upper part of the microtubes or on the underside of the<br />
aluminium foil.<br />

• Des réactions positives de + à ++++ indiquent la présence de l’antigène correspondant.<br />
• Positive reactions of + to ++++ indicate presence of the corresponding antigen.<br />
• Positive Reaktionen (+ bis ++++) bedeuten Nachweis des entsprechenden Antigens.<br />
• Des réactions positives apparaissent souvent comme une double population.<br />
Cependant, une double population dans le même microtube peut également indiquer<br />
la présence d’hématies positives et négatives de l’antigène correspondant (par<br />
exemple si la configuration du sang transfusé était différente du sang de patient).<br />
• Positive reactions often appear as a double population. However, a double population<br />
may also indicate the presence of cells positive and negative for the corresponding<br />
antigen (e.g. Post-transfusion, if the antigen configuration of the transfused blood was<br />
different from the patient’s).<br />
• Positive Reaktionen zeigen sich oft als Doppelpopulationen. Trotzdem kann eine<br />
Doppelpopulation in dem gleichen Mikroröhrchen das Vorliegen von<br />
Antigen-negativen und -positiven Erythrozyten anzeigen (z.B. wenn die<br />
Antigenkonfiguration des transfundierten Blutes verschieden war zum Patienten).<br />
• Des réactions fortement positive (++++) sont très rares.<br />
• Strong ++++ reactions are very rare.<br />
• Starke Reaktionen (++++) sind äusserst selten.<br />
• Des réactions négatives indiquent l’absence de l’antigène correspondant.<br />

• Negative Reaktionen bedeuten Abwesenheit des entsprechenden Antigens.<br />
Limites<br />
a) Les cartes-<strong>ID</strong> présentant des bulles d‘air dans le gel ou des goutelettes dans la partie<br />
supérieur des microtubes ou sur la languette métallique doivent être centrifugées<br />
avant utilisation.<br />
b) Des hématies présentant, in vivo ou in vitro, une polyagglutinabilité due à un<br />
cryptoantigène, tel que l’antigène T, peuvent réagir avec tous les antisérums humains.<br />
Dans ce cas, des investigations supplémentaires sont nécessaires.<br />
c) Des contaminations, bactériennes ou autres, du matériel utilisé peuvent provoquer<br />
des résultats faussement positifs ou faussement négatifs.<br />
d) Des résidus de fibrine dans la suspension d’hématies peuvent emprisonner quelques<br />
hématies non agglutinées, formant ainsi une fine ligne rose à la surface du gel, alors<br />
que la plupart des hématies sont dans le fond du microtube après centrifugation.<br />
e) L’observation stricte des méthodes et l’emploi de l’équipement recommandé sont<br />
essentiels. L’équipement doit être régulièrement contrôlé selon les procédures des<br />
GLP.<br />
f) L’utilisation de diluants autres que l’<strong>ID</strong>-Diluent 1 peut modifier les résultats.<br />
g) Des suspensions d’hématies trop concentrées ou trop diluées peuvent provoquer des<br />
résultats aberrants.<br />
Limitations<br />
a) <strong>ID</strong> cards which show air bubbles in the gel or drops in the upper part of the microtubes<br />
and/or the seal, must be centrifuged before use.<br />
b) Cells that have become polyagglutinable, due to cryptoantigen exposure, e.g. T<br />
antigen, either in vivo or in vitro, may react with all human antisera. Further investigation<br />
of such reactions is required.<br />
c) Bacterial or other contamination of materials used can cause false positive or false<br />
negative results.<br />
d) Fibrin residues in the red cell suspension may trap non-agglutinated cells presenting a<br />
fine pink line on top of the gel while most of the cells are on the bottom of the<br />
microtube after centrifugation.<br />
e) Strict adherence to the procedures and recommended equipment is essential. The<br />
equipment should be checked regularly according to GLP procedures.<br />
f) Use of suspension solutions for red cells other than <strong>ID</strong>-Diluent 1 may modify the<br />
reactions.<br />
g) Too heavy or too weak red cell suspensions can cause aberrant reactions.<br />
Einschränkungen<br />
a) <strong>ID</strong>-Karten mit Luftblasen im Gel oder Tropfen im oberen Bereich der Mikrokammern<br />
und/oder Versiegelung müssen vor Gebrauch zentrifugiert werden.<br />
b) Polyagglutinable Erythrozyten, z.B. infolge T-Aktivierung, sowohl in vivo wie auch in<br />
vitro, können mit allen humanen Testseren reagieren. Weitere Untersuchungen sind<br />
angezeigt.<br />
c) Bakterielle oder andere Kontaminationen des verwendeten Materials können falsch<br />
positive oder falsch negative Ergebnisse verursachen.<br />
d) Fibrinreste in der Erythrozytensuspension können nicht-agglutinierte Zellen verkleben,<br />
die sich als feine rosa Linie auf der Geloberfläche darstellen, während die meisten<br />
Zellen nach Zentrifugation am Boden der Mikroröhrchen ein kompaktes Sediment<br />
bilden.<br />
e) Striktes Befolgen der Anleitungen und Verwendung des erforderlichen Arbeitmaterials<br />
sind unerlässlich. Das Arbeitsmaterial sollte regelmässig entsprechend der<br />
GLP-Richtlinien überprüft werden.<br />
f) Der Gebrauch anderer Lösungen als <strong>ID</strong>-Diluent 1 für die Erythrozytensuspensionen<br />
kann die Reaktionen beeinflussen.<br />
g) Zu starke oder zu schwache Erythrozytensuspensionen können abnormale Reaktionen<br />
hervorrufen.<br />

L’Anti-Lu b può essere causa di reazioni trasfusionali lievi-moderate e di lievi forme della<br />
malattia emolitica del neonato (HDN).<br />
La reazione antigene-anticorpo produce piccoli agglutinati sparsi fra gli eritrociti non<br />
agglutinati, creando una caratteristica doppia popolazione.<br />
È raro incontrare anticorpi contro gli antigeni nel sistema Lutheran (simbolo secondo la<br />
termologia ISBT: LU, numero 005). Gli anticorpi Anti-Lu a e anti-Lu b vengono prodotti in<br />
seguito a gravidanza o trasfusione. Dato che alla nascita si ha una scarsa produzione di<br />
antigeni, gli anticorpi anti-Lu a non sono indiziati come causa della malattia emolitica del<br />
neonato (HDN) e non sono considerati clinicamente rilevanti.<br />

Se ha descrito la relación del anticuerpo anti-Lu b con reacciones post-transfusionales<br />
leves o moderadas y con casos leves de enfermedad hemolítica del recién nacido.<br />
La reacción antígeno-anticuerpo produce aglutinados pequeños y poco compactos,<br />
mezclados con eritrocitos no aglutinados, lo que da lugar a un aspecto característico de<br />
doble población [3].<br />
No es frecuente encontrar anticuerpos contra los antígenos del sistema Lutheran<br />
(símbolo LU y número 005 en la terminología ISBT). Los anticuerpos anti-Lu a y anti-Lu b<br />
se producen en respuesta al embarazo o a la transfusión. Los antígenos están poco<br />
desarrollados al nacer, por lo que no se ha observado que el anticuerpo anti-Lu a dé lugar<br />
a enfermedad hemolítica del recién nacido y no se considera que sea clínicamente<br />

Os anticorpos anti-Lu b são conhecidos como estando implicados em reacções<br />
transfusionais suaves a moderadas e como causa de HDN suave (doença hemolítica do<br />
recém-nascido).<br />
A reacção antigénio-anticorpo produz pequenos aglutinados livres, dispersos entre<br />
eritrócitos não aglutinados, dando origem à imagem característica de população dupla<br />
[3].<br />
Os anticorpos contra antigénios do sistema luterano (símbolo LU na terminologia ISBT,<br />
número 005) são raros. Os anticorpos anti-Lu a e anti-Lu b são produzidos na sequência de<br />
uma gravidez ou transfusão. Dado que os antigénios estão pouco desenvolvidos à<br />
nascença, os anticorpos anti-Lu a não foram identificados como causa da doença<br />
hemolítica do recém-nascido (HDN) e não são considerados clinicamente importantes.<br />

Campioni<br />
Per ottenere risultati attendibili, si consiglia di eseguire la determinazione su un<br />
campione fresco o conforme alle procedure del laboratorio per i criteri di accettazione dei<br />
campioni. I campioni devono essere prelevati preferibilmente in citrato, EDTA o CPD-A. Si<br />
possono comunque usare anche campioni prelevati in provette normali (senza<br />
anticoagulante).<br />
Muestras<br />
Para un resultado óptimo, la determinación debe realizarse con una muestra recién<br />
extraída, o cumpliendo la normativa local del laboratorio en cuanto a criterios de<br />
aceptabilidad de las muestras. Preferiblemente, las muestras de sangre deben recogerse<br />
utilizando citrato, EDTA o CPD-A como anticoagulante. También es posible utilizar<br />
muestras recogidas en tubos sin anticoagulante.<br />
Amostras<br />
Para obtenção dos resultados ideais, a determinação deve ser realizada numa amostra<br />
recentemente colhida, ou em conformidade com os critérios de aceitação do<br />
procedimento laboratorial local. As amostras de sangue devem, de preferência, ser<br />
colhidas em anticoagulante citrato, EDTA ou CPD-A. Também é possível utilizar amostras<br />
colhidas em tubos limpos (sem anticoagulante).<br />

2. Aggiungere 50 µL di sangue intero o 25 µL di emazie concentrate, miscelare<br />
delicatamente.<br />
3. Incubare la sospensione di eritrociti per 10 minuti a temperatura ambiente (18‐25 °C).<br />

• Reacções positivas de + a ++++ indicam a presença do antigénio correspondente.<br />
• As reacções positivas aparecem frequentemente como populações duplas. No<br />
entanto, uma população dupla no mesmo microtubo pode indicar a presença de<br />
eritrócitos positivos e negativos para o antigénio correspondente (por exemplo após<br />
transfusão, se a configuração antigénica do sangue transfundido for diferente da do<br />
doente).<br />
• As reacções intensas ++++ são muito raras.<br />
• As reacções negativas indicam a ausência do antigénio correspondente.<br />

• Son poco frecuentes reacciones intensas ++++ .<br />
• Las reacciones negativas indican ausencia del correspondiente antígeno.<br />
• Una reazione positiva da + a ++++ indica la presenza dell‘antigene corrispondente.<br />
• Reazioni positive si presentano spesso come doppia popolazione. Una doppia<br />
popolazione, tuttavia, può anche indicare la presenza di eritrociti negativi e positivi per<br />
l‘antigene corrispondente (ad es. post-trasfusione se la configurazione antigenica del<br />
sangue trasfuso era diversa da quella del paziente).<br />
• Le reazioni forti del tipo ++++ sono molto rare.<br />
• Una reazione negativa indica l‘assenza dell‘antigene corrispondente.<br />
Limitações<br />
a) Os cards-<strong>ID</strong> que tenham bolhas de ar no gel ou gotas na parte superior dos<br />
microtubos e/ou no segmento de alumínio devem ser centrifugados antes de usar.<br />
b) Os eritrócitos que se tenham tornado poli-aglutináveis devido à exposição a um<br />
criptoantigénio, por exemplo ao antigénio T, quer in vivo quer in vitro, podem reagir<br />
com todos os antisoros humanos. São necessárias investigações suplementares de<br />
tais reacções.<br />
c) A contaminação, bacteriana ou outra dos materiais utilizados pode causar resultados<br />
falsamente positivos ou falsamente negativos.<br />
d) Resíduos de fibrinas na suspensão de eritrócitos podem reter eritrócitos não<br />
aglutinados, formando assim uma fina linha corderosa à superfície do gel, ao passo<br />
que a maior parte dos eritrócitos migra para o fundo do microtubo após<br />
centrifugação.<br />
e) O cumprimento estrito dos procedimentos e a utilização do equipamento<br />
recomendado são essenciais. O equipamento deve ser verificado regularmente em<br />
conformidade com os procedimentos de BPL.<br />
f) A utilização de soluções de suspensão de eritrócitos diferentes de <strong>ID</strong>-Diluent 1 pode<br />
alterar as reacções.<br />
g) Suspensões de eritrócitos demasiado concentradas ou demasiado diluidas podem<br />
provocar resultados errados.<br />
Limitaciones<br />
a) Las <strong>ID</strong>-Tarjetas que muestren burbujas de aire en el gel o gotas en la parte superior de<br />
los microtubos y/o de la lámina de sellado, deben ser centrifugadas antes de usarlas.<br />
b) Los eritrocitos que hayan pasado a ser poliaglutinables debido a una exposición a un<br />
criptoantígeno, por ejemplo antígeno T, tanto in vivo como in vitro, pueden reaccionar<br />
con todos los antisueros humanos. Estas reacciones requieren una investigación<br />
adicional.<br />
c) La contaminación de los materiales empleados, bacteriana o de otro tipo, puede<br />
provocar reacciones falsamente positivas o falsamente negativas.<br />
d) Los restos de fibrina en la suspensión de eritrocitos pueden atrapar los hematíes no<br />

mientras que la mayoría de los eritrocitos se encuentran en el fondo del microtubo<br />
tras el centrifugado.<br />
e) Es esencial atenerse estrictamente a los procedimientos y equipos recomendados. El<br />
equipo debe comprobarse periódicamente según la normativa de prácticas de<br />
laboratorio correctas (GLP).<br />
f) El empleo de soluciones de suspensión para eritrocitos distintos al <strong>ID</strong>-Diluent 1 puede<br />
modificar las reacciones.<br />
g) Las suspensiones de eritrocitos demasiado concentradas o demasiado diluidas<br />
pueden dar lugar a reacciones aberrantes.<br />
Limitazioni<br />
a) Le <strong>ID</strong>-<strong>Cards</strong> che mostrano bolle d’aria nel gel o gocce nella parte superiore del<br />
micropozzetto e/o sul foglio di alluminio, devono essere centrifugate prima dell’uso.<br />
b) Gli eritrociti che sono divenuti poliagglutinabili a causa di esposizione a un<br />
criptoantigene, ad es. l‘antigene T, sia in vivo che in vitro, possono reagire con tutti gli<br />
antisieri umani. Questo tipo di reazione richiede ulteriori ricerche.<br />
c) Le contaminazioni batteriche o di altro tipo del materiale utilizzato possono essere<br />
causa di risultati falsamente negativi o positivi.<br />
d) Eventuali residui di fibrina nella sospensione di eritrociti possono aderire alle emazie<br />
non agglutinate, e farle apparire come una sottile linea rosa sulla superficie del gel,<br />
mentre la maggior parte degli eritrociti dopo la centrifugazione forma un bottone<br />
compatto sul fondo della provetta.<br />
e) E‘ indispensabile seguire scrupolosamente le istruzioni e impiegare il materiale di<br />
lavoro raccomandato. Il materiale di lavoro deve essere controllato regolarmente<br />
secondo le direttive GLP.<br />
f) L‘impiego di soluzioni diverse dal diluente <strong>ID</strong>-Diluent 1 per le sospensioni di eritrociti<br />
può influire sulle reazioni.<br />
g) Una sospensione di eritrociti troppo concentrata o troppo diluita può causare reazioni<br />
anomale.<br />
Observações A centrifugação repetida do Card-<strong>ID</strong> não afecta o desempenho do produto. Observaciones El centrifugado repetido de la tarjeta <strong>ID</strong>-Card no afecta al comportamiento del producto.
