Infections with ECHO viruses can be confirmed by direct detection of the pathogen or<br />

serological evidence of specific antibodies. The PCR is an important complement to the<br />

classical methods while in recent years ELISA tests have established themselves. Due to<br />

the large number of serotypes a test with broad reactivity is required in order to detect all<br />

the various serotypes with one test. The <strong>SERION</strong> ELISA classic Echovirus tests are based<br />

on a mixture of recombinant antigens derived from conserved and subtype specific<br />

epitopes of the VP1 proteins of ECHO viruses E6 and E9. These epitopes have been<br />

determined to be sufficiently cross-reactive to enable the detection of infections caused by<br />

other ECHO virus serotypes.<br />

An acute ECHO viruses infection can be confirmed by the detection of specific IgM and /or<br />

IgA antibodies or a rising IgG titer. The highest frequency of IgM positive sera are found in<br />

children between the ages of one and ten years. IgM antibodies are usually detectable for<br />

a period of six to eight weeks. In rare cases IgM titers may persist up to six months after<br />

aseptic meningitis or three to six months following pericarditis. Patients with recurrent<br />

pericarditis may have IgM detectable for more than five years. IgA detection in cases of<br />

acute infection is a valuable supplement to IgM serology. Persistent IgA may be detectable<br />

in cases of type 1 diabetes and chronic heart disease for six months up to several years.<br />

3 TEST PRINCIPLE <strong>SERION</strong> ELISA classic<br />

The ELISA (Enzyme Linked Immunosorbent Assay) is an immunoassay, which is<br />

particularly suited to the determination of antibodies in the field of infectious serology. The<br />

reaction is based on the specific interaction of antibodies with their corresponding antigen.<br />

The test strips of the <strong>SERION</strong> ELISA classic microtiter plate are coated with specific<br />

antigens of the pathogen of interest. If antibodies in the patient´s serum sample are<br />

present, they bind to the fixed antigen. A secondary antibody, which has been conjugated<br />

with the enzyme alkaline phosphatase, detects and binds to the immune complex. The<br />

colourless substrate p-nitrophenylphosphate is then converted into the coloured product<br />

p-nitrophenol. The signal intensity of this reaction product is proportional to the<br />

concentration of the analyte in the sample and is measured photometrically.<br />
