orgánica de hierro, ésta no se manifiesta con idéntica<br />
expresión clínica. Así, mientras que en la HH el<br />
hierro se acumula fundamentalmente en células parenquimatosas<br />
y respeta en cierta medida el sistema<br />
mononuclear fagocítico, en la propia del área subsahariana<br />
el acúmulo es generalizado.<br />
Por otra parte, dolor abdominal, astenia, disfunción<br />
hepática y artropatía constituyen las alteraciones<br />
clínicas predominantes en la HH, mientras que<br />
la afectación gonadal y la insuficiencia cardíaca suelen<br />
ser los síntomas de presentación inicial de la hemocromatosis<br />
juvenil. En la hemocromatosis neonatal<br />
son frecuentes las complicaciones durante la vida<br />
intrauterina, en forma de oligoamnios y durante los<br />
primeros días de vida extrauterina, con hipoglicemia,<br />
acidosis láctica, ictericia, cirrosis hepática, fallo<br />
hepático, en ocasiones aparición de hepatocarcinoma<br />
y muerte con excepción de aquellos casos en que<br />
se ha procedido al trasplante hepático con éxito 43 .<br />
El déficit hereditario de ceruloplasmina es un proceso<br />
autosómico recesivo que suele cursar con síntomas<br />
neurológicos graves, por acúmulo de hierro<br />
en los ganglios de la base, diabetes mellitus y anemia<br />
microcítica hipocroma. Este cuadro se diferencia<br />
además de los anteriores en presentar una sideremia<br />
y saturación de la transferrina bajas 24 . Por último,<br />
la hipo o atransferrinemia hereditaria es también autosómica<br />
recesiva, se produce siderosis tisular con<br />
anemia microcítica ferropénica. Hay un incremento<br />
en la absorción intestinal de hierro, debido al estímulo<br />
que provoca la eritropoyesis ferropénica, sin<br />
que ese Fe se pueda incorporar a la eritropoyesis por<br />
falta de su transportador fisiológico 42 .<br />
Otras anomalías<br />
Se han descrito algunos casos familiares de anemia<br />
ferropénica con malabsorción intestinal, que podrían ser<br />
causados por defectos de alguna de las proteínas<br />
anteriores 44 .<br />
También se han descrito algunos pacientes con<br />
anomalías del TfR y clínica muy similar a la de los modelos<br />
animales 45 .<br />
Sorprendentemente, se ha observado que varias<br />
familias con cataratas bilaterales congénitas presentan<br />
hiperferritinemia sin sobrecarga real de Fe debido<br />
a mutaciones en la región IRE del ARN-m de la<br />
L-Ferritina, que caracteriza el síndrome de hiperferritinemia<br />
y cataratas, el papel de la L-Ferritina en la formación<br />
de cataratas está por definir 46 .<br />
