1 - INTENDED USE<br />
The MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> test is used to confirm the presence of hepatitis<br />
B surface <strong>Ag</strong> (<strong>HBs</strong> <strong>Ag</strong>) in samples of human serum or plasma found reactive by the MONOLISA ® <strong>HBs</strong><br />
<strong>Ag</strong> <strong>ULTRA</strong> screening test (codes 72346/72348).<br />
2 - PRINCIPLE OF THE MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> TEST<br />
The MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> test uses the principle of neutralization by excess<br />
of antibodies to anti-<strong>HBs</strong> (anti-<strong>HBs</strong> diluent: neutralization reagent) of the <strong>Ag</strong> <strong>HBs</strong> found in serum or<br />
plasma samples.<br />
It is recommended to test samples repeatedly found positive by the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
screening test with the reagents from the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> kit.<br />
If a sample is positive in <strong>HBs</strong> antigen, the anti-<strong>HBs</strong> antibodies in the neutralization reagent saturate<br />
the <strong>HBs</strong> antigen determinants of the sample which can no longer bind to the antibody fixed on the<br />
solid phase. A decrease in optical density will be observed when comparing the same sample in<br />
which the neutralization reagent is replaced by a negative control diluent not containing anti-<strong>HBs</strong><br />
antibodies. Several dilutions of the sample will be analyzed, as required, to take into account<br />
variations in <strong>HBs</strong> <strong>Ag</strong> concentration.<br />
The test includes the following reactional phases :<br />
1) Incubation of controls and samples positive in <strong>HBs</strong> <strong>Ag</strong> with the neutralization reagent and the<br />
conjugate in the presence of anti-<strong>HBs</strong> antibodies fixed on the solid phase.<br />
For each sample, control wells are provided by replacing the neutralization reagent with a negative<br />
control diluent.<br />
The remainder of the procedure is identical to the protocol for the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit :<br />
2) Washing, then development of the enzyme activity bound to the solid phase by addition of the<br />
substrate.<br />
3) Stop of the development step, then reading of the optical densities at 450/620-700 nm and<br />
interpreting the results.<br />
3 - COMPOSITION OF THE KIT<br />

4 - PRECAUTIONS<br />
The quality of results is dependent upon the following good laboratory practices:<br />
• Do not use expired reagents.<br />
• Do not mix or combine reagents from MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> kits with<br />
different batch numbers during the same procedure.<br />
• Before use, allow the reagents to stabilize at ambient temperature (18-30°C) for 30 minutes.<br />
• Use glassware that is thoroughly washed and rinsed in distilled water or, preferably, use disposable<br />
material.<br />
• Refer to MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit package insert.<br />
5 - HEALTH AND SAFETY INSTRUCTIONS<br />
• All of the reagents included in the kit are intended for in vitro diagnostic use.<br />
• The material of human origin used for the preparation of reagents RA and RB has been tested and<br />
found negative in <strong>HBs</strong> antigen and in anti-HIV1, anti-HIV2 and anti-HCV antibodies. Since no<br />
method can absolutely guarantee the absence of HIV, HBV or HCV viruses or other infectious<br />
agents, consider these reagents, as well as patient samples, as potentially infections and handle<br />
them with precaution.<br />
• For common health and safety instructions, refer to the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit package<br />
insert.<br />
Some reagents contain ProClin 300 (0.04%, 0.1% and/or 0.5%)<br />

6 - MATERIAL REQUIRED BUT NOT PROVIDED<br />
Refer to the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit package insert.<br />
7 - SHELF LIFE - STABILITY<br />
• Store the kit between +2°C and +8°C before use.<br />
• Once opened, all of the reagents can be used up to the expiry date indicated on the box if they are<br />
stored between +2°C and +8°C.<br />
8 - SAMPLES<br />
• Refer to the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit package insert.<br />
• Samples with a very high concentration of <strong>HBs</strong> <strong>Ag</strong> can not be neutralized by the neutralization<br />
regent if they are tested in undiluted form.<br />
Refer to section 10 “CALCULATION AND INTERPRETATION OF RESULTS”, sub-section “Test<br />
validation criteria”.<br />
NOTE : samples with a high <strong>HBs</strong> <strong>Ag</strong> concentration, with an optical density greater than 3.000 with<br />
the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> test, can be tested directly when diluted to 1/100 in the sample<br />
diluent.<br />
9 - ASSAY PROCEDURE<br />
Proceed as follows using the reagents from the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit.<br />
1. Prepare the washing solution (R2) and the reconstituted conjugate solution (R6+R7) : refer to<br />
section 8 “PREPARATION OF REAGENTS”, sub-section “Reagents to be reconstituted” in the<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit package insert.<br />
2. Remove the microplate frame and the number of required strips (reagent R1) from the protective<br />
packing.<br />
3. Distribute in the wells in the following order :<br />
• Wells A1, A2 : 100 µl of the negative control (R3) from the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit.<br />
• Wells B1, B2 : 100 µl of the positive control (R4) from the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit.<br />
• Wells C1, C2, D1, D2 : 100 µl of the first sample to be confirmed.<br />
• Wells E1, E2, F1, F2 : 100 µl of the second sample to be confirmed.<br />

• Wells G1, G2, H1, H2 : 100 µl of the third sample to be confirmed, and so forth.<br />
Depending on the used system, it is possible to modify the position of controls or the order of<br />
distribution.<br />
4. Add 20 µl of the negative control diluent (RB) in wells A1, B1, C1, D1, E1, F1, G1, H1 and 20 µl of<br />
the neutralization reagent (RA) in wells A2, B2, C2, D2, E2, F2, G2, H2, etc.<br />
5. Distribute 50 µl of the reconstituted conjugate solution (R6+R7) in all the wells. Homogenize each<br />
mixture using a minimum of 3 aspirations.<br />
6. Follow with step 7 and all subsequent steps contained in section 11 “PROCEDURE” in the<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit package insert.<br />
10 - CALCULATION AND INTERPRETATION OF RESULTS<br />
1) Reactivity ratio of the controls and of the samples<br />
1. For the positive control (R4) and the negative control (R3) from the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> kit,<br />
calculate :<br />

3) Test validation criteria<br />
1. The values of the negative control (R3) with addition of negative control diluent (RB) or<br />
neutralization reagent (RA) must be less than or equal to 0.080 OD units.<br />
For this same negative control (R3), the reactivity ratio must be less than 0.8.<br />
For the positive control (R4) with addition of negative control diluent (RB), the recorded optical<br />
density must be greater than 0.800.<br />
For this same positive control (R4), the inhibition percentage must be greater than or equal to 50%.<br />
2. For samples with an inhibition percentage < 50% and with a reactivity ratio ≥ 0.8, it is necessary<br />
to re-test these samples diluted in the sample diluent (1/100; 1/10,000; 1/1,000,000; etc., 100 X<br />
dilution) in order to obtain a reactivity ratio < 0.8 (sample not confirmed in this case) or confirmation<br />
of the neutralization of the sample tested (inhibition percentage ≥ 50%).<br />
4) Interpretation of results<br />
A sample will be confirmed positive for <strong>HBs</strong> <strong>Ag</strong> if the reactivity ratio is greater than or equal to 0.8<br />
and the inhibition percentage is greater than or equal to 50%.<br />

11 - PERFORMANCES<br />
The performances of MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> assay associated with<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> has been determined by testing samples from clinical patients with<br />
acute and chronic hepatitis B infection or with diseases unrelated to hepatitis B infection, and from<br />
commercial samples and seroconversion panels.<br />
More over the sensitivity limit has been tested using the French EFS HB panel, and the WHO<br />
standards (NIBSC code 00/588).<br />
Analytical Sensitivity<br />
All diluted sample concentrations from 0.06 ng/ml to 2.2 ng/ml from the EFS HB sensitivity panel and<br />
all positive results from WHO NIBSC panel were all confirmed with MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong> assay.<br />
An <strong>HBs</strong> <strong>Ag</strong> concentration higher than 10 mg/ml was neutralised after dilution of this sample.<br />
Sensitivity<br />
Sensitivity studies have been performed on 337 positive samples from follow-up of chronic or acute<br />
HBV infected. All the positive samples were confirmed with MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong> assay.<br />
A total of 15 well documented commercial HBV seroconversion panels (45 samples) were also<br />
studied. All the positive samples were neutralised.<br />
Specificity and Cross Reactivity<br />
An internal panel of 16 false positive samples with MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> was tested with<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> assay and were not confirmed.<br />
The analysis of 102 patients showing different pathologies or status not linked to the hepatitis B<br />
(pregnant women, rheumatoid factor, anti-nuclear antibodies, anti-mouse Ig or other viral or bacterial<br />
infections) were tested with MONOLISA ® <strong>Ag</strong> <strong>HBs</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> and confirmed negative .<br />
More over, 37 samples positive with <strong>HBs</strong> <strong>Ag</strong> and also positive with different markers (Rheumatoid<br />
factors samples, antinuclear antibody samples, anti mouse Ig or viral infection samples) were tested<br />
with MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> assay; all these samples were confirmed positive<br />
for <strong>HBs</strong> <strong>Ag</strong>.<br />
23 <strong>HBs</strong> <strong>Ag</strong> positive samples from the co infection Panel PCA201 BBI (with HBV, HIV, HCV and /or<br />
HTLV) were all confirmed with MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> assay pure or after<br />
dilution.<br />

Assay Reproducibility<br />
The reproducibility of MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> assay has been determined, by<br />
the analysis of 4 samples : 1 negative sample (sample 1), 2 <strong>HBs</strong> <strong>Ag</strong> positive samples (samples 2 and<br />
3) and 1 high <strong>HBs</strong> <strong>Ag</strong> positive sample (sample 4).<br />
The intra assay reproducibility has been evaluated by testing these 4 samples 30 times in the same<br />
run.<br />
The inter assay reproducibility has been evaluated by testing these 4 samples in duplicate during 20<br />
days on 2 independent runs each days.<br />

The results obtained during these reproducibility studies were conformed to those expected.<br />
12 - LIMITS OF THE TEST<br />
The MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> test is strictly limited to the confirmation of the<br />
presence of the hepatitis B surface antigen (<strong>HBs</strong> <strong>Ag</strong>) in human serum and plasma.<br />
Samples which repeatedly test weakly positive (ratio less than 2) with the MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> test and which are determined not reactive for an undiluted sample with the MONOLISA ® <strong>HBs</strong><br />
<strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> test (reactivity ratio less than 0.8) must be interpreted with precaution.<br />
It is recommended to re-test these patients with another method or with a second sample taken later.<br />

Le test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> est utilisé pour confirmer la présence de l’<strong>Ag</strong><br />
de surface du virus de l’hépatite B (<strong>Ag</strong> <strong>HBs</strong>) dans les échantillons sériques ou plasmatiques humains<br />
trouvés réactifs par le test de screening <strong>MONOLISA®</strong> <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> codes 72346/72348.<br />
2 - PRINCIPE DU TEST MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong><br />
Le test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> utilise le principe de la neutralisation par un<br />
excès d'anticorps anti-<strong>HBs</strong> (diluant anti-<strong>HBs</strong>; réactif de neutralisation) de l'<strong>Ag</strong> <strong>HBs</strong> présent dans les<br />
échantillons sériques ou plasmatiques.<br />
Il est conseillé de tester les échantillons trouvés positifs répétables en test de screening MONOLISA ®<br />

saturent les déterminants antigéniques <strong>HBs</strong> de l’échantillon qui ne pourront plus se lier à l'anticorps<br />
immobilisé sur la phase solide. Une réduction de la densité optique sera observée lors de la<br />
comparaison avec le même échantillon où le réactif de neutralisation est remplacé par un contrôle<br />
diluant négatif ne contenant pas d'anticorps anti-<strong>HBs</strong>. Plusieurs dilutions de l'échantillon seront<br />
éventuellement contrôlées pour prendre en compte les variations de concentration en <strong>Ag</strong> <strong>HBs</strong> de<br />
l’échantillon.<br />
L'essai comprend les étapes réactionnelles suivantes :<br />
1) Incubation des contrôles ou des échantillons positifs en <strong>Ag</strong> <strong>HBs</strong> avec le réactif de neutralisation et<br />
le conjugué en présence d'anticorps anti-<strong>HBs</strong> immobilisés sur la phase solide.<br />
Pour chaque échantillon, des cupules témoins sont réalisées en remplaçant le réactif de<br />
neutralisation par un contrôle diluant négatif.<br />
La suite de la procédure est identique au protocole de la trousse MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> :<br />
2) Lavage puis révélation de l'activité enzymatique liée à la phase solide par addition du substrat.<br />
3) Arrêt de la révélation puis lecture des densités optiques à 450/620-700 nm et interprétation des<br />
résultats.<br />

4 - PRÉCAUTIONS<br />
La qualité des résultats dépend du respect des bonnes pratiques de laboratoire suivantes :<br />
• Ne pas utiliser des réactifs dont la validité est expirée.<br />
• Ne pas mélanger, ni associer des réactifs provenant de trousses MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong> portant des numéros de lots différents au cours d'une même manipulation.<br />
• Avant utilisation, attendre 30 minutes que les réactifs s'équilibrent à la température ambiante<br />
(18-30°C).<br />
• Utiliser une verrerie parfaitement lavée et rincée à l'eau distillée ou de préférence du matériel à<br />
usage unique.<br />
• Se reporter également à la notice de la trousse <strong>MONOLISA®</strong> <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
5 - CONSIGNES D'HYGIÈNE ET DE SÉCURITÉ<br />
• Tous les réactifs de la trousse sont destinés à l'usage du diagnostic in vitro .<br />
• Le matériel d'origine humaine utilisé pour la préparation des réactifs RA et RB a été testé et trouvé<br />
négatif en antigène <strong>HBs</strong>, en anticorps anti-VIH1, anti-VIH 2 et anti-VHC. Du fait qu’aucune<br />
méthode ne peut garantir de façon absolue l'absence des virus VIH, VHB, VHC ou d'autres agents<br />
infectieux, considérer ces réactifs, ainsi que les échantillons de patients, comme potentiellement<br />
infectieux et les manipuler avec les précautions d'usage.<br />

7 - CONSERVATION - VALIDITÉ<br />

coffret, si conservés à 2-8°C.<br />
8 - ÉCHANTILLONS<br />
• Se reporter à la notice de la trousse MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Certains échantillons ayant une concentration très élevée en <strong>Ag</strong> <strong>HBs</strong> peuvent ne pas être<br />
neutralisés par le réactif de neutralisation s'ils sont testés non dilués.<br />

NB : les échantillons ayant une concentration très élevée en <strong>Ag</strong> <strong>HBs</strong>, densité optique supérieure à<br />
3.000 avec le test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> Ultra, peuvent être testés directement dilués au 1/100 dans<br />
le diluant échantillon.<br />
9 - MODE OPÉRATOIRE<br />
Procéder de la façon suivante en utilisant les réactifs de la trousse MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
1. Préparer la solution de lavage R2 et la solution de conjugué reconstituée (R6+R7) : se reporter au<br />
chapitre 8 «PREPARATION DES REACTIFS», sous-chapitre «réactifs à reconstituer» de la notice de<br />
la trousse MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
2. Sortir de l'emballage protecteur le cadre support et le nombre de barrettes nécessaires (Réactif<br />
R1).<br />
3. Distribuer dans les cupules dans l'ordre suivant :<br />
• Cupules A1, A2 : 100 µl de contrôle négatif R3 de la trousse MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Cupules B1, B2 : 100 µl de contrôle positif R4 de la trousse MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Cupules C1, C2, D1, D2 : 100 µl du premier échantillon à confirmer.<br />

• Cupules E1, E2, F1, F2 : 100 µl du deuxième échantillon à confirmer<br />
• Cupules G1, G2, H1, H2 : 100 µl du troisième échantillon à confirmer, etc…<br />
En fonction du système utilisé, il est possible de modifier la position ou l’ordre de distribution des<br />
témoins.<br />
4. Rajouter 20 µl de contrôle diluant négatif RB dans les cupules A1, B1, C1, D1, E1, F1, G1, H1 et<br />
20 µl de réactif de neutralisation RA pour les cupules A2, B2, C2, D2, E2, F2, G2, H2 etc...<br />
5. Distribuer 50 µl de la solution de conjugué reconstitué (R6 + R7) dans toutes les cupules.<br />
Homogénéiser par 3 aspirations/refoulements au minimum.<br />
6. Poursuivre avec l’étape 7 et les étapes suivantes du chapitre 11 «MODE OPERATOIRE» de la<br />
notice de la trousse MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
10 - CALCUL ET INTERPRÉTATION DES RÉSULTATS<br />
1) Ratio de réactivité des contrôles et des échantillons<br />
1. Pour les contrôles positif R4 et négatif R3 de la trousse MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>, calculer :<br />

3) Conditions de validation du test<br />
1. Les valeurs du contrôle négatif (R3) additionné de contrôle diluant négatif (RB) ou de réactif de<br />
neutralisation (RA) doivent être inférieures ou égales à 0,080 unité de densité optique.<br />
Pour ce même contrôle négatif (R3), le ratio de réactivité doit être inférieur à 0.8<br />
Pour le contrôle positif (R4) additionné de contrôle diluant négatif (RB), la densité optique<br />
enregistrée doit être supérieure à 0.800.<br />
Pour ce même contrôle positif (R4), le pourcentage d’Inhibition doit être supérieur ou égal à 50%.<br />
2. Pour les échantillons avec un pourcentage d’Inhibition < 50% et avec un ratio de réactivité ≥ 0.8,<br />
il est nécessaire de retester cet échantillon dilué dans le diluant échantillon (1/100, 1/10 000,<br />
1/1 000 000, dilution de facteur 100) pour obtenir un ratio de réactivité < 0,8 (échantillon non<br />
confirmé dans ce cas) ou la confirmation de la neutralisation de l'échantillon testé (Pourcentage<br />
d’Inhibition ≥ 50%).<br />
4) Interprétation des résultats<br />
Un échantillon sera confirmé positif pour l'<strong>Ag</strong> <strong>HBs</strong> si le ratio de réactivité est supérieur ou égal à 0.8<br />
et le pourcentage d’Inhibition est supérieur ou égal à 50%.<br />

11 - PERFORMANCES<br />
Les performances du test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> associé au test<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> ont été déterminées à l’aide d’échantillons de patients atteints<br />
d’Hépatite B aiguë et chronique ou atteints d’infections sans relation avec l’Hépatite B ainsi que sur<br />
des échantillons provenant de panels commerciaux et de séroconversions commerciales.<br />
La limite de sensibilité a été étudiée à l’aide du panel HB EFS français ainsi que du second étalon<br />
international OMS 2004 ref (NIBSC 00/588)<br />
Sensibilité analytique<br />
Toutes les concentrations du panel HB EFS de 0.06 ng/ml à 2.2ng /ml ainsi que les concentrations<br />
du standard OMS de 0.031 UI /ml à 1 UI /ml trouvées positives avec le test Monolisa <strong>HBs</strong> <strong>Ag</strong> Ultra<br />
ont été confirmées à l’aide du test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>.<br />
Une concentration supérieure à 10 mg / ml d’ <strong>Ag</strong> <strong>HBs</strong> a été neutralisée après dilution de cet<br />
échantillon .<br />
Sensibilité<br />
L’étude de sensibilité a été réalisée sur 337 échantillons positifs provenant de suivis d’Hépatite B<br />
chronique ou aiguë de patients infectés.<br />
Tous les échantillons positifs ont été confirmés à l’aide du test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>.<br />
15 séroconversions VHB commerciales bien documentées (45 échantillons) ont été étudiées : tous<br />
les échantillons positifs ont été neutralisés.<br />
Spécificité et Réactions Croisées<br />
Un panel interne de 16 échantillons trouvés faux positifs avec le test Monolisa <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> ont<br />
été testés et trouvés non confirmés avec le test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>.<br />
102 échantillons provenant de patients avec différentes pathologies ou statut immunitaire sans<br />
relation avec l’Hépatite B (femmes enceintes, facteurs rhumatoïdes, anticorps anti nucléaires,<br />
anticorps anti souris ou autres infections virales et bactériennes) ont été testés avec le test<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> et sont confirmés négatifs.<br />
De plus 37 échantillons positifs à la fois en <strong>Ag</strong> <strong>HBs</strong> et pour d’autres marqueurs (facteurs<br />
rhumatoïdes, anticorps anti nucléaires, anticorps anti souris ou autres infections virales et<br />
bactériennes) ont été testés avec le test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> et confirmés<br />
avec celui ci.<br />

De plus, 23 échantillons provenant du panel de coinfection BBI PCA 201 ( coinfections VHB/VIH/<br />
VHC /VHTL) positifs en Monolisa HBS <strong>Ag</strong> Ultra ont été confirmés purs ou après dilution.<br />
Etudes de Reproductibilité<br />
La reproductibilité du test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> a été déterminée à l’aide<br />
de 4 échantillons : 1 échantillon négatif (échantillon 1), 2 échantillons positifs (échantillons 2 et 3 ) et<br />
1 échantillon positif fort (échantillon 4).<br />
L’étude intra essai a été réalisée en testant ces 4 échantillons 30 fois lors du même essai ;<br />
L’étude inter essai a été réalisée en testant ces 4 échantillons en duplicats à raison de 2 essais<br />
différents par jour et pendant une période de 20 jours .<br />
Les résultats exprimés en moyenne, écart type et CV sont présentés dans les tableaux suivants<br />

12 - LIMITES DU TEST<br />
Le test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> est strictement limité à la confirmation de la<br />
présence de l’antigène de surface du virus de l’hépatite B (<strong>Ag</strong> <strong>HBs</strong>) dans le sérum et le plasma<br />
humain.<br />
Les échantillons réactifs répétables faiblement positifs (ratio inférieur à 2) avec le test MONOLISA ®<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> et obtenus non réactifs sur l'échantillon non dilué dans le test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> <strong>CONFIRMATORY</strong> (ratio de réactivité inférieur à 0.8), doivent être interprétés avec précaution.<br />
Il est conseillé de retester ce patient avec une autre méthode ou avec un deuxième échantillon<br />
prélevé ultérieurement.<br />

1 - OBJETIVO DEL TEST<br />
La prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> se emplea para confirmar la presencia del<br />
antígeno de superficie del virus de la hepatitis B (<strong>Ag</strong> <strong>HBs</strong>) en las muestras séricas o plasmáticas<br />
humanas que hayan resultado positivas en la prueba de screening MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
códigos 72346/72348.<br />
2 - PRINCIPIO DE LA PRUEBA MONOLISA ® HBS AG <strong>ULTRA</strong> <strong>CONFIRMATORY</strong><br />
La prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> utiliza el principio de la neutralización por<br />
exceso de anticuerpos anti-<strong>HBs</strong> (diluyente anti<strong>HBs</strong>, reactivo de neutralización) del <strong>Ag</strong> <strong>HBs</strong> presente<br />
en las muestras séricas o plasmáticas.<br />
Se aconseja realizar la prueba a las muestras que hayan dado positivo dudoso en la prueba de<br />
screening MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> con los reactivos del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>. En el caso de una muestra positiva en antígeno <strong>HBs</strong>, los anticuerpos anti-<strong>HBs</strong> del<br />
reactivo de neutralización saturan los determinantes antigénicos <strong>HBs</strong> de la muestra, que no se<br />
podrán unir al anticuerpo inmovilizado en la fase sólida. Se observará una reducción de la densidad<br />
óptica al compararlo con la misma muestra en la que se ha sustituido el reactivo de neutralización<br />
por un control diluyente negativo que no contenga anticuerpos anti-<strong>HBs</strong>. Se controlarán<br />
eventualmente varias diluciones de la muestra para tomar en cuenta las variaciones de<br />
concentración en <strong>Ag</strong> <strong>HBs</strong> de la muestra.<br />
El ensayo incluye las siguientes etapas reactivas:<br />
1) Incubación de los controles o de las muestras positivas en <strong>Ag</strong> <strong>HBs</strong> con el reactivo de<br />
neutralización y el conjugado en presencia de anticuerpos anti<strong>HBs</strong> inmovilizados en la fase sólida.<br />
Por cada muestra, los pocillos testigo se realizan sustituyendo el reactivo de neutralización por un<br />
control diluyente negativo.<br />
El procedimiento continúa de la misma forma que el protocolo del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> :<br />
2) Lavado y posterior revelado de la actividad enzimática relacionada con la fase sólida por adición<br />
del sustrato.<br />
3) Parada del revelado y posterior lectura de las densidades ópticas a 450/620-700 mm e<br />
interpretación de los resultados.<br />

4 - PRECAUCIONES<br />
La calidad de los resultados depende del seguimiento de las siguientes buenas prácticas de<br />
laboratorio :<br />
• No utilizar reactivos caducados.<br />
• No mezclar ni asociar durante una misma manipulación reactivos procedentes de equipos<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> con números de lote diferentes.<br />
• Antes de su utilización, esperar 30 minutos para que los reactivos alcancen la temperatura<br />
ambiente (18-30ºC).<br />
• Utilizar instrumental de cristal perfectamente lavado y aclarado con agua destilada, o,<br />
preferiblemente, material de un solo uso.<br />
• Consultar también las instrucciones del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
5 - NORMAS DE HIGIENE Y SEGURIDAD<br />
• Todos los reactivos del equipo están destinados al uso diagnóstico in vitro.<br />
• El material de origen humano utilizado para la preparación de los reactivos RA y RB ha sido<br />
sometido a pruebas, resultando negativo en antígeno <strong>HBs</strong> y en anticuerpos anti-VIH1, anti-VIH2 y<br />
anti-VHC. Puesto que ningún método puede garantizar completamente la ausencia de los virus<br />
VIH, VHB, VHC o de otros agentes infecciosos, estos reactivos, así como las muestras de los<br />
pacientes, deberán considerarse como potencialmente infecciosos y deberán manipularse con<br />
precaución.<br />
• Para las normas de higiene y seguridad habituales, consultar las instrucciones del equipo<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
Algunos reactivos contienen ProClin 300 (0,04%, 0,1% y/o 0,5%)<br />

6 - MATERIAL NECESARIO NO INCLUIDO<br />
Consultar las instrucciones del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
7 - CONSERVACIÓN Y CADUCIDAD<br />
• Conservar el equipo a + 2-8ºC antes de su utilización.<br />
• Una vez abierto, todos los reactivos pueden utilizarse hasta la fecha de caducidad indicada en el<br />
estuche si se han conservado a 2-8ºC.<br />
8 - MUESTRAS<br />
• Consultar las instrucciones del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Es posible que algunas muestras con una concentración muy elevada de <strong>Ag</strong> <strong>HBs</strong> no puedan ser<br />
neutralizadas por el reactivo de neutralización si no se diluyen antes.<br />
Consultar el capítulo 10 “CÁLCULO E INTERPRETACIÓN DE LOS RESULTADOS”, sección:<br />
“Condiciones de validación de la prueba”.<br />
NOTA : A las muestras con una concentración muy elevada de <strong>Ag</strong> <strong>HBs</strong> y densidad óptica superior a<br />
3.000 con la prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> Ultra se les puede hacer la prueba diluidas directamente<br />
al 1/100 en el diluyente para muestras.<br />
9 - PROCEDIMIENTO<br />
Proceder de la siguiente forma cuando se utilicen los reactivos del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong>.<br />
1. Preparar la solución de lavado R2 y la solución de conjugado reconstituido (R6+R7): consultar el<br />
capítulo 8 “PREPARACIÓN DE LOS REACTIVOS”, sección “Reactivos que se deben reconstituir”<br />
de las instrucciones del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
2. Sacar del envoltorio protector el bastidor de soporte y el número de unidades necesarias (reactivo<br />
R1).<br />
3. Distribuir en los pocillos en el orden siguiente:<br />
• Pocillos A1 y A2: 100 µl de control negativo R3 del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />

• Pocillos B1 y B2: 100 µl de control positivo R4 del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Pocillos C1, C2, D1 y D2: 100 µl de la primera muestra que se desea confirmar.<br />
• Pocillos E1, E2, F1 y F2: 100 µl de la segunda muestra que se desea confirmar.<br />
• Pocillos G1, G2, H1 y H2: 100 µl de la tercera muestra que se desea confirmar, etc.<br />
En función del sistema utilizado, se puede modificar la posición o el orden de distribución de los<br />
controles.<br />
4. Añadir 20 µl de control diluyente negativo RB en los pocillos A1, B1, C1, D1, E1, F1, G1, H1 y 20 µl<br />
de reactivo de neutralización RA en los pocillos A2, B2, C2, D2, E2, F2, G2, H2, etc.<br />
5. Distribuir 50 µl de la solución de conjugado reconstituido (R6+R7) en todos los pocillos.<br />
Homogeneizar mediante 3 aspiraciones/dispensaciones como mínimo.<br />
6. Proseguir con la etapa 7 y siguientes del capítulo 11 “PROCEDIMIENTO” de las instrucciones del<br />
equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
10 - CÁLCULO E INTERPRETACIÓN DE LOS RESULTADOS<br />
1) Relación entre reactividad de los controles y de las muestras<br />
1. Para los controles positivo R4 y negativo R3 del equipo MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>, calcular :<br />

3) Condiciones de validación de la prueba<br />
1. Los valores del control negativo (R3) al que se ha añadido control diluyente negativo (RB) o<br />
reactivo de neutralización (RA) deben ser iguales o inferiores a 0,080 unidades de densidad óptica.<br />
Para el propio control negativo (R3), la relación de reactividad debe ser inferior a 0,8.<br />
Para el control positivo (R4) al que se ha añadido control diluyente negativo (RB) la densidad óptica<br />
registrada debe ser superior a 0,800.<br />
Para el propio control positivo (R4), el porcentaje de inhibición debe ser igual o superior al 50%.<br />
2. Para las muestras con un porcentaje de inhibición
4) Interpretación de los resultados<br />
Una muestra se confirmará como positiva en <strong>Ag</strong> <strong>HBs</strong> si la relación de reactividad es igual o superior<br />
a 0,8 y el porcentaje de inhibición es igual o superior al 50%.<br />

11 - RENDIMIENTOS<br />
Los rendimientos de la prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> asociada a la prueba<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> se determinaron mediante muestras de pacientes afectados por<br />
hepatitis B aguda y crónica o afectados por infecciones sin relación con la hepatitis B, así como a<br />
partir de muestras procedentes de paneles comerciales y de seroconversiones comerciales.<br />
El límite de sensibilidad fue estudiado con ayuda del panel HB EFS francés, así como del segundo<br />
estándar internacional OMS 2004 (ref. NIBSC 00/588).<br />
Sensibilidad analítica<br />
Todas las concentraciones del panel HB EFS de 0,06 ng/ml a 2,2 ng/ml, así como las<br />
concentraciones del estándar OMS de 0,031 UI/ml a 1 UI/ml que resultaron positivas con la prueba<br />
MONOLISA <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> fueron confirmadas con ayuda de la prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>.<br />
Una concentración superior a 10 mg/ml de <strong>Ag</strong> <strong>HBs</strong> se neutralizó tras la dilución de esta muestra.<br />
Sensibilidad<br />
El estudio de sensibilidad se realizó sobre 337 muestras positivas procedentes de seguimientos de<br />
hepatitis B crónica o aguda de pacientes infectados.<br />
Todas las muestras positivas fueron confirmadas con ayuda de la prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> <strong>CONFIRMATORY</strong>.<br />
Se estudiaron 15 seroconversiones VHB comerciales bien documentadas (45 muestras). Todas las<br />
muestras positivas fueron neutralizadas.<br />

Especificidad y reacciones cruzadas<br />
Un panel interno de 16 muestras que dieron falsos positivos con la prueba MONOLISA <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> fue sometido a pruebas y resultaron no confirmadas con la prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> <strong>CONFIRMATORY</strong>.<br />
Se sometieron a la prueba MONOLISAv <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> 102 muestras procedentes<br />
de pacientes con diferentes patologías o estado inmunitario sin relación con la hepatitis B (mujeres<br />
embarazadas, factores reumatoides, anticuerpos antinucleares, anticuerpos antirratón u otras<br />
infecciones virales y bacterianas); confirmándose todas ellas como negativas.<br />
Además se sometieron a la prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> 37 muestras<br />
positivas a la vez en <strong>Ag</strong> <strong>HBs</strong> y en otros marcadores (factores reumatoides, anticuerpos antinucleares,<br />
anticuerpos antirratón u otras infecciones virales y bacterianas), que fueron confirmadas con esta<br />
prueba.<br />
También se confirmaron como positivas con MONOLISA <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> 23 muestras, puras o después<br />
de dilución, procedentes del panel de coinfección BBI PCA 201 (coinfecciones VHB/VIH/VHC/VHTL).<br />
Estudios de reproductibilidad<br />
La reproductibilidad de la prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> quedó determinada<br />
con 4 muestras: 1 muestra negativa (muestra 1), 2 muestras positivas (muestras 2 y 3) y 1 muestra<br />
positiva fuerte (muestra 4).<br />
El estudio intraensayo se realizó sometiendo estas 4 muestras a la prueba 30 veces durante el<br />
mismo ensayo.<br />
El estudio intraensayo se realizó sometiendo estas 4 muestras por duplicado a la prueba, a razón de<br />
2 ensayos diferentes al día durante un periodo de 20 días.<br />
Los resultados expresados en media, desviación típica y CV se presentan en las tablas siguientes :<br />

Los resultados obtenidos durante estos estudios se corresponden con los que se esperaban.<br />
12 - LÍMITES DE LA PRUEBA<br />
La prueba MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> está limitada exclusivamente a la<br />
confirmación de la presencia del antígeno de superficie del virus de la hepatitis B (<strong>Ag</strong> <strong>HBs</strong>) en el<br />
suero y el plasma humano.<br />
Las muestras reactivas dudosas ligeramente positivas (relación inferior a 2) con la prueba<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> y que hayan resultado no reactivas en la muestra no diluida de la prueba<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> (relación de reactividad inferior a 0,8), deben<br />
interpretarse con precaución. Se aconseja volver a realizar la prueba al paciente con otro método o<br />
con una segunda muestra extraída posteriormente.<br />

1. VERWENDUNGSZWECK<br />
Der MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT dient der Bestätigung des Vorhandenseins<br />
von Hepatitis-B-Oberflächenantigen (<strong>HBs</strong> <strong>Ag</strong>) in Humanserum- oder Humanplasma-Proben, die im<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> Screening-Test reaktiv waren (Bestellnummer 72346/72348).<br />
2 - PRINZIP DES MONOLISA ® HBS AG <strong>ULTRA</strong> BESTÄTIGUNGSTESTS<br />
Der MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT verwendet das Prinzip der Neutralisation<br />
durch überschüssige <strong>HBs</strong>-Antikörper (Anti-<strong>HBs</strong>-Verdünnungsmittel: Neutralisationsreagenz) von in<br />
Plasma- und Serumproben vorhandenem <strong>HBs</strong> <strong>Ag</strong>.<br />
Es empfiehlt sich, das Ergebnis von Proben, die im MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> Screeningtest<br />
wiederholt reaktiv waren, unter Verwendung der Reagenzien aus dem MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>-KIT zu bestätigen. Wenn eine Probe <strong>HBs</strong>-Antigen enthält, sättigen die Anti-<strong>HBs</strong>-<br />
Antikörper im Neutralisationsreagenz die in der Probe vorhandenen <strong>HBs</strong>-Antigendeterminanten und<br />
verhindern somit deren Bindung an die Antikörper auf der Festphase. Bei dem Vergleich der selben<br />
reaktiven Probe, bei der das Neutralisationsreagenz durch eine Negativkontrolle ohne <strong>HBs</strong>-<br />
Antikörper ersetzt wurde, wird eine Abnahme der optischen Dichte beobachtet. Entsprechend der<br />
unterschiedlich hohen <strong>HBs</strong> <strong>Ag</strong>-Konzentration kann es erforderlich sein, die Probe mehrfach zu<br />
verdünnen.<br />
Das Testverfahren besteht aus folgenden Reaktionsschritten :<br />
1) Inkubation des Kontrollserums und der <strong>HBs</strong>-<strong>Ag</strong>-positiven Proben mit Neutralisationsreagenz und<br />
Konjugat in Anwesenheit von Anti-<strong>HBs</strong>-Antikörpern auf der Festphase.<br />
Für jedes Serum werden durch Ersetzen des Neutralisationsreagenzes durch eine Negativkontrolle<br />
Kontrollvertiefungen mitgetestet.<br />
Die übrigen Testschritte sind identisch zu denen im Protokoll des MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> Kits :<br />
2) Waschzyklus und anschließende Entwicklung der an die Festphase gebundenen Enzymaktivität<br />
durch Zugabe von Substrat.<br />
3) Stoppen der Reaktion, anschließend Ablesen der optischen Dichte bei 450/620-700 nm und<br />
Ergebnisauswertung.<br />

4 - VORSICHTSMASSNAHMEN<br />
Die Qualität der Ergebnisse hängt von der Einhaltung folgender GLP-Richtlinien ab :<br />
• Die Reagenzien nicht über das Verfallsdatum hinaus verwenden.<br />
• Keine Reagenzien aus dem MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT mit Kits anderer<br />
Chargen innerhalb einer Messreihe vermischen oder miteinander verwenden.<br />
• Vor der Verwendung 30 Minuten abwarten, bis die Reagenzien Raumtemperatur (18-30°C) erreicht<br />
haben.<br />
• Sorgfältig gewaschene und mit destilliertem Wasser gespülte Glasgefäße oder vorzugsweise<br />
Einwegmaterial verwenden.<br />
• Die Hinweise in der Packungsbeilage des Kits MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> beachten.<br />
5 - SICHERHEITSVORSCHRIFTEN<br />
• Alle Reagenzien in diesem Kit sind für die In-vitro-Diagnostik bestimmt.<br />
• Das Material menschlichen Ursprungs, das zur Herstellung der Reagenzien RA und RB verwendet<br />
wurde, ist mit negativem Ergebnis auf <strong>HBs</strong>-Antigen und auf Antikörper gegen HIV1, HIV2 und HCV<br />
getestet worden. Da keine Testmethode mit absoluter Sicherheit ausschließen kann, dass HIV-,<br />
Hepatitis B- oder C-Viren oder andere infektiöse Erreger vorhanden sind, sollten alle Reagenzien<br />
und Patientenproben als potentiell infektiös angesehen und mit entsprechender Sorgfalt<br />
gehandhabt werden.<br />
• Allgemein gültige Sicherheitshinweise sind der Packungsbeilage des MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
Kits zu entnehmen.<br />

7 - HALTBARKEIT – STABILITÄT<br />
• Lagerung des Kits vor der Verwendung bei 2 - 8°C.<br />
• Nach dem Öffnen können alle Reagenzien bis zu dem auf der Schachtel aufgedruckten<br />
Verfalldatum verwendet werden, wenn sie bei +2-8°C gelagert werden.<br />
8 - PROBEN<br />
• Die Hinweise in der Packungsbeilage des Kits MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> beachten.<br />
• Proben mit einer sehr hohen <strong>HBs</strong>-<strong>Ag</strong>-Konzentration werden womöglich nicht durch das<br />
Neutralisationsreagenz neutralisiert, wenn sie unverdünnt getestet werden.<br />

Anmerkung: Proben mit hoher Konzentration von <strong>HBs</strong> <strong>Ag</strong> mit einer optischen Dichte über 3.000 im<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-Test können direkt getestet werden, wenn sie 1/100 im<br />
Probenverdünnungsmittel verdünnt werden.<br />
9 - TESTVERFAHREN<br />
Zur Testdurchführung die folgenden Schritte beachten und die Reagenzien aus dem MONOLISA ®<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-Kit verwenden.<br />
1. Die Waschlösung (R2) und die rekonstituierte Konjugatlösung (R6 + R7) vorbereiten : Siehe<br />
Kapitel 8 „AUFBEREITUNG DER REAGENZIEN“, Abschnitt „Aufzulösende Reagenzien“ in der<br />
Packungsbeilage des MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-Kit.<br />
2. Den Mikrotiterplattenrahmen und die benötigte Anzahl an Streifen (Reagenz R1) aus der<br />
Schutzpackung nehmen.<br />
3. Die Verteilung in die Vertiefungen erfolgt in dieser Reihenfolge:<br />
• Vertiefung A1, A2 : 100 µl Negativkontrolle R3 aus dem Kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Vertiefung B1, B2 : 100 µl Positivkontrolle R4 aus dem Kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />

• Vertiefungen C1, C2, D1, D2: 100 µl der ersten zu bestätigenden Probe.<br />
• Vertiefungen E1, E2, F1, F2 : 100 µl der zweiten zu bestätigenden Probe.<br />
• Vertiefungen G1, G2, H1, H2 : 100 µl der dritten zu bestätigenden Probe usw.<br />
Ja nachdem, welches System verwendet wird, können die Positionen der Kontrolllösungen oder<br />
die Reihenfolge der Dispensierung verändert werden.<br />
4. 20 µl Negativkontrolle RB in die Vertiefungen A1, B1, C1, D1, E1, F1, G1, H1 und 20 µl<br />
Neutralisationsreagenz RA in die Vertiefungen A2, B2, C2, D2, E2, F2, G2, H2, usw. geben.<br />
5. 50 µl der Konjugatarbeitslösung (R6+R7) in alle Vertiefungen geben. Das Reaktionsgemisch durch<br />
mindestens 3-faches Aufsaugen homogenisieren.<br />
6. Mit Schritt 7 und allen anschließenden Arbeitsschritten in Kapitel 11 „TESTVERFAHREN“ in der<br />
Packungsbeilage des MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> Kit fortfahren.<br />
10 - BERECHNUNG UND INTERPRETATION DER ERGEBNISSE<br />
1) Verhältnis der Reaktivität von Kontrollen und Proben<br />
1. Für die Positivkontrollen (R4) und die Negativkontrollen (R3) aus dem Kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> sind folgende Berechnungen durchzuführen :<br />

3) Kriterien für die Testvalidierung<br />
1. Die Werte der Negativkontrolle (R3) mit Zugabe der Negativkontrolle/Verdünnungsmittel (RB) oder<br />
des Neutralisationsreagenz (RA) dürfen bei höchstens 0,080 OD-Einheiten liegen.<br />
Für dieselbe Negativkontrolle (R3) muss das Verhältnis der Probenreaktivität unter 0.8 betragen.<br />
Die gemessene optische Dichte der Positivkontrolle (R4) mit Zugabe von Negativkontrolle (RB)<br />
sollte über 0,800 liegen.<br />
Für dieselbe Positivkontrolle (R4) muss das Verhältnis der Probenreaktivität mindestens 50%<br />
betragen.<br />
2. Bei Proben mit einer prozentualen Inhibition < 50% und einem Reaktivitätsverhältnis > 0,8 sollten<br />
die mit RB verdünnte Proben (1/100; 1/10.000; 1/1.000.000; etc., 100-fache Verdünnungen) erneut<br />
getestet werden, bis ein Probenreaktivitätsverhältnis < 0,8 erhalten wird (die Probe wurde in<br />
diesem Fall nicht bestätigt) oder bis eine Bestätigung der Neutralisation der getesteten Probe<br />
erreicht wurde (d. h. prozentuale Inhibition ≥ 50%).<br />
4) Interpretation der Ergebnisse<br />
Eine Probe wird als positiv auf <strong>HBs</strong> <strong>Ag</strong> bestätigt, wenn das Probenreaktivitätsverhältnis mindestens<br />
0,8 oder höher und die prozentuale Inhibition mindestens 50% oder höher beträgt.<br />

11 - LEISTUNGSMERKMALE DES TESTS<br />
Die Leistung des MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT in Verbindung mit dem<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> wurden durch Testung der Proben von Klinikpatienten mit akuter oder<br />
chronischer Hepatitis-B-Infektion oder einer Erkrankung, die nicht mit einer Hepatitis-B-Infektion<br />
zusammenhängt, sowie von handelsüblichen Proben und Serokonversionsreihen bestimmt.<br />
Ferner wurde die Empfindlichkeitsgrenze mit dem französischen EFS HB und den WHO-Standards<br />
(NIBSC, Nr. 00/588) getestet.<br />
Analytische Empfindlichkeit<br />
Alle verdünnten Probenkonzentrationen von 0,06-2,2 ng/ml aus der EFS HB-Empfindlichkeitsreihe<br />
und alle positiven Ergebnisse der WHO NIBSC-Reihe wurden mit dem MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>-KIT bestätigt.<br />
Eine <strong>HBs</strong> <strong>Ag</strong>-Konzentration über 10 mg/ml wurde nach Verdünnung der betreffenden Probe<br />
neutralisiert.<br />
Empfindlichkeit<br />
Die Studien zur Empfindlichkeit wurden an 337 Positivproben aus der Nachbeobachtung chronischer<br />
und akuter HBV-Infektionen durchgeführt. Alle diese Positivproben wurden mit dem MONOLISA ®<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT bestätigt.<br />
Außerdem wurden insgesamt 15 handelsübliche, gut dokumentierte HBV-Serokonversionsreihen (45<br />
Proben) untersucht. Alle Positivproben wurden neutralisiert.<br />
Spezifität und Kreuzreaktivität<br />
Eine interne Reihe von 16, mit dem MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> ermittelten falsch positiven Proben<br />
wurde mit dem MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT getestet und nicht bestätigt.<br />
Die Analysen von 102 Patienten mit verschiedenen Erkrankungen oder Zuständen, die nicht mit der<br />
Hepatitis B zusammenhängen (schwangere Frauen, Rheumafaktor, antinukleäre Antikörper, Anti-<br />
Maus-Ig oder andere virale oder bakterielle Infektionen), wurden mit dem MONOLISA ® <strong>Ag</strong> <strong>HBs</strong><br />
<strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT getestet und als negativ bestätigt.<br />
Ferner wurden 37 Positivproben für <strong>HBs</strong> <strong>Ag</strong> und verschiedene Marker (Rheumafaktor, antinukleäre<br />
Antikörper, Anti-Maus-Ig oder virale Infektionen) mit dem MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>-KIT getestet und durchwegs als <strong>HBs</strong> <strong>Ag</strong>-positiv bestätigt.<br />
23 <strong>HBs</strong> <strong>Ag</strong>-positive Proben der Reihe PCA201 BBI mit Co-Infektionen (HBV, HIV, HCV oder HTLV)<br />
wurden vom MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT in unverdünnter und verdünnter<br />
Form bestätigt.<br />

Reproduzierbarkeit des Tests<br />
Die Reproduzierbarkeit des MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT wurde durch Analyse<br />
von 4 Proben bestimmt : 1 Negativprobe (Probe 1), 2 <strong>HBs</strong> <strong>Ag</strong>-positive Proben (Probe 2 und 3) sowie<br />
1 für <strong>HBs</strong> <strong>Ag</strong> hochpositive Probe (Probe 4).<br />
Die Intra-Assay-Reproduzierbarkeit wurde durch Testung dieser 4 Proben dreißigmal in ein und<br />
demselben Testdurchlauf bewertet.<br />
Die Inter-Assay-Reproduzierbarkeit wurde durch Doppeltestung dieser 4 Proben an 20 Tagen in 2<br />
unabhängigen Testdurchläufen pro Tag bewertet.<br />
Die Ergebnisse sind aus den folgenden Tabellen zu entnehmen:<br />

12 - GRENZEN DES TESTS<br />
Das MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-KIT ist ausschließlich für die Bestätigung des<br />
Vorhandenseins des Hepatitis-B-Oberflächenantigens (<strong>HBs</strong> <strong>Ag</strong>) in Humanserum oder -plasma<br />
bestimmt.<br />
Proben, die im MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> Test wiederholt schwach reaktiv getestet wurden<br />
(Verhältnis unter 2) und die als unverdünnte Probe im MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-<br />
KIT nicht reaktiv sind (Reaktivitätsverhältnis unter 0,8) sind mit Vorsicht zu interpretieren. Diese<br />
Patienten sollten mit einer anderen Methode erneut getestet bzw. es sollte zu einem späteren<br />
Zeitpunkt eine weitere Probe getestet werden.<br />

mediante un eccesso di anticorpi anti-<strong>HBs</strong> (diluente anti-<strong>HBs</strong>; reagente di neutralizzazione) dell'<strong>Ag</strong><br />
<strong>HBs</strong> presente nei campioni di siero o di plasma.<br />
Si consiglia di analizzare i campioni risultati positivi ripetibili al test di screening MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> con i reagenti forniti insieme al kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>. In caso di<br />
campione positivo per l'antigene <strong>HBs</strong>, gli anticorpi anti-<strong>HBs</strong> del reagente di neutralizzazione saturano<br />
i determinanti antigenici <strong>HBs</strong> del campione affinché non possano più legarsi all'anticorpo<br />
immobilizzato su fase solida. Durante il confronto con lo stesso campione in cui il reagente di<br />
neutralizzazione è stato sostituito da un controllo negativo (diluente) non contenente anticorpi anti-<br />
<strong>HBs</strong>, si osserverà una riduzione della densità ottica. Al fine di prendere in considerazione le variazioni<br />
di concentrazione dell'<strong>Ag</strong> <strong>HBs</strong> del campione potrà essere necessario controllarne più diluizioni.<br />
Il test comprende le seguenti fasi reattive :<br />
1) Incubazione dei controlli o dei campioni positivi per l'<strong>Ag</strong> <strong>HBs</strong> con il reagente di neutralizzazione e<br />
il coniugato in presenza di anticorpi anti-<strong>HBs</strong> immobilizzati su fase solida.<br />
Per ogni campione vengono realizzati pozzetti di controllo sostituendo il reagente di<br />
neutralizzazione con un controllo negativo (diluente).<br />
Il resto della procedura è identico al protocollo del kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> :<br />
2) Lavaggio, quindi rivelazione dell'attività enzimatica legata alla fase solida mediante l'aggiunta del<br />
substrato.<br />
3) Arresto della rivelazione, lettura delle densità ottiche a 450/620-700 nm e interpretazione dei<br />
risultati.<br />

4 - PRECAUZIONI<br />
L'affidabilità dei risultati dipende dalla corretta implementazione delle buone prassi di laboratorio<br />
riportate di seguito:<br />
• Non utilizzare reagenti scaduti.<br />
• Non mischiare, né combinare reagenti del kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong><br />
provenienti da lotti diversi in uno stesso ciclo.<br />
• Prima dell'uso, attendere 30 minuti per consentire al reagente di raggiungere la temperatura<br />
ambiente (18-30°C).<br />
• Utilizzare preferibilmente materiale monouso oppure elementi in vetro perfettamente lavati e<br />
risciacquati con acqua distillata.<br />
• Consultare anche il foglietto delle istruzioni contenuto nel kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
5 - ISTRUZIONI DI SICUREZZA E IGIENE<br />
• Tutti i reagenti forniti nel kit sono destinati all'uso esclusivo nella diagnostica in vitro.<br />
• Il materiale di origine umana utilizzato nella preparazione dei reagenti è stato analizzato e<br />
classificato negativo per l'antigene <strong>HBs</strong> e per gli anticorpi anti-HIV1, anti-HIV 2 e anti-HCV. Dato<br />
che nessun metodo può garantire con assoluta certezza l'assenza dei virus HIV, HBV, HCV o di altri<br />
agenti infettivi, manipolare detti reagenti e i campioni dei pazienti come potenzialmente infetti<br />
seguendo le apposite precauzioni d’uso.<br />
• Per le istruzioni di sicurezza e igiene comuni, consultare il foglietto delle istruzioni contenuto nel kit<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />

• Se viene conservato a +2-8°C prima dell'apertura, ogni componente può essere utilizzato fino alla<br />
data di scadenza indicata sull'etichetta applicata al kit.<br />
8 - CAMPIONI<br />
• Consultare il foglietto delle istruzioni contenuto nel kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Se analizzati senza previa diluizione, alcuni campioni con concentrazioni particolarmente elevate<br />
di <strong>Ag</strong> <strong>HBs</strong> potrebbero non essere neutralizzati dal reagente di neutralizzazione.<br />
Consultare il capitolo 10 "CALCOLO E INTERPRETAZIONE DEI RISULTATI", sottocapitolo:<br />
"Condizioni di convalida del test".<br />
N.B.: i campioni che presentano una concentrazione particolarmente elevata di <strong>Ag</strong> <strong>HBs</strong> e una<br />
densità ottica superiore a 3.000 con il test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> Ultra, possono essere analizzati<br />
direttamente diluiti in rapporto 1/100 nel diluente campione.<br />
9 - PROCEDURA<br />
Procedere come segue utilizzando i reagenti del kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
1. Preparare la soluzione di lavaggio R2 e la soluzione del coniugato ricostituito (R6+R7): fare<br />
riferimento al capitolo 8 "PREPARAZIONE DEI REAGENTI", sottocapitolo "Reagenti da ricostituire"<br />
del foglietto illustrativo contenuto nel kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
2. Estrarre il vassoio e il numero di strip necessarie dall'involucro protettivo (Reagente R1).<br />
3. Distribuire nei pozzetti nel seguente ordine:<br />
• Pozzetti A1, A2 : 100 µl di controllo negativo R3 del kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Pozzetti B1, B2 : 100 µl di controllo positivo R4 del kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Pozzetti C1, C2, D1, D2 : 100 µl del primo campione da confermare.<br />

• Pozzetti E1, E2, F1, F2 : 100 µl del secondo campione da confermare<br />
• Pozzetti G1, G2, H1, H2 : 100 µl del terzo campione da confermare, ecc.<br />
In funzione del sistema utilizzato, è possibile modificare la posizione dei oppure l’ordine di<br />
distribuzione dei controlli.<br />
4. <strong>Ag</strong>giungere 20 µl di controllo negativo (diluente) RB nei pozzetti A1, B1, C1, D1, E1, F1, G1, H1 e<br />
20 µl di reagente di neutralizzazione RA nei pozzetti A2, B2, C2, D2, E2, F2, G2, H2 ecc...<br />
5. Distribuire 50 µl della soluzione di coniugato ricostituito (R6 + R7) in tutti i pozzetti. Omogeneizzare<br />
con almeno 3 aspirazioni/dispensazioni.<br />
6. Procedere con la fase 7 e successive del capitolo 11 "PROCEDURA" del foglietto illustrativo fornito<br />
con il kit MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
10 - CALCOLO E INTERPRETAZIONE DEI RISULTATI<br />
1) Rapporto di reattività dei controlli e dei campioni<br />

Per questo stesso controllo negativo (R3), il rapporto di reattività deve essere inferiore a 0,8<br />
Per il controllo positivo (R4) addizionato al controllo negativo (diluente) (RB), la densità ottica<br />
registrata deve essere superiore a 0,800.<br />
Per questo stesso controllo positivo (R4), la Percentuale di inibizione deve essere superiore o<br />
uguale al 50%.<br />
2. Per i campioni con una Percentuale di inibizione < 50% e con un rapporto di reattività ≥ 0,8, è<br />
necessario rianalizzare questo campione diluito nel diluente campione (1/100, 1/10.000,<br />
1/1.000.000, fattore di diluizione 100) al fine di raggiungere un rapporto di reattività < 0,8<br />
(campione non confermato in questo caso) oppure ottenere la conferma della neutralizzazione del<br />
campione analizzato (Percentuale di inibizione > 50%).<br />
4) Interpretazione dei risultati<br />
Un campione viene confermato positivo per l'<strong>Ag</strong> <strong>HBs</strong> se il rapporto di reattività è superiore o uguale<br />
a 0,8 e la percentuale di inibizione è superiore o uguale al 50%.<br />

11 - EFFICACIA<br />
L'efficacia del test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> associato al test MONOLISA ® <strong>HBs</strong><br />
<strong>Ag</strong> <strong>ULTRA</strong> è stata determinata utilizzando campioni di pazienti affetti da epatite B acuta e cronica o<br />
da infezioni non correlate all'epatite B, nonché campioni provenienti da panel commerciali e da<br />
sieroconversioni commerciali.<br />
Il limite di sensibilità è stato studiato basandosi sul panel HB EFS francese e sul secondo campione<br />
di riferimento internazionale OMS 2004 rif. (NIBSC 00/588)<br />
Sensibilità analitica<br />
Tutte le concentrazioni del panel HB EFS da 0,06 ng/ml a 2,2 ng /ml, nonché le concentrazioni dello<br />
standard OMS da 0,031 UI /ml a 1 UI /ml risultate positive al test Monolisa <strong>HBs</strong> <strong>Ag</strong> Ultra sono state<br />
confermate dal test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>.<br />
Una concentrazione superiore a 10 mg / ml d'<strong>Ag</strong> <strong>HBs</strong> è stata neutralizzata previa diluizione di questo<br />
campione.<br />
Sensibilità<br />
Lo studio di Sensibilità è stato condotto su 337 campioni positivi provenienti da monitoraggi di<br />
epatite B cronica o acuta di pazienti infetti.<br />
Tutti i campioni positivi sono stati confermati mediante il test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>.<br />
Sono state studiate 15 sieroconversioni HBV commerciali ben documentate (45 campioni): tutti i<br />
campioni positivi sono stati neutralizzati.<br />
Specificità e reazioni crociate<br />
Un panel interno costituito da 16 campioni risultati falsi positivi al test Monolisa <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> sono<br />
stati analizzati e non confermati dal test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>.<br />
102 campioni provenienti da pazienti con diverse patologie o stati immunitari non correlati all'epatite<br />
B (donne incinte, fattori reumatoidi, anticorpi anti-nucleari, anticorpi anti-topo o altre infezioni virali o<br />
batteriche) sono stati analizzati con il test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> e<br />
confermati negativi.<br />
Inoltre, 37 campioni positivi sia per l'<strong>Ag</strong> <strong>HBs</strong> sia per altri marcatori (fattori reumatoidi, anticorpi antinucleari,<br />
anticorpi anti-topo o altre infezioni virali e batteriche) sono stati analizzati e confermati con<br />
il test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>.<br />
Infine, 23 campioni provenienti dal panel di coinfezione BBI PCA 201 (coinfezioni HBV / HIV / HCV /<br />
HTLV) positivi al test Monolisa HBS <strong>Ag</strong> Ultra sono stati confermati puri o previa diluizione.<br />

Studi di riproducibilità<br />
La riproducibilità del test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> è stata determinata<br />
sulla base di 4 campioni: 1 campione negativo (campione 1), 2 campioni positivi (campioni 2 e 3) e<br />
1 campione positivo forte (campione 4).<br />
Al fine di valutare la riproducibilità intra-saggio tutti e 4 i campioni sono stati analizzati 30 volte<br />
durante lo stesso ciclo.<br />
Al fine di valutare la riproducibilità inter-saggio tutti e 4 i campioni sono stati analizzati in duplicati in<br />
2 diversi cicli al giorno e per un periodo di 20 giorni.<br />
La tabella riportata di seguito fornisce la media dei rapporti, la deviazione standard e il coefficiente<br />

12 - LIMITI DEL TEST<br />
Il test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> è strettamente limitato alla conferma della<br />
presenza dell’antigene di superficie del virus dell'epatite B (<strong>Ag</strong> <strong>HBs</strong>) nel siero e nel plasma umano.<br />
I campioni reattivi ripetibili debolmente positivi (rapporto inferiore a 2) con il test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> e risultati non reattivi nel campione non diluito al test MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong> (rapporto di reattività inferiore a 0,8) devono essere interpretati con precauzione. Si<br />
consiglia di rianalizzare questo paziente con un altro metodo o con un secondo campione prelevato<br />

O teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> é utilizado para confirmar a presença do<br />
antigénio de superfície do vírus da hepatite B (<strong>Ag</strong> <strong>HBs</strong>) nas amostras de soro ou plasma humanos,<br />
consideradas reactivas para o teste de rastreio MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>, códigos 72346/72348.<br />
2 - PRINCÍPIO DO TESTE MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong><br />
O teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> utiliza o princípio da neutralização por<br />
excesso de anticorpos anti-HB (diluente anti-HB; reagente de neutralização) do antigénio da hepatite<br />
B presente nas amostras de soro ou plasma.<br />
Recomenda-se a análise das amostras consideradas positivas e repetíveis no teste de rastreio<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> com os reagentes do conjunto MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>. No caso de uma amostra positiva para o antigénio HB, os anticorpos anti-HB do<br />
reagente de neutralização saturam os determinantes antigénicos <strong>HBs</strong> presentes na amostra, que<br />
não poderão mais ligar-se ao anticorpo imobilizado na fase sólida. É observada uma redução da<br />
densidade óptica quando da comparação com a mesma amostra cujo reagente de neutralização é<br />
substituído por um diluente de controlo negativo que não contém anticorpos anti-<strong>HBs</strong>.<br />
Eventualmente, serão controladas várias diluições da amostra para ter em consideração as<br />
variações de concentração em <strong>Ag</strong> <strong>HBs</strong> da mesma.<br />
O ensaio inclui as seguintes etapas de reacção :<br />
1) Incubação dos controlos ou das amostras positivas para <strong>Ag</strong> <strong>HBs</strong> com o reagente de neutralização<br />

4 - PRECAUÇÕES<br />
A qualidade dos resultados depende do cumprimento das boas práticas de laboratório seguintes:<br />
• Não utilizar reagentes cujo prazo de validade tenha expirado.<br />
• Não misturar ou associar reagentes provenientes de outros conjuntos MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong> com números de lote diferentes, durante a mesma manipulação.<br />
• Antes da utilização, aguarde 30 minutos para que os reagentes equilibrem à temperatura ambiente<br />
(18-30°C).<br />
• Utilizar objectos de vidro perfeitamente lavados e enxaguados com água destilada ou, de<br />
preferência, material descartável.<br />
• Consultar igualmente o folheto informativo do conjunto MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
5 - INSTRUÇÕES DE HIGIENE E SEGURANÇA<br />
• Todos os reagentes do conjunto destinam-se a utilização de diagnóstico in vitro.<br />
• O material de origem humana utilizado para a preparação dos reagentes RA e RB foi testado e<br />
considerado negativo em antigénios <strong>HBs</strong>, em anticorpos anti-VIH1, anti-VIH 2 e anti-VHC. Dado<br />
que nenhum método pode garantir de forma absoluta a ausência dos vírus VIH, VHB, VHC ou de<br />
outros agentes infecciosos, deve considerar estes reagentes, bem como as amostras dos<br />
pacientes, como potencialmente infecciosos e manuseá-los com as precauções de utilização<br />
recomendadas para o efeito.<br />

• Após abertura dos frascos, todos os reagentes são utilizáveis até à data de expiração indicada na<br />
embalagem, quando conservados a 2-8°C.<br />
8 - AMOSTRAS<br />
• Consultar o folheto informativo do conjunto MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Algumas amostras com uma concentração muito elevada em <strong>Ag</strong> <strong>HBs</strong> podem não ser<br />
neutralizadas pelo reagente de neutralização se forem testadas não diluídas.<br />
Consultar o capítulo 10 «CÁLCULO E INTERPRETAÇÃO DOS RESULTADOS», sub-capítulo :<br />
«Condições de validação do teste».<br />
OBSERVAÇÃO : as amostras que apresentem uma concentração muito elevada em <strong>Ag</strong> <strong>HBs</strong> e<br />
densidade óptica superior a 3.000 com o teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> Ultra, podem ser testadas<br />
directamente diluídas na relação de 1/100 com o diluente de amostras.<br />
9 - MODO DE UTILIZAÇÃO<br />
Proceder da seguinte forma ao utilizar os reagentes do conjunto MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
1. Preparar a solução de lavagem R2 e a solução de conjugado reconstituído (R6+R7) : consultar o<br />
capítulo 8 «PREPARAÇÃO DOS REAGENTES», sub-capítulo «reagentes para reconstituição» do<br />
folheto informativo do conjunto MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
2. Retirar da embalagem de protecção a placa de suporte e o número de tiras necessárias (Reagente<br />
R1).<br />
3. Distribuir nos recipientes pela seguinte ordem:<br />
• Recipientes A1, A2: 100 µl de controlo negativo R3 do conjunto MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
• Recipientes B1, B2: 100 µl de controlo positivo R4 do conjunto MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />

• Recipientes C1, C2, D1, D2: 100 µl da primeira amostra a confirmar.<br />
• Recipientes E1, E2, F1, F2: 100 µl da segunda amostra a confirmar.<br />

Em função do sistema utilizado é possível modificar a posição ou a ordem de distribuição dos<br />
controlos.<br />
4. Adicionar 20 µl de diluente de controlo negativo RB nos recipientes A1, B1, C1, D1, E1, F1, G1,<br />
H1 e 20 µl de reagente de neutralização RA nos recipientes A2, B2, C2, D2, E2, F2, G2, H2 etc...<br />
5. Distribuir 50 µl da solução de conjugado reconstituído (R6 + R7) em todos os recipientes.<br />
Homogeneizar através de, pelo menos, 3 aspirações/ descargas.<br />
6. Prosseguir com a etapa 7 e as etapas seguintes do capítulo 11 «MODO DE UTILIZAÇÃO» do<br />
folheto informativo do conjunto MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>.<br />
10 - CALCULO E INTERPRETAÇÃO DOS RESULTADOS<br />

1. Os valores do controlo negativo (R3) a que se adicionou diluente de controlo negativo (RB) ou<br />

óptica.<br />
Para este mesmo controlo negativo (R3), o rácio de reactividade deve ser inferior a 0,8<br />
Para o controlo positivo (R4) a que se adicionou diluente de controlo negativo (RB), a densidade<br />
óptica registada deve ser superior a 0,800.<br />
Para este mesmo controlo positivo (R4), a percentagem de inibição deve ser superior ou igual a<br />
50%.<br />
2. Em relação às amostras com uma percentagem de inibição < 50% e com um rácio de reactividade<br />
≥ 0,8, é necessário repetir a análise das mesmas, diluindo-as com diluente de amostras (1/100,<br />
1/10 000, 1/1 000 000, diluição de factor 100) para obter um rácio de reactividade < 0,8 (amostra<br />
não confirmada neste caso) ou a confirmação da neutralização da amostra analisada<br />
(Percentagem de inibição > 50%).<br />

11 - CARACTERÍSTICAS DO DESEMPENHO<br />
Os desempenhos do teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> associado ao teste<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> foram determinados com a ajuda de amostras de doentes portadores<br />
de Hepatite B aguda e crónica ou com infecções não relacionadas com Hepatite B, bem como com<br />
amostras provenientes de painéis comerciais e de seroconversões comerciais.<br />
O limite de sensibilidade foi estudado com a ajuda do painel HB EFS francês, bem como do 2º<br />
Padrão Internacional da OMS 2004 refª (NIBSC 00/588)<br />
Sensibilidade analítica<br />
Todas as concentrações do painel HB EFS entre 0,06 ng/ml e 2,2 ng /ml, bem como as<br />
concentrações do Padrão OMS entre 0,031 UI /ml e 1 UI /ml consideradas positivas com o teste<br />
Monolisa <strong>HBs</strong> <strong>Ag</strong> Ultra foram confirmadas com a ajuda do teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>.<br />
Uma concentração superior a 10 mg / ml de <strong>Ag</strong> <strong>HBs</strong> foi neutralizada após diluição desta amostra.<br />
Sensibilidade<br />
O estudo da sensibilidade foi realizado em 337 amostras positivas provenientes do<br />
acompanhamento de doentes infectados com Hepatite B crónica ou aguda.<br />
Todas as amostras positivas foram confirmadas com a ajuda do teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>.<br />
Foram estudadas 15 seroconversões comerciais do VHB bem documentadas (45 amostras) : todas<br />
as amostras positivas foram neutralizadas.<br />

Especificidade e reacções cruzadas<br />
Um padrão interno de 16 amostras consideradas amostras falsas positivas com o teste Monolisa<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> foi testado com o teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>, não tendo<br />
sido confirmado o resultado positivo.<br />
102 amostras provenientes de doentes com diferentes patologias ou estatuto imunitário sem relação<br />
com a Hepatite B (grávidas, factores reumatóides, anticorpos anti-nucleares, anticorpos anti-rato ou<br />
outras infecções virais ou bacterianas) foram analisadas com o teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong> e confirmadas como sendo negativas.<br />
Além disso, 37 amostras positivas em simultâneo para <strong>Ag</strong> <strong>HBs</strong> e para outros marcadores (factores<br />
reumatóides, anticorpos anti-nucleares, anticorpos anti-rato ou outras infecções virais ou<br />
bacterianas) foram analisadas com o teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> e<br />
confirmadas com o mesmo.<br />
Finalmente, 23 amostras provenientes do painel de co-infecção BBI PCA 201 (co-infecções<br />
VHB/VIH/ VHC /VHTL), positivas com o Monolisa HBS <strong>Ag</strong> Ultra, foram confirmadas no estado puro<br />
ou após diluição.<br />
Estudos de reprodutibilidade<br />
A reprodutibilidade do teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> foi determinada com a<br />
ajuda de 4 amostras : 1 amostra negativa (amostra 1), 2 amostras positivas (amostras 2 e 3) e<br />
1 amostra positiva forte (amostra 4).<br />
O estudo intra-ensaio foi realizado analisando 4 amostras, 30 vezes durante o mesmo ensaio;<br />
O estudo inter-ensaio foi realizado analisando 4 amostras em duplicado, à razão de 2 ensaios<br />
diferentes por dia e durante um período de 20 dias.<br />
Os resultados expressos segundo a média, desvio padrão e CV são apresentados nos quadros<br />

12 - LIMITAÇÕES DO TESTE<br />
O teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> é estritamente limitado à confirmação da<br />
presença do antigénio de superfície do vírus da hepatite B (<strong>Ag</strong> <strong>HBs</strong>) no soro e plasma humanos.<br />
As amostras reactivas repetíveis, com resultados positivos fracos (rácio inferior a 2) com o teste<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> e consideradas não reactivas (amostra não diluída) quando analisadas<br />
com o teste MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> (rácio de reactividade inferior a 0,8),<br />
devem ser interpretadas com precaução. Recomenda-se a realização de um novo teste a este<br />
paciente, utilizando um método diferente ou uma segunda amostra colhida posteriormente.<br />

MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testet använder principen för neutralisering genom<br />
överskott av antikroppar till anti-<strong>HBs</strong> (anti-<strong>HBs</strong>-utspädning: neutraliseringsreagens) för <strong>Ag</strong> <strong>HBs</strong> i<br />
serum- eller plasmaprover.<br />
Det rekommenderas att man testar prover som upptäcks vara positiva av MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong>-undersökningstest med reagens från MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-satsen.<br />
Om ett prov är positivt i <strong>HBs</strong> antigen, kommer anti-<strong>HBs</strong>-antikroppar i neutraliseringsreagensen att<br />
mätta <strong>HBs</strong> antigenämnen i provet som inte längre kan binda antikropparna som är fixerade i solid<br />
form. En minskning av den optiska densiteten kommer att ses när man jämför samma prov i vilken<br />
neutraliseringsreagensen byts ut mot ett negativt kontrollutspädningsmedel som inte innehåller anti-<br />
<strong>HBs</strong> antikroppar. Flera utspädningar av provet kommer att analyseras, efter behov, för att ta med<br />
variationer i <strong>HBs</strong> <strong>Ag</strong>-koncentration i beräkningen.<br />
Testet inbegriper följande reaktionella faser:<br />
1) Inkubation av kontroller och prover som är positiva i <strong>HBs</strong> <strong>Ag</strong> med neutraliseringsreagens och<br />
konjugering vid förekomst av anti-<strong>HBs</strong> antikroppar som är fixerade i solid form.<br />
För varje prov tillhandahålls kontroller genom att neutraliseringsreagensen byts ut mot ett negativt<br />
kontrollutspädningsmedel.<br />

4 - FÖRSIKTIGHETSÅTGÄRDER<br />
Kvaliteten på resultaten beror på följande goda laboratorieseder:<br />
• Använd inte utgångna reagenser.<br />
• Blanda eller kombinera inte reagenser från MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-satser<br />
med olika satsnummer under samma procedur.<br />
• Före användning måste reagenserna stabiliseras i rumstemperatur (18–30°C) i 30 minuter.<br />
• Använd glas som har tvättats och sköljts noggrant i destillerat vatten eller helst engångsmaterial.<br />

5 - HÄLSO- OCH SÄKERHETSANVISNINGAR<br />
• Alla reagenser i satsen är avsedda för diagnostisk in vitro-användning.<br />
• Det mänskligt härledda materialet för preparering av reagenserna RA och RB har testats och visat<br />
sig vara negativa för <strong>HBs</strong> antigen och för anti-HIV1, anti-HIV2 och anti-HVC-antikroppar. Eftersom<br />
ingen metod absolut kan garantera frånvaron av HIV-, HBV- eller HVC-virus eller andra<br />
infektionsämnen, betrakta dessa reagenser, samt patientprover, som potentiella infektioner och<br />
hantera dem med försiktighet.<br />

9 - TESTPROCEDUR<br />
Fortsätt enligt följande genom att använda reagenserna från MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-satsen.<br />
1. Preparera tvättlösningen (R2) och den ombildade konjugeringslösningen (R6+R7): se avsnitt 8<br />
”PREPARARERING AV REAGENSER”, underrubriken ”Reagenser för ombildning” i MONOLISA ®<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-satsens förpackningsinlägg.<br />
2. Ta bort mikrotiterplattan och antalet erfordrade remsor (reagens R1) från skyddsemballaget.<br />
3. Distribuera i fördjupningarna i följande ordningsföljd :<br />
• Fördjupningarna A1, A2: 100 µl av negativ kontroll (R3) från MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-satsen.<br />
• Fördjupningarna B1, B2: 100 µl av positiv kontroll (R4) från MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-satsen.<br />

1. Värdena för den negativa kontrollen (R3) med tillsats av negativt kontrollutspädningsmedel (RB)<br />
eller neutraliseringsreagens (RA) måste vara mindre eller lika med 0,080 OD-enheter.<br />
För samma negativa kontroll (R3), måste reaktivitetsvärdena vara mindre än 0,8.<br />
För den positiva kontrollen (R4) med tillsats av negativt kontrollutspädningsmedel (RB), måste den<br />
registrerade optiska densiteten vara högre än 0,800.<br />
För samma positiva kontroll (R4), måste inhibitionsprocenten vara högre än, eller lika med, 50 %.<br />
2. För prover med en inhibitionsprocent < 50 % och ett reaktivitetsvärde ≥ 0,8, är det nödvändigt att<br />
ompröva dessa prover utspädda i provlösningen (1/100; 1/10 000; 1/1 000 000 osv., 100 X<br />
utspädning) för att få ett reaktivitetsvärde < 0,8 (prov inte bekräftat i detta fall) eller fastställande av<br />
neutralisering av det testade provet (inhibitionsprocent ≥ 50 %).<br />
4) Tolkning av resultaten<br />
Ett prov kommer att fastställas vara positivt för <strong>HBs</strong> <strong>Ag</strong> om reaktivitetsvärdet är högre än, eller lika<br />

11 - UTFÖRANDEN<br />
Utförandena av MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-analys tillsammans med MONOLISA ®<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> har bestämts genom testning av prover från kliniska patienter med akut och kronisk<br />
hepatit B-infektion eller med sjukdomar utan samband med hepatit B-infektion, och från<br />
kommersiella prover och serokonversionspaneler.<br />
Dessutom har sensitivitetsgränsen testats med hjälp av den Franska French EFS HB-panelen, och<br />
WHO-standarderna (NIBSC-kod 00/588).<br />
Analytisk sensitivitet<br />
Alla utspädda provkoncentrationer från 0,06 ng/ml till 2,2 ng/ml från EFS HB sensitivietspanel och<br />
alla positiva resultat från WHO NIBSC-panelen bekräftades med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>-analys.<br />
En <strong>HBs</strong> <strong>Ag</strong>-koncentration på över 10 mg/ml neutraliserades efter spädning av detta prov.<br />
Sensitivitet<br />
Sensitivitetsstudier har utförts på 337 positiva prover från uppföljning av kroniskt eller akut HBVinfekterade<br />
personer. Alla positiva prover bekräftades med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>-analys.<br />
Totalt 15 väldokumenterade kommersiella HBV serokonversionspaneler (45 prover) studerades<br />
också. Alla positiva prover neutraliserades.<br />
Specificitet och korsreaktivitet<br />
En intern panel på 16 falska positiva prover med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> testades med<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-analys och bekräftades inte.<br />
Analysen av 102 patienter som visade olika patologier eller tillstånd ej kopplade till hepatit B<br />
(havande kvinnor, reumatoid faktor, antinukleära antikroppar, anti-mus Ig eller andra virala eller<br />
bakteriella infektioner) testades med MONOLISA ® <strong>Ag</strong> <strong>HBs</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> och bekräftades<br />
som negativa.<br />
Dessutom testades 37 positiva prover med <strong>HBs</strong> <strong>Ag</strong> samt positiva prover med olika markörer (prover<br />
med reumatoida faktorer, antinukleära antikroppsprover, anti-mus Ig eller virala infektions-prover)<br />
med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-analys; alla dessa prover bekräftades som<br />
positiva för <strong>HBs</strong> <strong>Ag</strong>.<br />
23 <strong>HBs</strong> <strong>Ag</strong> positiva prover från co-infektionspanelen PCA201 BBI (med HBV, HIV, HCV och/eller<br />
HTLV) bekräftades alla med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-analys, rena eller efter<br />
spädning.<br />

Analysens reproducerbarhet<br />
Reproducerbarheten för MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-analysen har bestämts<br />
genom analys av 4 prover : 1 negativt prov (prov 1), 2 <strong>HBs</strong> <strong>Ag</strong> positiva prover (prov 2 och 3) och 1<br />
högt <strong>HBs</strong> <strong>Ag</strong> positivt prov (prov 4).<br />
Reproducerbarheten inom analysen har utvärderats genom testning av dessa 4 prover 30 gånger i<br />
samma körning.<br />
Reproducerbarheten mellan analyser har utvärderats genom testning av dessa 4 prover i duplikat<br />
under 20 dagar med 2 oberoende körningar varje dag.<br />

MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testet är strikt begränsat till fastställande av<br />
förekomst av hepatit B surface antigen (<strong>HBs</strong> <strong>Ag</strong>) i mänskligt serum eller plasma.<br />
Prover som upprepade gånger uppmäts vara svagt positiva (värde mindre än 2) med MONOLISA ®<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-testet och som fastställs vara icke-reaktiva för ett outspätt test med MONOLISA ®<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testet (reaktivitetsvärde mindre än 0,8) måste tolkas med<br />
försiktighet. Det rekommenderas att man testar om dessa patienter med en annan metod eller med<br />
ett andra prov senare.<br />

MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen anvendes til at bekræfte tilstedeværelsen af<br />
hepatitis B-overfladeantigen (<strong>HBs</strong><strong>Ag</strong>) i prøver af humant serum eller plasma, der blev fundet reaktive<br />
i MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-screeningstesten (kode 72346/72348).<br />
2 - PRINCIP FOR MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> TEST<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen anvender princippet om neutralisering ved<br />
overskud af antistoffer mod anti-<strong>HBs</strong> (anti-<strong>HBs</strong>-fortynder: neutraliseringsreagens) af det <strong>HBs</strong><strong>Ag</strong>, der<br />
findes i plasma- eller serumprøver.<br />
Det anbefales at teste de prøver, der gentagne gange er fundet positive i MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong>-screeningstesten, med reagenserne fra MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>sættet.<br />
Hvis en prøve er positiv for <strong>HBs</strong>-antigen, mætter anti-<strong>HBs</strong>-antistofferne i<br />
neutraliseringsreagenset <strong>HBs</strong>-antigendeterminanterne i prøven, som ikke længere kan binde sig til<br />
det antistof, der er bundet til den faste fase. Der kan observeres et fald i optisk densitet, når der<br />
foretages sammenligning med en tilsvarende prøve, hvor neutraliseringsreagenset er blevet erstattet<br />
med en negativ kontrolfortynder uden anti-<strong>HBs</strong>-antistoffer. Der analyseres flere fortyndinger af<br />
prøven efter behov for at tage højde for variationerne i <strong>HBs</strong><strong>Ag</strong>-koncentration.<br />
Testen omfatter følgende faser i reaktionen:<br />
1) Inkubation af kontroller og prøver, der er positive for <strong>HBs</strong><strong>Ag</strong>, med neutraliseringsreagenset og<br />
konjugatet ved tilstedeværelse af anti-<strong>HBs</strong>-antistoffer bundet til den faste fase.<br />
For hver prøve oprettes der kontrolbrønde ved at erstatte neutraliseringsreagenset med den<br />
negative kontrolfortynder.<br />
Resten af proceduren er den samme som protokollen for MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-sættet .<br />
2) Vask og derefter udvikling af den enzymaktivitet, der er bundet til den faste fase, via tilsætning af<br />
substrat.<br />
3) Standsning af udviklingstrinnet, derefter aflæsning af optiske densiteter ved 450/620–700 nm og<br />

Kvaliteten af resultaterne afhænger af følgende regler for god laboratoriepraksis:<br />
• Brug ikke reagenser efter udløbsdatoen.<br />
• Undlad at blande eller kombinere reagenser fra MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>sættet<br />
med forskellige partinumre i den samme procedure.<br />
• Vent 30 minutter, før reagenserne tages i brug, så de kan stabilisere sig ved stuetemperatur<br />
(18-30°C).<br />
• Benyt engangsmateriale, eller hvis dette ikke er muligt, udstyr af glas, som er vasket grundigt og<br />
skyllet med destilleret vand.<br />

9 - ANALYSEPROCEDURE<br />

1. Forbered vaskeopløsningen (R2) og den rekonstituerede konjugatopløsning (R6+R7) : Se afsnit 8<br />
”FORBEREDELSE AF REAGENSER,” underafsnittet ”Reagenser til rekonstituering” i<br />
indlægssedlen til MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong>-sættet.<br />
2. Tag mikropladerammen og det påkrævede antal strimler (reagens R1) ud af beskyttelsesposen.<br />
3. Fordel reagenserne i brøndene i nedenstående rækkefølge:<br />

1. Værdierne for den negative kontrol (R3) tilsat negativ kontrolfortynder (RB) eller<br />
neutraliseringsreagens (RA) skal være mindre end eller lig med 0,080 OD-enheder.<br />
For denne samme negative kontrol (R3) skal reaktivitetsforholdet være mindre end 0,8.<br />
For den positive kontrol (R4) tilsat negativ kontrolfortynder (RB) skal den registrerede optiske<br />
densitet være større end 0,800.<br />
For denne samme positive kontrol (R4) skal hæmningsprocenten være større end eller lig med<br />
50 %.<br />
2. Prøver med en hæmningsprocent < 50 % og med et reaktivitetsforhold ≥ 0,8 skal testes igen<br />
fortyndet med prøvefortynder (1/100, 1/10.000, 1/1.000.000 osv., 100 X fortynding) for at få et<br />
reaktivitetsforhold < 0,8 (prøven er i så fald ikke bekræftet) eller en bekræftelse af neutraliseringen<br />
i den testede prøve (hæmningsprocent ≥ 50 %).<br />

11 - YDEEVNE<br />
Ydeevnen for MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen i forbindelse med MONOLISA ®<br />
<strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> er fastlagt ved at teste prøver fra kliniske patienter med akut og kronisk hepatitis B-<br />
infektion eller med sygdomme, der ikke har relation til hepatitis B-infektion, og fra kommercielle<br />
prøver og serokonverteringspaneler.<br />
Desuden er sensitivitetsgrænsen testet vha. det franske EFS HB-panel og WHO-standarderne<br />
(NIBSC-kode 00/588).<br />
Analytisk sensitivitet<br />
Alle fortyndede prøvekoncentrationer fra 0,06 ng/ml til 2,2 ng/ml fra EFS HB-sensitivitetspanelet og<br />
alle positive resultater fra WHO NIBSC-panelet blev bekræftet med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>-testen.<br />
En <strong>HBs</strong><strong>Ag</strong>-koncentration, der var højere end 10 mg/ml, blev neutraliseret efter fortynding af denne<br />
prøve.<br />
Sensitivitet<br />
Der er udført sensitivitetsundersøgelser på 337 positive prøver fra opfølgning af kroniske eller akutte<br />
HBV-inficerede. Alle de positive prøver blev bekræftet med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong><br />
<strong>CONFIRMATORY</strong>-testen.<br />
I alt 15 grundigt dokumenterede kommercielle HBV-serokonverteringspaneler (45 prøver) blev også<br />
undersøgt. Alle de positive prøver blev neutraliseret.<br />
Specificitet og krydsreaktivitet<br />
Et internt panel på 16 falsk positive prøver med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> blev testet med<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen og blev ikke bekræftet.<br />

Analysen af 102 patienter, der viste forskellige patologier eller status, der ikke var forbundet med<br />
hepatitis B (gravide kvinder, reumatoid faktor, antinukleare antistoffer, anti-muse-Ig eller andre virale<br />
eller bakterielle infektioner), blev testet med MONOLISA ® <strong>Ag</strong> <strong>HBs</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong> og<br />
bekræftet negativ.<br />
Desuden blev 37 prøver, der var positive med <strong>HBs</strong><strong>Ag</strong> og også positive med forskellige markører<br />
(prøver med reumatoid faktor, antinukleare antistoffer, anti-muse-Ig eller virale infektioner), testet med<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen. Alle disse prøver blev bekræftet positive for<br />
<strong>HBs</strong><strong>Ag</strong>.<br />
23 <strong>HBs</strong><strong>Ag</strong>-positive prøver fra koinfektionspanelet PCA201 BBI (med HBV, HIV, HCV og/eller HTLV)<br />
blev alle bekræftet med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen, rene eller efter<br />
fortynding.<br />
Reproducérbarhed af testen<br />
Reproducérbarheden af MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen er fastlagt vha.<br />
analysen af 4 prøver: 1 negativ prøve (prøve 1), 2 <strong>HBs</strong><strong>Ag</strong>-positive prøver (prøve 2 og 3) og 1 høj<br />
<strong>HBs</strong><strong>Ag</strong>-positiv prøve (prøve 4).<br />
Reproducérbarheden inden for testen er evalueret ved at teste disse 4 prøver 30 gange i den samme<br />
kørsel.<br />
Reproducérbarheden mellem test er evalueret ved at dobbeltteste disse 4 prøver i løbet af 20 dage<br />
på 2 selvstændige kørsler hver dag.<br />

12 - TESTENS BEGRÆNSNINGER<br />
MONOLISA ® <strong>HBs</strong> <strong>Ag</strong> <strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen er strengt begrænset til bekræftelse af<br />
tilstedeværelsen af hepatitis B-overfladeantigen (<strong>HBs</strong><strong>Ag</strong>) i humant serum eller plasma.<br />
Prøver, som gentagne gange testes svagt positive (forhold mindre end 2) med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong>-testen, og som bestemmes ikke-reaktive ved en ufortyndet prøve med MONOLISA ® <strong>HBs</strong> <strong>Ag</strong><br />
<strong>ULTRA</strong> <strong>CONFIRMATORY</strong>-testen (reaktivitetsforhold mindre end 0,8), skal fortolkes med forsigtighed.<br />
Det anbefales, at disse patienter testes igen med en anden metode eller med en anden prøve udtaget<br />
