PRÓLOGO El desarrollo que ha experimentado la broncoscopia en los últimos 50 años se puede considerar de admirable y vertiginoso. Sin duda no existe otra área de la neumología que haya logrado avances tan notorios, relevantes y profundos en tan poco tiempo. En la actualidad tanto las aplicaciones diagnósticas de la broncoscopia como las terapéuticas han aumentado de forma muy considerable pasando a ocupar un puesto muy relevante no sólo dentro de la Neumología y de la Cirugía Torácica, sino también dentro de otras disciplinas como: la Otorrinolaringología, la Anestesia, la Medicina Intensiva y la Pediatría. Esta expansión ha tenido su base principal en el gran desarrollo tecnológico ocurrido en diferentes áreas como son: la instrumentación, las ópticas, concretamente las fibras ópticas, las fuentes de iluminación, la electrónica y las técnicas anestésicas por mencionar sólo algunas de las más importantes. El hito histórico que permitió la eclosión de los procedimientos endoscópicos de la vía aérea fue sin duda la introducción por Shigedo Ikeda entre 1964-65 del broncofibroscopio. Su objetivo inicial fue mejorar la iluminación y la visualización obtenida por el broncoscopio rígido superando sus limitaciones, pero esa búsqueda le llevó al desarrollo del broncofibroscopio. Muy pronto fueron patentes sus ventajas permitiendo ampliar la exploración del árbol traqueobronquial hasta bronquios subsegmentarios de quinta generación, llevándole a reclasificar la anatomía endoscópica del sistema respiratorio. Como consecuencia de la aparición del broncofibroscopio hemos sido testigos la mayoría de nosotros del desarrollo pujante de la endoscopia respiratoria experimentado desde entonces. Junto a la expansión de las técnicas de broncofibroscopia diagnósticas como el lavado broncoalveolar o la punción del mediastino con agujas de adenopatías, hemos presenciado el resurgimiento de la broncoscopia rígida como instrumento terapéutico para la aplicación de los nuevos tratamientos de las obstrucciones centrales de la vía aérea. El broncoscopio rígido ha pasado a ser el instrumento de elección para la aplicación del láser, las endoprótesis, la crioterapia, y la electrocoagulación adquiriendo una importancia capital en la broncoscopia terapéutica. A su vez muchos de esos tratamientos mencionados como la resección con láser y las endoprótesis se han consolidado en el tratamiento de las complicaciones obstructivas del carcinoma broncogénico, de otras tumoraciones malignas, y de la patología inflamatoria postintubación pasando a ser tratamientos habituales. 7
