Universidad de San Diego, E.U. http://meded.ucsd.edu/clinicalmed/vital.htm Un grupo de oftalmólogos y epidemiólogos llegó a dos conclusiones importantes después de analizar un importante número de pacientes adultos mayores. La primera conclusión es que la retinopatía hipertensiva sirve para pronosticar a largo plazo el riesgo de un derrame cerebral (apoplejía), independientemente de la presión sanguínea, incluso en hipertensos con buen control de la enfermedad. La segunda conclusión es que la evaluación de los signos de retinopatía hipertensiva con fotografías de la retina es una herramienta útil para controlar el riesgo de derrames cerebrales, en pacientes atendidos durante muchos años, aun antes de los grandes avances en el campo de las imágenes diagnósticas para Oftalmología. Hypertension, journal of American Heart Association. 113.01414 Publicado en línea antes de Agosto 12, 2013. doi: 10.1161/hypertensionaha.113.01414 el Estudio ARIC de Estados Unidos (Riesgo de Aterosclerosis en sus Comunidades) a 2907 personas hipertensas de 50 a 73 años, que cuando se examinaron entre 1993 y 1995, tenían fotografías de la retina y ningún antecedente de diabetes mellitus, derrame cerebral o enfermedad coronaria. Pero además, esta población tenía datos más recientes de derrame cerebral. Al revisar las fotografías de la retina, buscaron signos de retinopatía hipertensiva y los clasificaron como ninguno, medio, y moderado/severo. Luego, identificaron y validaron cualquier incidente de derrame, infarto cerebral o apoplejía hemorrágica. Después de un periodo de seguimiento promedio de 13,0 años, se encontró que 165 personas tuvieron un incidente: 146 infartos cerebrales y 15 apoplejías hemorrágicas. 28 <strong>Franja</strong> <strong>Visual</strong> <strong>132</strong> Vol. 23 Por último, hicieron los ajustes estadísticos por edad, género, presión sanguínea u otros factores de riesgo, y encontraron que aquellas personas con retinopatía hipertensiva moderada tenían más probabilidad de presentar un derrame (moderado versus sin retinopatía: tasa de riesgo multivariable 2,37 [95% intervalo de confianza, 1,39- 4,02]). Entre los hipertensos, se hizo un análisis de los medicamentos y su relación con el control de la presión sanguínea. Así, se relacionó la retinopatía hipertensiva con un aumento en el riesgo de infartos cerebrales (retinopatía media: tasa de riesgo, 1,96 [95% intervalo de confianza, 1,09-3,55] y retinopatía moderada: tasa de riesgo, 2,98 [95% intervalo de confianza, 1,01-8,83]).
