CRONOGRAMA PROCESO EVALUACIÓN 2011<br />
ACREDITACIÓN DE REQUISITOS PARA REBAJA<br />
EMPRESAS CON MÁS DE 100 TRABAJADORES,<br />
ACTIVIDADES MINERA, INDUSTRIAL Y COMERCIAL.<br />
A<strong>de</strong>más <strong>de</strong> los anteriores:<br />
Contar con Departamento <strong>de</strong> Prevención <strong>de</strong> Riesgos Profesionales, dirigido por<br />
un experto en la materia. TÍtulo III D.S. N° 40 <strong>de</strong> 1969. Mediante la presentación<br />
<strong>de</strong> carta, informe, <strong>de</strong>claración simple o jurada ante notario <strong>de</strong>l representante<br />
legal, indicando:<br />
•Fecha <strong>de</strong> contrato <strong>de</strong>l Experto.<br />
•N°<strong>de</strong> registro <strong>de</strong> experto otorgado por el respectivo Servic io <strong>de</strong> Salud.<br />
•Número <strong>de</strong> jornadas por la cual ha sido contratado el experto<br />
•Copia <strong>de</strong>l contrato <strong>de</strong> trabajo o <strong>de</strong> la resolución correspondiente.<br />
Si los antece<strong>de</strong>ntes para rebaja los entrega <strong>de</strong>spués <strong>de</strong>l 31 <strong>de</strong> octubre, pero antes<br />
<strong>de</strong>l 01 <strong>de</strong> enero <strong>de</strong>l año siguiente, la cotización inferior que resulte se aplicará a<br />
contar <strong>de</strong>l 01 día <strong>de</strong>l tercer mes siguiente al cumplimiento <strong>de</strong> requisitos (marzo<br />
2012).
