s I write this column, it<br />

strikes me that the difficult<br />

and uncertain economic<br />

times facing our community and<br />

nation mirrors our families’<br />

situations when confronted with a<br />

diagnosis of Alzheimer’s disease or<br />

other dementing illnesses.<br />

Families, on a daily basis, face<br />

many unknowns, much uncertainty<br />

and many uncontrollable<br />

circumstances. Yet we know that<br />

with preparation and support from the Alzheimer’s <strong>Association</strong><br />

and respite services, family members can begin<br />

to deal with this prolonged disease and caregiving experience<br />

with a degree of toughness and resolve. For the most<br />

part caregivers do not falter when faced with challenging<br />

circumstances. It is that toughness in families that I as the<br />

executive director have come to admire. Thus it is in this<br />

spirit that I write this note of reassurance to you our readers,<br />

donors, and users of services. Know that our Chapter<br />

is also prepared to deal with present and future financial<br />

challenges and to do so with a toughness and resolve that<br />

defines who we are as an organization and the people we<br />

serve.<br />

Interestingly, we see a phenomenon emerging.<br />

Despite these difficult and turbulent times, we observe<br />

that donors have not abandoned us. They continue their<br />

generous giving, and in doing so, ensure continuation of<br />

necessary free family educational and supportive services<br />

without interruption to programming. Our board<br />

members take their responsibilities seriously. They support<br />

the Chapter in all ways using their time and talent to<br />

diligently and prudently monitor and provide oversight of<br />

our financial situation and operational decisions.<br />

On a regular basis, Chapter staff reviews practices<br />

across all functional areas so that we can be more efficient<br />

and productive. We too, are attempting to do more with<br />

less (like our families) but our goal always is to continue<br />

to provide a high level of service and support in all areas,<br />

without skipping a beat (like our families). So friends,<br />

generous and committed leadership volunteers and<br />

donors, be reassured that we are a determined group and<br />

as always will continue to work hard and smart on your<br />

behalf.<br />

Once again we thank you for your unwavering and<br />

constant support during some very difficult times. We are<br />

Schneider Joins<br />

Board of Directors<br />

The Alzheimer’s <strong>Association</strong> of Greater Cincinnati<br />

recently welcomed Tracy Schneider to its board of<br />

directors.<br />

Ms. Schneider, vice president of In-flight Services for<br />

Comair, joined the board in January.<br />

“We are certainly pleased<br />

to have Tracy join our board,”<br />

said Executive Director Clarissa<br />

Rentz. “Her enthusiasm and<br />

expertise will greatly help us<br />

move our chapter forward.”<br />

A native of Madison, Wis.,<br />

Programs and<br />

partnership is in the making between Alzheimer’s<br />

<strong>Association</strong> chapters serving the state of Ohio and<br />

the Ohio Attorney General. The goal of this collaboration<br />

is to create a statewide training for law<br />

enforcement officers with a focus on Alzheimer’s and dementia.<br />

This training will help law enforcement officers to recognize<br />

individuals who have cognitive impairment and to<br />

interact effectively with them in emergency situations. Individuals<br />

with dementia could become involved in many<br />

dangerous situations such as traffic accidents, wandering<br />

incidents, financial exploitation scams and public disturbances<br />

where the person’s behavior may be misinterpreted<br />

as threatening to others. Knowing the proper response can<br />

make these situations easier for police and less upsetting for individuals with cognitive<br />

impairment. The training will include information about the MedicAlert® +<br />

Safe Return® program, a nationwide system for identifying persons with dementia<br />

who are at risk of becoming lost or having a medical emergency.<br />

The Ohio Attorney General’s office will also provide information about the<br />

Ohio Missing Adult Alert, which has been in operation since June, 2008 and has<br />

helped locate several missing persons with dementia. This alert system, also known<br />

as the “Silver Alert,” is used in the same way an Amber Alert works for a missing<br />

Collaborative Spirit Fuels Education Efforts<br />

lacing a family member with Alzheimer’s disease or dementia in a longterm<br />

care facility is one of the most difficult decisions a family will face.<br />

The timing of that decision is based on many factors; most are unique to<br />

the individual with the diagnosis and the family’s particular concerns.<br />

Alzheimer’s disease is a progressive, degenerative illness that requires additional<br />

caregiving on the part of the family as the disease moves into middle to late<br />

stages. The most common challenges that cause families to decide on placement<br />

include:<br />

• increased care needs (incontinence, decreased mobility, nutrition problems)<br />

• safety issues (wandering, household hazards, combativeness) and<br />

• health concerns of the primary caregiver.<br />

Families may be forced to make a sudden placement decision when a drastic<br />

change in health or care needs occurs in either the person with dementia or the<br />

primary caregiver.<br />

It is helpful to discuss long-term care plans before the need arises. It can also<br />

be helpful to involve as many family members as possible to gather support and<br />

to help plan for a smooth transition for the person with dementia. Sometimes,<br />

a person in the early stages of Alzheimer’s disease may recognize their need for<br />

more assistance and be able to work with the family to make the decision. However,<br />

most individuals with memory loss do not initiate a move from their home<br />

and may be resistant to the idea.<br />

When placement can be planned, there is opportunity to ease the transition<br />

for the individual with memory loss, as well as the family. A few points to<br />

consider:<br />

• Decide WHERE your family member would be best placed.<br />

There is no single, perfect care facility, but there can be a care facility that is best<br />

suited to meet your family’s particular needs. Take the time to visit many types of<br />

facilities to compare services. Ask questions, observe interactions and get feedback<br />

from others. Narrow down your choices to 2 or 3 places and visit again. Keep in<br />

mind that caregiving does not end with placement—you will play an important<br />

role in helping the facility learn about your family member as the adjustment<br />

process occurs.<br />

• Decide WHO should tell your family member about the move.<br />

Identify the best person to tell your family member about the move to a new<br />

place. It may be another family member, a friend or doctor they know and respect.<br />

For some individuals, this discussion is more comfortable when more than<br />

child. Alerts are posted on Artemis boards on highways and fax alerts are sent to<br />

police departments and other agencies, including the Alzheimer’s <strong>Association</strong>, to<br />

assist in search and rescue efforts.<br />

The law enforcement trainings will be conducted at regional locations in June<br />

of this year.<br />

The Alzheimer’s <strong>Association</strong> of Greater Cincinnati has also partnered with the<br />

Southwestern Ohio Caregiver Coalition to provide a two-day conference for family<br />

and professional caregivers in October.<br />

The Caregiver Coalition consists of many community agencies including the<br />

Council on Aging of Southwestern Ohio, Catholic Charities Southwestern Ohio,<br />

Jewish Family Services, Hospice of Cincinnati and other service providers in the tristate<br />

area. These organizations have joined together to raise awareness of caregiving<br />

concerns and to coordinate information and resources that families need in caring<br />

for those who are ill or incapacitated.<br />

Several of the Coalition organizations usually offer their own annual education<br />

programs, but this year we are joining efforts to offer one large conference to the entire<br />

tri-state area. The conference will be held at the Manor House in Mason, Ohio<br />

on Wednesday, Oct. 14 and Thursday, Oct. 15 and consists of many informative<br />

workshops, keynote speakers and community resource exhibits. More details will<br />

follow in a later issue of Cornerstone. This conference will replace the Alzheimer’s<br />

<strong>Association</strong> annual Symposium in 2009.<br />

Difficult Transitions: Long-Term Care Placement<br />

one person is delivering the message. For others, a one-on-one conversation is<br />

most helpful.<br />

• Decide WHEN to tell your family member about the move.<br />

Think about times in the past when you talked about an event ahead of time.<br />

Did it help prepare your family member for the event or did it cause unnecessary<br />

anxiety? Some families choose to tell their family member about the move only<br />

a few hours before to avoid repetitive questions and prolonged anxiety. Others<br />

may tell the person a few weeks or days ahead and allow their family member to<br />

participate, at some level, in planning and preparing for the move.<br />

• Decide WHAT to tell your family member about the move.<br />

A short, simple and consistent explanation is usually best for an individual with<br />

memory loss. Too many details can be confusing for your family member. Using<br />

the same words in every answer may be comforting and help your family member<br />

grasp the information being shared. Make sure that everyone involved in the<br />

move gives the same response each time and understands the importance of being<br />

consistent to avoid further confusion. Also, acknowledge your family member’s<br />

feelings of anger or loss over the move and offer reassurance.<br />

Plan Ahead For Safety During Summer Months<br />

ummer is a time of traveling and outdoor events that can bring enjoyment to<br />

families, but can also bring anxiety and fear if your loved one with Alzheimer’s<br />

disease or dementia becomes lost. When a memory-impaired individual is missing,<br />

a quick response is critical in locating, identifying and returning the person<br />

safely. The MedicAlert® + Alzheimer’s <strong>Association</strong> Safe Return® program can help!<br />

MedicAlert + Safe Return is a 24-hour nationwide emergency response service to<br />

assist individuals with dementia who become lost or have a medical emergency. The<br />

program provides a stainless steel identification bracelet or necklace, wallet card and<br />

other materials. These identification products include:<br />

• a 24-hour emergency response phone number<br />

• the term “memory impaired” and other critical medical condition or allergy<br />

• an identification number specific to the individual<br />

When someone in the community discovers a missing person and calls the emergency<br />

number, the MedicAlert operator accesses a database and immediately notifies<br />

the primary contact person that the individual has been found. The system also allows<br />

a caregiver to report an individual missing, which initiates a fax alert system to local law<br />

enforcement and the local chapter of the Alzheimer’s <strong>Association</strong>.<br />

There is an initial fee of $49.95 to enroll in MedicAlert + Safe Return and an annual<br />

fee of $25.<br />

If your family member is already registered in the MedicAlert + Safe Return<br />

program and you plan to travel this summer, please call the registration line at 1-888-<br />

572-8566 to inform them of where you can be reached. Contact names and phone<br />

numbers can be temporarily changed so that you can be reached immediately in case<br />

of a wandering incident while you are traveling.<br />

Some Facts about Wandering Behavior and Alzheimer’s Disease:<br />

• Wandering is aimless or purposeful movement that can cause a person with<br />

Alzheimer’s Disease (AD) to become lost, leave a safe environment or intrude<br />

in inappropriate places.<br />

• Approximately 60% of people with AD will wander or become lost at some<br />

point during the course of the disease.<br />

• People with AD not only wander by foot, but also by car or other modes of<br />

transportation.<br />

• People with AD are usually found within a half-mile radius from where they<br />

disappeared.<br />

• 46% of people with AD may die if not found within 24 hours.<br />

• People with AD do not usually cry out for help.<br />

• People with AD do not usually respond to shouts.<br />

• People with AD may be in search of something from their past, i.e., a former<br />

workplace or a childhood home.<br />

Milne and Ellison Join Program Staff<br />

nhancing its family support and educational services, the Alzheimer’s <strong>Association</strong><br />

of Greater Cincinnati recently welcomed two new members to its<br />

program staff.<br />

Janet Milne, MSN, RN, CNP, was appointed the Chapter’s Director of Family<br />

and Professional Education in March while Susan Ellison, RN, was named Branch<br />

Program Manager in Northern Kentucky in January.<br />

“Janet and Susan are dedicated professionals who have many well established relationships<br />

and associations with key providers in the healthcare and aging network,”<br />

said Chapter Executive Director Clarissa Rentz. “We are very pleased to have them<br />

both on staff.”<br />

A native of Cincinnati, Ms. Milne earned a Masters in Nursing from the University<br />

of Kentucky. She has taught in a variety of academic settings and is familiar<br />

with all aspects of curriculum development and evaluation. She has also worked in a<br />

variety of acute and long term care settings, administering to those aging individuals<br />

who are dealing with critical, life threatening and end stage disease processes. Before<br />

joining the Alzheimer’s <strong>Association</strong>, she was a palliative care nurse practitioner at The<br />

An African-American Alzheimer’s Family Support Group meets the third Monday<br />

A native of Northern Kentucky, Ms. Ellison will coordinate educational and<br />

support programs in the nine Kentucky counties within the Chapter’s service<br />

area. A graduate of Good Samaritan School of Nursing, she also works part-time<br />

at St. Elizabeth Medical Center.<br />

“I am very excited to be part of the Alzheimer’s <strong>Association</strong> and the chance to<br />

serve families in my home area,” said Ms. Ellison.<br />

Ms. Ellison and her husband, Glenn, reside in Independence. The couple has<br />

six children and two grandchildren.<br />

Ginkgo Biloba Does Not<br />

Prevent Alzheimer’s Disease<br />

Ginkgo biloba has no impact on the development of Alzheimer’s and<br />

dementia, according to study results published in the November issue of the<br />

Journal of the American Medical <strong>Association</strong> (JAMA).<br />

Ginkgo was tested because it was thought to have antioxidant and other<br />

properties that might preserve memory. The Chinese have used the leaves of<br />

the ginkgo tree for thousands of years to treat diseases of the heart and lungs<br />

and to “benefit the brain.” Today ginkgo biloba is one of the most extensively<br />

researched herbs in the world and is commonly prescribed by doctors in<br />

Europe.<br />

The Ginkgo Evaluation of Memory (GEM) Study was a large multicenter<br />

trial led by researchers at the University of Pittsburgh School of Medicine.<br />

The study was conducted at five leading medical centers between 2<strong>000</strong> and<br />

2008. The goal of the study was to find out if medicine made from the plant<br />

ginkgo biloba could prevent or delay changes in memory, thinking, and<br />

personality as people got older. It was the first study designed to have the<br />

necessary participant numbers and monitoring years to be able to measure<br />

ginkgo’s effectiveness and safety profile in Alzheimer’s and dementia prevention.<br />

More than 3,<strong>000</strong> people age 75 or older who had either no dementia or mild<br />

cognitive impairment were randomly assigned to take twice-daily doses of either<br />

120 milligrams of ginkgo biloba extract or a placebo.<br />

The researchers found no statistical difference in dementia or Alzheimer’s<br />

rates between the groups. Among those taking ginkgo, 277 developed dementia.<br />

Among those in the placebo group, 246 developed dementia. Mortality rates<br />

were also similar. According to the researchers, it is possible that an effect would<br />

have been observed if the study had gone on longer because it takes many years<br />

to progress from initial brain changes to clinical dementia. As such, the research<br />

team intends to conduct a follow-up analysis of brain function and structure<br />

in a subset of the participants using magnetic resonance scanning and in some<br />

cases positron emission tomography (PET) scans.<br />

The trial was funded by the National Center for Complementary and<br />

Alternative Medicine and the National Institute on Aging, with support from<br />

the National Heart, Lung and Blood Institute. A similarly-sized trial assessing<br />

the effectiveness of ginkgo is under way in Europe.<br />

While growing up in Scioto County as one<br />

of eight children, she and her twin sister would volunteer<br />

from time to time with their mother at a senior<br />

retirement center - her father’s place of employment.<br />

By the time she was in high school, Jamie was<br />

volunteering each summer at the center.<br />

“I think my parents (James and Wilma Rockwell)<br />

were a strong influence on how I developed an<br />

interest in working with seniors,” said Ms. Williams,<br />

who is Activities coordinator at the United Scioto Senior<br />

Activities (USSA) Adult Day Center in Portsmouth.<br />

A graduate of Shawnee State University with a degree in Social Science, Ms. Williams<br />

was first introduced to the Alzheimer’s <strong>Association</strong> about nine years ago when the<br />

Adult Day Center began offering respite service for families attending the <strong>Association</strong>’s<br />

local Family Support Group meetings.<br />

A few years later, when Portsmouth hosted its first Alzheimer’s Memory Walk, she<br />

was one of the workers who volunteered to work on a Saturday to provide day care for<br />

affected individuals so that interested family members could participate in the walk. Two<br />

Vitamin B Does Not Slow<br />

Cognitive Decline<br />

n a study published in the Journal of the American Medical <strong>Association</strong><br />

(JAMA) last October, researchers from the Alzheimer’s Disease Cooperative<br />

Study conducted a multicenter, randomized, placebo-controlled clinical<br />

trial to determine if reduction of homocysteine levels with high-dose folic acid,<br />

vitamin B6, and vitamin B12 supplementation for 18 months would slow the<br />

rate of cognitive decline in individuals with mild to moderate Alzheimer’s.<br />

Many studies suggest that elevated homocysteine is characteristic of<br />

Alzheimer’s disease, and laboratory research has associated homocysteine with<br />

Alzheimer’s plaques and tangles, though exactly how they are connected in not<br />

yet clear. Elevated homocysteine is also associated with vascular disease and<br />

stroke.<br />

In this study, a total of 340 participants (202 in the active treatment<br />

group and 138 in the placebo group) completed the trial while taking study<br />

medication. Although the vitamin supplement regimen was effective in<br />

reducing homocysteine levels, it had no beneficial effect on the primary<br />

cognitive measure, rate<br />

of change in ADAS-cog<br />

score during 18 months,<br />

or on any secondary<br />

years later, she helped organize the USSA Adult Day Center team and eventually became<br />

a member of the Portsmouth Memory Walk planning committee. Last year, her team<br />

raised more than $4,500, earning the honor of top fundraising team.<br />

“It is such a joy to have Jamie on the planning committee,” said Special Events Coordinator<br />

Marcy Hawkins. “For the past few years Jamie has even opened up the Adult<br />

Day Center as a local Memory Walk headquarters. She has been a huge help to us.”<br />

Ms. Williams said the growth in the walk has been a community effort.<br />

“Even though this is a rural area and the economy has been difficult the past couple<br />

of years, we have improved our fundraising each year,” she said. “I think as more people<br />

have become aware of the Family Support Group, the Memories in the Making© program<br />

and the services offered by the Alzheimer’s <strong>Association</strong> in Portsmouth, the more<br />

support has grown. People really see how it is benefitting our families locally.”<br />

As a leadership volunteer, Ms. Williams helps secure event sponsorships, assists in<br />

team recruitment and anything else that is asked of her, according to Ms. Hawkins.<br />

“Jamie shows great dedication and passion for our mission,” she said.<br />

Jamie and her husband, Aaron, reside in Portsmouth. For his part, Mr. Williams has<br />

become an active member of the Memory Walk team.<br />

A bequest by will or revocable trust can be tailored to complement<br />

your personal lifestyle and financial goals and also support the Alzheimer’s <strong>Association</strong><br />

of Greater Cincinnati. If you have not already done so, please consider putting the<br />

Alzheimer’s <strong>Association</strong> in your will or revocable trust.<br />

By making a bequest to the Alzheimer’s <strong>Association</strong> you can actually preserve more<br />

of your estate for the benefit of your loved ones while helping other families by investing<br />

in the Chapter’s future programs and services. A bequest is simple to create, if you<br />

already have a will or trust it is easy to add a codicil that names the Alzheimer’s <strong>Association</strong><br />

of Greater Cincinnati as a beneficiary.<br />

Will my gift be deductible? A charitable bequest or trust distribution is deductible<br />

for federal estate tax purposes, and there is no limit on the deduction your estate<br />

can claim. In addition, the gift is usually exempt from state inheritance taxes.<br />

What if I’ve already written my will or trust? You can amend a will or trust<br />

to make a gift without rewriting the entire document. Your attorney can prepare the<br />

simple statement, called a codicil that adds a new bequest to us while reaffirming the<br />

other terms of your will. Similarly, he or she can prepare an amendment to your revocable<br />

trust to add us as a beneficiary.<br />

What’s the difference between a will and a trust?<br />

• A will is your instruction manual to your survivors about how you want your<br />

property distributed. It’s a revocable, private document that only takes effect<br />

after your death.<br />

• A revocable trust (sometimes called a living trust) is a legal entity that holds<br />

assets during your lifetime, then transfers ownership of them — or benefit<br />

from them — upon your death. Unlike a will, a trust must take title to<br />

assets before it can pass them to your survivors.<br />

There is no difference between wills and revocable trusts in how transfers from<br />

them are taxed. In some states, however, the probate and distribution process is simpler<br />

with a revocable trust. Your financial advisors can guide you in choosing which vehicle<br />

will work better for you.<br />

ou certainly don’t have to be a political analyst to<br />

know what has been uppermost in the minds of<br />

most Americans over the past year.<br />

Actually, pervasive concerns over the economy date<br />

back to the summer of 2007 when oil prices began to spike<br />

and grumblings over an unsustainable housing market grew<br />

in earnest. At the same time, an increasing number of states<br />

began experiencing a measurable drop in tax revenue and the<br />

need to dip into reserve funds to balance their annual operating<br />

budgets.<br />

But even the most pessimistic economists at the time could<br />

not predict the depth of the current crisis.<br />

Over the past six months, we have been bombarded with a<br />

litany of bad economic news – a faltering credit and bank<br />

system; plunging stock prices, rising unemployment, foreclosures and bankruptcies.<br />

Just as troubling has been the loss of retirement savings through the diminished value<br />

of investment accounts and pensions for many Americans, especially for those near or<br />

in retirement.<br />

Whether it be Washington, D.C., Columbus or Frankfort, the state of the<br />

economy is clearly affecting every decision being made by our legislators.<br />

So in this current climate, some may ask if the message of the Alzheimer’s <strong>Association</strong><br />

is doomed to be relegated to a political “back seat” when it comes to legislative<br />

priorities. The answer to that question may well depend on the effectiveness and level<br />

of commitment of the <strong>Association</strong> and our advocates in conveying our message to<br />

Congress and state legislators.<br />

Despite the poor economy, there are a number of encouraging signs that Alzheimer’s<br />

disease remains a top health care priority for our lawmakers. They include:<br />

• For the first time in nearly seven years, federal funding of Alzheimer research<br />

is expected to be increased.<br />

• A growing number of states, including Kentucky, have adopted formal<br />

Alzheimer’s State Plans to help address the current and future challenges that<br />

Alzheimer’s disease poses to state health care systems and the population at<br />

large.<br />

• In Ohio, the Alzheimer’s Respite Line Item has provided millions of dollars<br />

in support of essential education and respite programs (many provided by<br />

<strong>Association</strong> chapters) for more than a decade. This year, Gov. Ted Strickland<br />

and the Ohio Department of Aging have been strong in advocating for no<br />

major cuts to that funding in the next biennium budget.<br />

As an <strong>Association</strong> we continue to build a stronger and better informed<br />

network of advocates - both locally and nationally – and the results have been<br />

significant. But if we are to truly succeed in our mission of advocacy on behalf of<br />

the millions of families touched by this disease today and the many more to follow,<br />

our base of advocates must grow larger and our message more effectively delivered.<br />

Today, more than ever, your voice needs to be heard.<br />

A Commonwealth Cause<br />

In an effort to educate state lawmakers and increase public awareness<br />

about Alzheimer’s disease in Kentucky, the Alzheimer’s <strong>Association</strong> conducted<br />

its third annual Kentucky Alzheimer’s Advocacy Day and Rally at the<br />

Kentucky Statehouse in Frankfort on Feb. 11.<br />

This year’s theme was “Alzheimer’s<br />

Touches Everyone” and to make the<br />

point, advocates wore gloves to show<br />

their support. More than 120 Alzheimer’s<br />

advocates from across the state participated<br />

in the day-long event. Pictured,<br />

Forget Me Not Classic Bike Ride – Sunday, May 31 (3rd party event)<br />

Be a part of this exciting new event! Over 150 volunteers are needed<br />

to make this event a success! Assist with registration on Friday or<br />

Saturday or be a part of the race day team helping with anything from<br />

event set-up to route marking, or provide support at bike rest stops.<br />

The race starts at Little Miami High School in Morrow.<br />

“Terri has been a consistent, loyal supporter of our chapter for years,”<br />

said Ms. Rentz, who presented the award to Ms. Groteke. “In her own quiet<br />

way, she has given so much to us.”<br />

In addition to her long-time service to the board of directors, Ms.<br />

Groteke has served on the chapter’s finance and executive committees and has<br />

been a long-time supporter of various fundraising events.<br />

The Chapter also honored Ben Koenig and Geoff Smith for their<br />

outstanding contributions to the chapter.<br />

Ben Koenig of Cincinnati was presented the John Horn “Roll Up Your<br />

Sleeves” Memorial Award for his fundraising efforts on behalf of the annual<br />

Memory Walk. Mr. Koenig’s late wife, Ruth, had Alzheimer’s disease and his<br />

participation in the event has been in honor of her.<br />

“Ben has been one of the top individual fundraisers for our Memory<br />

Walk for a number of years. His dedication and enthusiasm serves as a real<br />

inspiration to all of us involved in the walk,” said Marcy Hawkins, Special<br />

Events coordinator.<br />

Professional consultant Geoff Smith of Cincinnati was presented the<br />

Elizabeth Bolles Outstanding Service Award for his work in facilitating the<br />

creation of a new strategic plan for the chapter.<br />

“Geoff’s generous and significant gift of time and expertise allowed our<br />

board of directors and senior management team to create a strategic plan that<br />

clearly defines our vision as a chapter over the next three years and beyond,”<br />

said Ms. Rentz.<br />
