ACCIDENTAL DEATH & DISMEMBERMENT (AD&D)<br />

Coverage is effective on your first day of work. See Benefit <strong>Enrollment</strong> Information page for details on<br />

how to enroll for benefit coverage. The cost of the coverage is listed on the following page. On your<br />

second day of employment, come prepared to complete your beneficiary designation forms.<br />

If you waive coverage or fail to enroll within two months from your start date, you will receive no<br />

coverage for the current calendar year, and you must wait until the next annual enrollment period to<br />

enroll for coverage.<br />

Plan Highlights<br />

Accidental Death and Dismemberment (AD&D) provides benefits for certain injuries or death resulting<br />

from a covered accident. The following coverage is available to you and your spouse or domestic<br />

partner:<br />

� AD&D coverage for yourself, in increments of $10,000 – up to $500,000 (not to exceed 10 times<br />

your base salary).<br />

� AD&D coverage for your spouse or domestic partner, in increments of $10,000 – up to $300,000<br />

(not to exceed the employee coverage level).<br />

� AD&D <strong>Employee</strong> coverage includes automatic coverage for your dependent children, equal to<br />

10 percent of your coverage level, for each dependent.<br />

Plan <strong>Benefits</strong><br />

In the event of a covered accident resulting in a loss, the benefit will be paid based on the greatest loss<br />

suffered in the accident, according to the chart below:<br />

Permanent and total disability 100 percent<br />

(less any other benefit paid out<br />

Two or more of the following: hand, foot, sight of an eye 100 percent<br />

Total paralysis of both upper and lower limbs 100 percent<br />

Total paralysis of both lower limbs 75 percent<br />

One of the following: hand, foot, sight of an eye, speech, hearing<br />

(both ears)<br />

50 percent<br />

Hearing of one ear, or thumb and index finger of the same hand 25 percent<br />
