BASIC & OPTIONAL LIFE
Coverage is effective on your first day of work. See Benefit Enrollment Information page for details on
how to enroll for Optional Life coverage.
Dow Corning provides Basic Life Insurance (equal to one times your base salary, rounded up to the next
thousand) at no cost to you. Optional Life Insurance is also available to help provide financial security
for your family. The cost of Optional Life is listed on the following page. On your second day of
employment, come prepared to complete your beneficiary designation form for Basic and Optional Life
coverage. There is only one form for Basic and Optional Life, so you must complete it even if you
chose to waive Optional Life coverage.
If you waive coverage or fail to enroll for Optional Life Insurance within two months from your start
date, you will receive no additional coverage for the current calendar year, and you must wait until the
next annual enrollment period to enroll for coverage. At that time Evidence of Insurability (EOI) would
be required.
Plan Highlights
You may purchase Optional Life Insurance in increments of one times your annual base salary, up to a
maximum of eight times your annual base salary. If you wish to enroll for six to eight times your base
salary, you will be required to provide Evidence of Insurability (EOI). A representative from the
HR Service Center will follow up with you regarding the required forms.
