The University of La Verne will provide three ceremonies for our Spring 2009 Bachelor and Master degree
candidates. In order to confirm which ceremony in which you have been assigned to participate, please refer to the
Commencement Ceremony/Degree Chart at http://www.ulv.edu/commencement-spring/event/ceremonies.phtml. You
must review the chart to assist you in ordering the correct graduation announcements for your ceremony. Please fill out
this form to order your graduation announcement package for the Spring 2009 Commencement Ceremony. You may fax,
mail or email the information with this form and your check for $40.00 (tax included). To have them shipped to you via
