1. SUPPORT AGENCY UIC. Enter the UIC of the support activity that receives, holds, or works on the item.<br />

2. DATE. Enter the Julian date the form was initiated.<br />

3. ORGANIZATION UIC. Enter the UIC of the owning unit or organization.<br />

4. WARRANTY/EIR EXHIBIT/TMDE/OTHER. Mark the block to show the use of the form. If form is being used for other, print the use of<br />

the form at the bottom of block 2 Date below the Julian date.<br />

5. NSN. Enter the NSN of the item.<br />

6. NOUN NOMENCLATURE. Print the noun abbreviation of the item to be repaired.<br />

7. PD. Enter the priority designator (PD) that applies to the action. <strong>The</strong> unit or organization listed in block 3 normally assigns the PD.<br />

When the form supports a customer maintenance request, use the PD of the maintenance request.<br />

8. PD AUTHENTICATION. <strong>The</strong> commander or the commander’s designated representative signs when a PD of 01 through 08 is in block<br />

7.<br />

9. END ITEM NOUN NOMENCLATURE. Enter the noun nomenclature of the end item or end item from which the component came.<br />

10. MODEL. Enter the model of the end item or end item from which the component came.<br />

11. SERIAL NO. Enter the serial number of the end item or end item from which the component came.<br />

12. DEFICIENCY OR SYMPTOM. Enter the deficiency or symptom that caused this maintenance transaction to occur.<br />

Blocks 13-19. To be filled in by the maintenance support unit.<br />

13. DATE ACCEPTED. Enter the Julian date when the item was accepted.<br />

14. SIGNATURE. <strong>The</strong> shop office representative or inspector responsible for repairs signs here.<br />

15. NMCS. Enter yes or no if this part of the component causes the end item to be not mission capable.<br />

16. WON. Enter the work order number that was submitted with the item.<br />

17. INITIALS. <strong>The</strong> shop foreman enters his/her initials validating that the information on the form is correct and accepting the item.<br />

18. DATE REPAIRED. Enter the date that the repair is completed or the warranty claim or EIR exhibit are released.<br />

19. INITIALS. <strong>The</strong> shop foreman enters his/her initials when the repairs have been completed or the warranty claim or EIR exhibit are<br />

released.<br />
