3–8. DA Form 2402 (<strong>Maintenance</strong> Tag)<br />

a. Purpose. DA Form 2402 serves as an identification tag.<br />

b. Use.<br />

(1) To identify items held for warranty claims.<br />

(2) To identify EIR exhibits.<br />

(3) As a receipt for TMDE items needing calibration.<br />

(4) To identify other items as needed.<br />

c. General instructions. A separate DA Form 2402 is used for each item (fig 3–10). DA Form 2402 has four copies<br />

and is used as follows:<br />

(1) Copy one is normally used as a unit receipt.<br />

(2) Copy two is a receipt for the battalion level except for warranty claim items. When DA Form 2402 is used to<br />

identify or show action completed on a warranty item or claim exhibit, copy two is sent to the supporting warranty<br />

control office (WARCO). <strong>The</strong> WARCO uses DA Form 2402 to close out or complete any needed warranty actions or<br />

claims.<br />

(3) Copy three serves as a receipt for support units.<br />

(4) Copy four stays with the item until it is repaired and reissued. After repair is done, the tag identifies the item as<br />

fixed. This form is attached to each item sent to supporting maintenance shops (direct support (DS), general support<br />

(GS), depot, or a contractor for warranty repairs).<br />

(5) Depending on the item, repair needed, and level of work, not all copies may be needed.<br />

d. Disposition.<br />

(1) DA Form 2402 is destroyed when the part or component it applies to is installed or disposed of.<br />

(2) After the action is completed, copies used as a receipt are destroyed.<br />

(3) When DA Form 2402 identifies a warranty claim or SF 368 (Product Quality Deficiency Report) exhibit, DA<br />

Form 2402 stays with the equipment until the item is no longer needed for a claim or exhibit.<br />

3–9. DD Form 314<br />

a. Purpose. DD Form 314 is a record of scheduled and performed unit maintenance including lubrication services,<br />

annual small arms weapons gauging, borescope and pullover gauging, and 180-day verification maintenance. It also<br />

records (NMC maintenance (NMCM)/NMCS) time; except for missile system/missile subsystem and FAA flight check<br />

data of ATC navigational aids.<br />

b. Use. DD Form 314 is used for the following actions.<br />

(1) Schedule. Periodic services on equipment, to include components in a system or subsystem, are scheduled when<br />

the technical manual requires a PMCS service to be performed by unit maintenance personnel. This form is also used<br />

to schedule the following services performed under the supervision of unit maintenance personnel:<br />

(a) All nonoperator services are scheduled one service in advance.<br />

(b) <strong>The</strong> next scheduled due date may fall in the following year. In that case, enter the date, miles, and hours due in<br />

the Remarks block until a new DD Form 314 is started.<br />

(c) Weekends and holidays may be blocked out. When these are blocked out, services are scheduled on the next<br />

working day.<br />

(d) <strong>The</strong> following symbols are used to show the type of service scheduled:<br />

1. T—any test.<br />

2. I—any inspection.<br />

3. L—lubrication.<br />

4. R—recoil exercise.<br />

5. W—weekly service.<br />

6. M—monthly (1 month) service.<br />

7. Q—quarterly (3 months) service.<br />

8. S—semiannual (6 months) service.<br />

9. A—annual (1 year) (12 months) service.<br />

10. E—18 months service.<br />

11. B—biennial (2 years) service.<br />

12. F—quadrennial (4 years) service.<br />

13. H—tire rotation/inspection.<br />

14. Z—oil sampling.<br />

(e) <strong>The</strong> symbol L is used for all periodic lubes required by a lubrication order (LO). <strong>The</strong> interval block on an LO<br />

only tells when to schedule the lubes. It does not tell what services to schedule or symbol to use.<br />

(f) Miles, kilometers, or hours between services are obtained from the TM and/or LO.<br />

(g) Other symbols or subsymbols may be used as long as they do not conflict with the symbols required by this<br />
