DATE (YYMMDD). <strong>The</strong> dispatcher puts the date the form is started. <strong>The</strong> date is reflected as two places for the year, two for the<br />

month, and two for the day (for example, 970201).<br />

TYPE OF EQUIPMENT. <strong>The</strong> dispatcher enters the equipment noun and model.<br />
