DATE DISPATCHED. <strong>The</strong> date equipment is dispatched. ULLS default entry.<br />

TIME DISPATCHED. <strong>The</strong> military time equipment is dispatched. ULLS default entry.<br />

EQUIPMENT DATA. Administrative number, equipment model number, equipment noun, equipment NSN, equipment serial number,<br />

registration number, equipment license number, and key number are retrieved automatically from equipment data file; no entries are<br />

required from the operator.<br />

SERVICE DUE DATA. Information in this section is retrieved automatically from the ULLS database; no entries required from the<br />

operator. Operator/supervisor reviews this section and takes appropriate actions as required.<br />

DISPATCH INFORMATION<br />

OFFICIAL USER NAME/PHONE NUM. <strong>The</strong> operator requesting the dispatch provides the last name, first name, middle initial, rank/<br />

grade, and telephone number to the dispatcher. Dispatcher enters the name of the person to whom the operator is to report (official<br />

user). This person is responsible for the equipment when in use.<br />

DESTINATION. <strong>The</strong> dispatcher enters into the ULLS system the location of dispatch area.<br />

EXPECTED DATE/TIME OF RETURN. Dispatcher enters close of business or the actual time the user expects to return with the<br />

equipment. <strong>The</strong> operator ensures the dispatcher reviews the expected date/time of return. If equipment cannot be returned because of<br />

mission, operator notifies the official user who requests an extended dispatch.<br />

EQUIP DISPATCHER’S SIGNATURE. <strong>The</strong> dispatcher signs his/her name.<br />

1ST OPERATOR’S SIGNATURE. <strong>The</strong> operator signs his/her name. If operators are changed while the vehicle is dispatched, annotate<br />

the hours/miles/kilometers on the equipment to the right of the operator’s signature.<br />

2ND OPERATOR’S SIGNATURE. This line will be used if operators are changed while the equipment is on dispatch. This normally<br />

happens when an operator becomes sick or overly tired (for example, during convoy operations). <strong>The</strong> operator signs his/her name.<br />

AUTHORIZATION. This block/area can be used for off post authorization. <strong>The</strong> commander or the commander’s designated<br />

representative signs and enters rank for off post travel. For second or third operator authorization: When a situation occurs requiring a<br />

change of operators and there is no dispatcher present to verify a drivers qualifications, enter the name and rank of person authorizing<br />

the change or modification.<br />

RELEASED BY. Official user will sign when equipment is returned and the mission is completed.<br />

DISPATCH OUT REMARKS. <strong>The</strong> dispatcher enters all towed equipment by the prime mover. If equipment was extended the operator<br />

writes the words EXTENDED DISPATCH, the name and rank/grade of the person authorizing the extension, and expected date of<br />

return. <strong>The</strong> official user or the commander’s designated representative signs and enters rank when operator is released or mission is<br />

completed.<br />

END ITEM USAGE DATA. Equipment noun, miles/hours/kilometers and current reading are ULLS- generated entries.<br />

EQUIPMENT NOUN. Displays the equipment noun.<br />

M/H/K. This displays how equipment is tracked, by either miles/hours/kilometers.<br />

CURRENT READING. Displays the reading of previous block, in miles/hours/kilometers, prior to dispatch.<br />

READING AT RETURN. Operator enters this at time of return.<br />

FUEL USAGE (IN GALLONS). <strong>The</strong> operator enters the amount of fuel in gallons added while the equipment was on dispatch.<br />

COMPONENT(S) USAGE DATA. Components serial number, noun, miles/hours/kilometers, and current reading are ULLS-generated<br />

entries.<br />

READING AT RETURN. <strong>The</strong> operator enters reading when the equipment is returned. If the miles/hours/kilometers meter is broken or<br />

missing, estimate the miles/hours/kilometers used on equipment.<br />

OIL ADDED (IN QUARTS). <strong>The</strong> operator enters the amount of oil in quarts added while the equipment was on dispatch.<br />
