1 Operator and supervisor or leader names are used for two purposes. If the folder is lost or misplaced, the finder has names to track down. Most<br />

important, those names show who is responsible for the equipment, the forms in the folder, and the information on the equipments condition.<br />

2 <strong>The</strong> back of the card may be used for locally required information. For example, if the command asks for a monthly mileage report, enter the start and<br />

end dates and miles on the card in pencil. Miles traveled are shown on DD Form 1970.<br />

1. BUMPER NO. Enter the bumper number of the end item or equipment.<br />

2. MODEL. Enter the model number.<br />

3. NOUN. Enter the noun or noun abbreviation.<br />

4. NSN. List the end item NSN.<br />

5. SERIAL NO. List the serial number for the equipment. For equipment managed by registration number, enter the item’s registration<br />

number on the card.<br />

6. AOAP SAMPLE. Enter the date and hours the next AOAP sample is due. Get this information from the equipments DD Form 314 or<br />

AOAP lab printout. When making this entry, only use pencil. This entry is only needed for equipment under AOAP.<br />

7. NEXT SERVICE AT. Enter the date and/or miles, kilometers, or hours when the next scheduled service is due on the equipment.<br />

Get this information from DD Form 314. Pencil entry.<br />

8. NEXT LUBE AT. Enter the date and/or miles, kilometers, or hours when the next scheduled lubrication service is due on the<br />

equipment. Pencil entry. Get this information from DD Form 314.<br />

9. OPERATOR. <strong>The</strong> operator’s last name and rank go here. Pencil entry. Leave blank if more than one operator is assigned to the<br />

equipment.<br />

10. SUPERVISOR. Enter the last name and rank of the operators leader or supervisor here. Pencil entry.<br />
