35898–7466. <strong>The</strong> command that manages the item can be found by looking up the item’s NSN on the <strong>Army</strong> Master<br />

Data File. <strong>The</strong> command is identified by the first position of the MATCAT code.<br />

a. a. Request to add equipment includes the following information:<br />

(1) <strong>The</strong> nomenclature of the item.<br />

(2) <strong>The</strong> item’s NSN.<br />

(3) Model or identification number.<br />

(4) Line item number.<br />

(5) Item manager (the command that manages the item).<br />

(6) <strong>The</strong> item’s equipment category code (see table B–18).<br />

(7) Is the item currently in the <strong>Army</strong> inventory? Give the current and projected densities. (Normally the command<br />

that manages the item provides this information.)<br />

(8) Identify the type of historical records required.<br />

(9) Define the purpose of each form to include—<br />

(a) Data to be collected.<br />

(b) Who uses the data.<br />

(c) How long the data must be retained and why.<br />

(d) A picture of the equipment.<br />

b. Request for DA Form 2408–9 information—<br />

(1) Defines the use and purpose for DA Form 2408–9 data being requested.<br />

(2) Defines the data elements required for summaries/reports that must be made on DA Form 2408–9 data that will<br />

be collected. Describe the required format for the intended summaries/reports.<br />

(3) Identifies the frequency for the intended summaries/reports and the activity that is responsible for using the data.<br />

E–6. Usage reporting<br />

Usage reporting for all combat and selected tactical vehicles has been deleted. Usage for these items is now being<br />

obtained through the AOAP. Tactical vehicles not enrolled in the AOAP are still identified in appendix E as DA Form<br />

2408–9 USAGE reportable and is reported as of 1 November of each year. Units with ULLS-G are required to report<br />

usage to LOGSA monthly through ULLS-G using the AMSS end-of-period report process on all equipment with an X<br />

in the columns labeled “24089 USAGE” or “DD2026 USAGE” in figures E–1 through E–4.<br />
