Department of the <strong>Army</strong> List of Items on Which Historical Records are to be Maintained<br />

E–1. Introduction<br />

This appendix explains which equipment requires historical records and which forms to keep. <strong>The</strong>se records and forms<br />

represent required data elements and a unique relationship to either the end item or the component. <strong>The</strong> <strong>Army</strong> uses<br />

web-based databases, AIT devices, and business process redesign to transition from a form-based maintenance system<br />

to a digitized process-based maintenance system in support of <strong>TAMMS</strong>. In addition to required forms listed in this<br />

appendix, DA Form 2408–20 is maintained on equipment enrolled in the AOAP. <strong>The</strong> AOAP equipment is identified in<br />

TB 43–0211. However, if the supporting AOAP laboratory is automated and printout(s) or automated forms are<br />

received reflecting all the data from DA Form 2408–20, DA Form 2408–20 is not required unless directed by local<br />

standing operation procedures.<br />

E–2. Forms<br />

a. Required. Required forms are identified opposite the nomenclature of the equipment found in tables E1 through<br />

E4. DA Form 2408–9 A/T/G/L refers to reporting of acceptance, transfers, losses, gains, and NSN redesignations. DA<br />

Form 2408–9 USAGE refers to usage. DA Form 2408–9 OVHL refers to overhaul reporting, and to the repair of<br />

selected combat and tactical vehicles under the CONUS Tactical Wheeled Vehicle Program and for outside the<br />

continental United States, under the <strong>The</strong>ater Intermediate General Support Repair Program (formerly <strong>The</strong> <strong>The</strong>ater<br />

<strong>Army</strong> Repair Program), or other DA-approved programs requiring DA Form 2408–9 reports. All track and wheeled<br />

items of equipment subject to the U.S. <strong>Army</strong> Vehicle Registration Program are required to submit DA Forms 2408–9<br />

per chapter 5 of both AR 710–3 and this pamphlet. <strong>The</strong>se items have been included in appendix E of this publication.<br />

When AIT devices are embedded on end items and components and can be leveraged by the GCSSA <strong>Maintenance</strong><br />

Module, paper-based form requirements listed in this section are no longer required.<br />

b. Forms information. <strong>The</strong> purpose, detailed requirements, use, preparation, processing and disposition of DA forms<br />

are contained in other chapters of this pamphlet.<br />

c. <strong>TAMMS</strong> DA Forms. Commanders may direct the preparation of <strong>TAMMS</strong> DA forms not listed for equipment for<br />

local management purposes. However, all copies of forms used is retained within the command.<br />

d. Initiation of DA Form 2408–5. DA Form 2408–5 is initiated only upon notification of the first published<br />

DAMWO.<br />

e. When to start a form. Do not start a form until entries are required on it.<br />

and arranged by ECC. <strong>The</strong> column headings are ECC, NOUN, MODEL, LIN, NSN, EIC, REG# REQD, and FORMS<br />

REQUIRED. <strong>The</strong> ECC is a two character alpha designation that shows the general and subgroupings for the items (see<br />

table B–18).<br />

E–4. Railway equipment and nontactical wheeled vehicles<br />

Railway equipment and nontactical wheeled vehicles are listed in figures E–3 and E–4 and are arranged by ECC/LIN/<br />

NSN. <strong>The</strong> column headings are ECC, NOUN, LIN, NSN, EIC, REG# REQD, and FORMS REQUIRED.<br />

E–5. Requests to add equipment<br />

Requests to add equipment to appendix E is submitted in writing through the AMC major subordinate command that<br />
