22E. Enter all the details of what was happening before the equipment failed. <strong>The</strong>se details help the investigators. Be sure to tell if any<br />

modifications are directly related to the problem. Any other information that might help the investigator should also be give.<br />

22F. Provide a brief, but thorough description of the problem.<br />

22G. Outline the most likely cause of the problem.<br />

22H. Provide a short summary of what was done to correct the problem. If an item is fixed and put back in service, describe the repair.<br />

List exhibit information: exhibit held, destroyed, pictures or drawings made, or item turned into supply.<br />

22I. Provide any suggestions to improve equipment, stop future failures, improve design, and modify or revise instructions. Note: If an SF<br />

368 is sent in as a result of an accident or a safety related, note in block 22 if a DA Form 285 (U.S. <strong>Army</strong> Accident Investigation Report)<br />

has been sent in and give the date it was sent.<br />

22J. Leave blank.<br />

23. Enter the unit name, location, and ZIP Code or APO number where the deficient materiel is located.<br />

NSN, PART NO. AND ITEM DESCRIPTION. Enter the NSN, part number, type, model, series, and item name.<br />

NEXT INSPECTION DUE. Leave blank.<br />

CONDITION CODE. Enter the letter L.<br />

INSPECTION ACTIVITY. Enter the units name or UIC of the activity that is preparing the PQDR/EIR.<br />

REASON OR AUTHORITY. Enter “PQDR/EIR Exhibit.”<br />

SERIAL NUMBER/LOT NO. Enter the items serial number. If the item is bulk (for example, cord, webbing, rubber), enter the lot number.<br />

UNIT OF ISSUE. Enter the unit of issue; for example, each (ea), gross (gr), dozen (dz).<br />

CONTRACT OR PURCHASE ORDER NO. If known, enter the contract number or the purchase order number. Contract number can be<br />

found on item data plate. If it cannot be found, enter UNK.<br />

QUANTITY. Enter the quantity of the item.<br />

INSPECTOR’S NAME OR STAMP AND DATE. Leave blank.<br />

REMARKS. Comment as appropriate.<br />
