. Accidents and incidents with chemical warfare ammunition are reported in accordance with AR 50–6 and<br />

command directives.<br />

8–3. General instructions<br />

a. Responsibility for submission.<br />

(1) Commanding officers of organizations using, handling, or storing ammunition, guided missiles, and large rockets<br />

prepare and submit timely ammunition reports to the appropriate command.<br />

(2) For guided missiles and large rockets, the unit submits a DA Form 2407 for bad components. In addition, a DA<br />

Form 2415 may be required to comply with condition reporting identified in para 8–4.<br />

b. Classification. Reports are classified by the latest security regulations (see AR 380–5 and AR 380–86).<br />

c. Acknowledge of receipt and replies.<br />

(1) Except as noted below, a final reply is given. Replies show the corrective action taken and direct action to be<br />

taken or state that no action is necessary. Replies also give disposition of defective items.<br />

(2) Replies are not given for reports on unserviceable new materiel for which blanket shipping orders have been<br />

issued to return the item to the vendor.<br />

d. Forms. <strong>The</strong> Air Force, Navy, and Defense Nuclear Agency organizations, that use <strong>Army</strong> designed or manufactured<br />

materiel and related instructions, may send in reports on their own service forms.<br />

e. Exhibits.<br />

(1) Exhibits are samples of an item with a discrepancy that is chosen to support materiel reports. <strong>The</strong> use of<br />

photographs, drawings, and supporting data in place of actual materiel exhibits is encouraged.<br />

(2) Tag exhibits held for disposition instructions. A DA Form 2402 is used for tagging exhibits (except for<br />

ammunition lots to which SB 742–1 applies). For SB 742–1 ammunition lots, use DD Form 1575 (Suspended Tag—<br />

Materiel), DD Form 1576 (Test/Modification Tag—Materiel), or DD Form 1577 (Unserviceable (Condemned) Tag—<br />

Materiel) as required.<br />

(3) Mark the forms to identify the reported item and the lot they go with. Keep them in order so they wont get lost<br />

or mixed up. If instructions to return the bad item(s) for checkout are received, mark the shipping paper and materiel<br />

like this: Exhibit for MIF#/DIF#. (For conventional ammunition, the tagged exhibits would be marked for Malfunction<br />

I n v e s t i g a t i o n F i l e o r D e f i c i e n c y I n v e s t i g a t i o n F i l e , n o t a n A C R . O n l y M I F / D I F f i l e s ( n o t A C R s ) f u n d f o r<br />

investigations.<br />

(4) If instructions are not provided within 90 days, dispose of the exhibit(s) in accordance with applicable SOPs,<br />

ARs, and DOD directives. If the exhibit cannot be kept, the activity to which the exhibit is sent must keep the exhibit.<br />

(5) Package exhibits carefully for shipping to prevent any more damage.<br />

(6) Take equipment apart to get exhibits only when that action is within the normal maintenance level.<br />

8–4. DA Form 2415 (Ammunition Condition Report) (RCS CSGLD–1202)<br />

A DA Form 2415 provides management information on unserviceable, and permanently suspended ammunition items.<br />

a. DA Form 2415 is used to report ammunition, and may be initiated as a result of but not limited to the following<br />

actions:<br />

( 1 ) A m m u n i t i o n i n s p e c t i o n : P e r i o d i c i n s p e c t i o n , r e c e i p t i n s p e c t i o n , s a f e t y - i n - s t o r a g e i n s p e c t i o n , a n d s p e c i a l<br />

inspection.<br />

(2) Permanent suspense assignment by owning service.<br />

(3) As specifically requested by higher headquarters.<br />

(4) DA Form 2415 submittal is also contingent on satisfying conditions as established by the appropriate command.<br />

b. A DA Form 2415 is prepared as follows:<br />

(1) For conventional ammunition items, submit all data through the following Web site: https://www6.osc.army.mil/<br />

DemilMaint/MainMenu.asp.<br />

(2) For missile items, submit all data through the following Web site: https://webdesk.redstone.army.mil.<br />

(3) Individual DA Form 2415 is made for each owner of assets being reported. Individual DA Forms are made for<br />

each line item reported. More than one lot number with the same NSN may be reported on the same ACR so long as<br />

associated NSN and ammunition lot number integrity are maintained.<br />

(4) If a continuation sheet is needed to finish a report, a blank sheet of bond paper (8–1/2 by 11 inches) may be<br />

used. Each continuation sheet must be marked with the organizations name. ACR number, and date of report. This<br />

information is placed at the top of each page. Pages will be numbered “Page ## of ## Pages”. For conventional<br />

ammunition items submitted through the Web site link above, attach a narrative, PDF files, and so on, using an e-mail<br />

attachments option.<br />

(4) Instructions for correctly filling out a DA Form 2415 are contained in figure 8–1. (For conventional ammunition<br />

ACRs, see the Web site input instructions in the Help option on the ACR main menu.)<br />
