(c) <strong>The</strong> examples and illustrations should be used as guides only. Read the text and figure instructions, then fill out<br />

the forms showing your unit, equipment, and status. If there is a conflict between the form and the instructions in the<br />

figure, use the instructions.<br />

(d) Commanders may appoint a representative to authenticate/sign some forms and records. When a representative is<br />

appointed, that authority must be in writing on a memorandum, on orders, or on a DA Form 1687 (see DA Pam<br />

710–2–1).<br />

(e) Commanders comply with the disposition instructions provided for each <strong>TAMMS</strong> form and record. A form/<br />

record may be retained in the unit beyond the prescribed period when required locally to assist management or in<br />

special situations. A form is not be retained beyond the prescribed time merely for inspection purposes.<br />

c. General guidance on preparing manual forms.<br />

(1) <strong>The</strong> required forms and records provide a picture of the equipments condition, use, operation, and needs. <strong>The</strong><br />

ultimate purpose of this information is to have the equipment safe and ready for combat.<br />

(2) Operators, dispatchers, records clerks, mechanics, prescribed load list clerks, supervisors, and commanders have<br />

an equal stake in maintaining the forms.<br />

(3) <strong>The</strong> forms and records are not redone just for neatness. Redo historical forms and records, as shown below, only<br />

when the original form is lost or so damaged that the information is no longer readable.<br />

(4) When a historical form is redone, move all the information from the old form to the new one. In the remarks<br />

block of the new form or in the top or bottom margin, print: New Form Initiated and the date. <strong>The</strong> commander or the<br />

commander’s designated representative signs the entry. Enter UNK (unknown) in any block that cannot be read. Throw<br />

away the national maintenance point (NMP) copies of forms made to replace lost or damaged forms. See the following<br />

instructions.<br />

(a) <strong>The</strong>se instructions apply only when the original form was on hand but was lost or damaged.<br />

(b) If equipment requiring a DA Form 2408–9 (Equipment Control Record) arrives in the unit without a form or<br />

there is no record of a DA Form 2408–9 on it, use the instructions in paragraph 5–2a.<br />

(5) If a record is lost, damaged, falsified, or destroyed whether intentionally or through negligence, disciplinary<br />

action may result. <strong>The</strong>se forms and records are required.<br />

Chapter 2<br />

Operational Processes, Forms, Records and Dispatch Procedures<br />

2–1. General procedures<br />

This chapter describes how to plan, manage, fill out and use forms for equipment operation, dispatch, and control of<br />

equipment when using ULLS–G or manual procedures.<br />

a. <strong>The</strong> ULLS is the <strong>Army</strong>’s Unit Level Logistics <strong>System</strong>. ULLS collects operator licensing, dispatch, and equipment<br />

control data and provides management information at the unit level.<br />

b. <strong>The</strong> ULLS–G automates/replaces portions of <strong>TAMMS</strong>. Commanders ensure that portions of <strong>TAMMS</strong> not<br />

replaced by ULLS are accomplished using the manual procedures outlined in this pamphlet. <strong>The</strong> following DA forms/<br />

DD forms /optional forms (OF) have been automated, and ULLS–G generated printouts (shown with an -E) are<br />

authorized replacements:<br />

(1) DA Form 5823 (not required if operating with ULLS–G; this information is on the dispatch printout).<br />

(2) DA Form 5987–E (Motor Equipment Dispatch), DA Form 5987–1E (Alert Motor Equipment Dispatch), and DD<br />

Form 1970 (Motor Equipment Utilization Record).<br />

(3) DA Form 5982–E (Dispatch Control Log) and DA Form 2401 (Organizational Control Record for Equipment).<br />

(4) DA Form 5984–E (Operators Permit Record) (fig 2–5).<br />

c. DA Form 348 (Equipment Qualification Record) is the only authorized record for documenting equipment<br />

operator’s qualifications (fig 2–1). ULLS–G produces a Form 348–E that reflects equipment qualifications that have<br />

been locally transferred into ULLS–G from the DA Form 348; however, this is not an authorized replacement of DA<br />

Form 348. This process was designed to help streamline automated dispatch procedures through the use of a relational<br />

database. <strong>The</strong> ULLS–G generated Form 348–E does not provide the necessary documentation for a commander to<br />

make an appropriate determination for authorization of equipment use. Commanders document equipment qualifications<br />

using the DA Form 348 as prescribed in AR 600–55. Units maintain a copy of all authorized operator’s DA Form<br />

348s on file. <strong>The</strong> ULLS-G generated Form 348–E is used for local purposes only. Units review operator qualification<br />

information quarterly to ensure ULLS–G is synchronized with manning rosters and review training qualifications to<br />

ensure accuracy.<br />

d. Units operating ULLS–G supersede all manual procedures. In cases where there is a conflict on form disposition<br />

between this pamphlet and the user manual, this pamphlet takes precedence.<br />

e. All units, organizations, and activities that operate self-powered vehicles, towed vehicles, and stationary powered<br />
