a. For all nontactical wheeled vehicles, as listed in figure E–4, enter the code that applies to the equipment’s use in this block: A—<strong>Army</strong><br />

operated, includes any <strong>Army</strong>-owned passenger carrying and general purpose vehicles listed in figure E–4, used or operated by <strong>Army</strong><br />

personnel (Government owned, Government operated, or GOGO); B—Contractor operated, includes any <strong>Army</strong>-owned passenger<br />

carrying and general-purpose vehicles listed in figure E–4, that are furnished to contractors by the Department of the <strong>Army</strong> for contractor<br />

use (Government owned, contractor operated, or GOCO); R—Facilities engineering vehicles, includes special purpose (commercial) and<br />

military design vehicles used by installation real property management activities; X—All other special purpose vehicles, includes all other<br />

special purpose vehicles not covered by code R.<br />

b. Leave blank for other equipment.<br />

6. NOMENCLATURE. Enter the noun of the equipment.<br />

7. MODEL. For OTHER than passenger carrying and general-purpose vehicle, enter the model of the item. For passenger-carrying and<br />

general purpose vehicles, this block contains the LIN for the item as listed in figure E–4, or the FED LOG. For watercraft, enter the hull<br />

design number.<br />

8. NATIONAL STOCK NO. Contains the NSN of the item.<br />

9. SERIAL NO.<br />

a. Enter the complete serial number of the item. Make sure to list ALL characters and numbers making up the serial number and to<br />

include any preprinted suffix or prefix.<br />

b. For equipment with no serial number, use the preprinted control number on the form. <strong>The</strong> control number becomes the permanent<br />

serial number for the equipment on all forms and records.<br />

c. For watercraft, enter the hull number.<br />

d. For vehicle mounted weapon systems, like the M113A2 TOW, and so on. Use the vehicle’s serial number.<br />

10. REGISTRATION NO.<br />

a. Enter the registration number assigned to the item. Do not use the alpha character “O” or “I”—always use 0 (zero) or 1 (one).<br />

b. If the equipment is not under the <strong>Army</strong> Vehicle Registration Program, leave blank. If a registration number is needed, contact LOGSA.<br />

c. For watercraft, enter the registration number if one has been assigned. If not, use the hull number.<br />

11. YEAR OF MFG. Enter the four-digit model year, for example, 2004.<br />

a. For OTHER than passenger carrying and general-purpose vehicles, enter the two-digit year the equipment was manufactured. Enter<br />

the letter M before the two numbers. For example, for an item manufactured in 1990, enter M90.<br />

b. For nontactical passenger carrying and general-purpose vehicles, enter the four-digit model year (1998, 1999, and so).<br />

12. MANUFACTURER (MFG Code). Enter the name and five-digit code for the manufacturer here. Manufacturers codes are found in SB<br />

708–43.<br />

13. CONTRACT NO. Enter the contract number under which the item was bought. For passenger carrying and general-purpose vehicles,<br />

this block contains the procurement contract number, for example, DAAE07–71KK-C–005 or GS–00S–05892.<br />

14. PURCHASE ORDER NO. Leave blank.<br />

15. WARRANTY PERIOD. Enter the warranty period as stated in the contract.<br />

16. TYPE REPORT. Leave blank.<br />

17. REPORT CODE. On line a, enter the code for the source of the item in the corresponding box: A—For equipment accepted from the<br />

manufacturer, B—For items accepted from local procurement.<br />

18. USAGE. Leave blank.<br />

19. SHIPPED TO.<br />

a. ORGANIZATION. Leave blank.<br />

b. SHIPPED TO UIC. Leave blank.<br />

20. RECEIVED FROM.<br />

a. ORGANIZATION. Leave blank.<br />

b. For vehicles with radio mounts, identify the installation kit. This information is needed to identify the equipment for local use and when<br />

the equipment is transferred.<br />

22. INSPECTOR’S SIGNATURE. <strong>The</strong> person accepting the item into the <strong>Army</strong> inventory signs and enters his or her telephone number.<br />

23. JULIAN DATE. Enter the four-digit Julian date of acceptance (for example, 4045).<br />
