5–11. Records that go with equipment<br />

a. General. <strong>The</strong> following forms go with the equipment when it is—<br />

(1) Manufactured, assembled, overhauled, or rebuilt. Depots and supply activities make sure the required forms are<br />

with the equipment in an equipment record folder or binder. A copy of DA Form 2404 used to show a technical<br />

inspection on the item also goes with the equipment.<br />

(2) Turned-in or transferred. DD Form 314, as well as equipment records, goes with the equipment.<br />

(3) Sent to Defense Reutilization and Marketing Office (DRMO). <strong>The</strong> organization or activity that has the equipment<br />

just before sending it to the DRMO or salvage handles the forms as follows:<br />

(a) DA Form 2408–9 Loss Report (NMP copy) is sent to the Commander, LOGSA, ATTN: AMXLS-MD, Redstone<br />

Arsenal, AL 35898–7466. Automated systems are now available and operational at LOGSA and are now the preferred<br />

order of submission.<br />

(b) DA Form 2408–20 and DA Form 2408–4 are sent with the equipment.<br />

(c) DA Form 2408–9 logbook copies are destroyed.<br />

(d) A copy of the most current DA Form 5988E/DA Form 2404 is sent with the equipment.<br />

(4) Fired missiles. <strong>The</strong> missile log and all its records are sent to the Commander, AMCOM, Air Defense <strong>System</strong>s or<br />

Land Combat <strong>System</strong>s, ATTN: AMSAM-MMC-LS-MM, Redstone Arsenal, AL 35898.<br />

b. U.S. <strong>Army</strong> equipment on loan. Send the records with equipment listed in appendix E when the equipment is<br />

loaned to other DOD agencies. Send the records with equipment loaned or contracted to other U.S. Government<br />

departments or agencies when directed to do so.<br />

c. U.S. <strong>Army</strong> equipment transferred. U.S. <strong>Army</strong> equipment transferred to other U.S. Government departments and<br />

agencies is handled as follows:<br />

(1) Send records with equipment transferred to other departments or agencies when directed to do so.<br />

(2) DA Form 2408– 9, Loss Report for equipment transferred to other departments or agencies is sent to the<br />

Commander, LOGSA as directed in paragraph 5–11a(3)(a). Automated systems are now available and operational at<br />

LOGSA and are now the preferred order of submission.<br />

d. Military assistance programs.<br />

(1) Records with equipment given to other nations under the military assistance program are sent as follows:<br />

(a) A new DA Form 2408–4 with block 3 blank. All information must be current. <strong>The</strong> old form is sent to<br />

TACOM—Rock Island, AMSTA-LC-CIPPA, Rock Island, IL 61299–6000.<br />

(b) DA Form 2408–9 Loss Report to Commander, LOGSA, ATTN: AMXLS-RRA (<strong>TAMMS</strong>), Redstone Arsenal,<br />

AL 35898–7466, as directed in 5–11a (3)(a).<br />

(c) A DA Form 2408–5 for all MWOs applied.<br />

(2) Any other forms are destroyed.<br />
