c. Special information.<br />

(1) Instructions and use. DA Form 2408–9 instructions and use apply to all <strong>Army</strong> units, organizations, and activities<br />

as shown below.<br />

(a) <strong>Army</strong> depots fill out and send in DA Forms 2408–9 even though the property accounts for depot stock are kept<br />

at the AMC major subordinate command level.<br />

(b) <strong>The</strong> property book officer or the officer accountable for the property of the parent unit makes out DA Form<br />

2408–9. Defense Reutilization and Marketing Offices (DRMOs) are not responsible for completing DA Form 2408–9<br />

Loss Reports. Loss Reports are completed by the accountable officer of the unit of the owning organization prior to<br />

sending the equipment to DRMO. He or she is also responsible for distributing the copies to the appropriate elements<br />

and especially to LOGSA for the inclusion in the TEDB.<br />

(c) For this pamphlet, a parent unit is one that owns equipment authorized under tables of organization and<br />

equipment (TOE) and the property book level for tables of distribution and allowances (TDA). For depot operating and<br />

mission stocks, the depot level is the parent unit. TOE separate companies that are not part of larger units are their own<br />

parent units.<br />

(d) <strong>The</strong> maintenance officer is responsible for distributing copies of DA Form 2408–9 Usage Report when not<br />

reported by OASIS (AOAP) or AMSS.<br />

(e) If a remark is needed on a DA Form 2408–9 because the original form was lost or damaged, move all the<br />

information from the old form to the new form. In the remarks block of the new form, print NEW FORM INITIATED<br />

and the date. If blocks 1, 2, 3, and 4 of the lost or damaged form are unknown, unit information is provided in these<br />

blocks. Throw away the NMP copies of forms made to replace lost or damaged forms.<br />

(f) Serial numbers that have been assigned to an item are not changed during its life cycle, regardless of changes in<br />

configuration. <strong>The</strong> exception to this rule is for the correction of errors resulting in duplication of numbers. Any item<br />

declared as excess does not have its serial numbers destroyed or removed, in accordance with AR 750–1, para 3–6.<br />

(2) Electronic 2408–9. Automated systems are now available and operational at LOGSA and take precedence over<br />

manual reporting of DA Form 2408–9. DA Form 2408–9 is located on the LOGSA Weblog: https://weblog.logsa.army.<br />

mil. Enter WebLOG using the WebLOG icon and look for the Asset <strong>Management</strong> column (right of the web page) and<br />

click on DA Form 2408–9. A password, which can be attained on the Web site by submitting a <strong>System</strong> Access<br />

Request, is required to access DA Form 2408–9 reporting. All data requirements to complete the form either manually<br />

or electronically are outlined in this pamphlet.<br />

(3) Acceptance and registration. A DA Form 2408–9 is used when equipment needing this form is accepted into the<br />

U.S. <strong>Army</strong> inventory as shown below (fig 5–7).<br />

(a) Equipment may be accepted on delivery from a manufacturer.<br />

(b) Normally the Government representative at the manufacturing facility accepts equipment into the <strong>Army</strong> inventory,<br />

and the Acceptance Report is forwarded to LOGSA; however, some equipment may arrive from a manufacturer<br />

without a completed Acceptance Report. When this happens, the organization that discovers the need for the form fills<br />

out, distributes and submits a copy of the DA Form 2408-9 to LOGSA. This also applies when equipment is locally<br />

purchased.<br />

(c) Use tables 5–1 and 5–2 to reduce required data for transmission to LOGSA.<br />
