4. In column 11, PCW (previously complied with) or whatever applies is printed. <strong>The</strong> name of the maintenance<br />

activity that verified that the MWO has been applied is then added.<br />

5. <strong>The</strong> PID of the person who confirmed the MWOs in column 12 is entered.<br />

6. <strong>The</strong> already applied MWO is reported on a DA Form 2407 or using MMIS. Paragraph 3–7 explains how to<br />

handle DA Form 2407.<br />

(f) A completed MWO entry may be found on DA Form 2408–5 when the MWO has not been applied.<br />

1. <strong>The</strong> MWO entry is lined through.<br />

2. <strong>The</strong> MWO information is entered on the next open line in column 8 through 14. In pencil, the calendar date the<br />

MWO is required to be completed is noted in column 10.<br />

3. If an MWO that does not apply has been entered on DA Form 2408–5, the entry is lined through. “Not<br />

applicable” is entered in column 14 on the same line.<br />

4. If a change comes out on an MWO listed on DA Form 2408–5, draw a line through the entry. Enter the new<br />

information on the next open line.<br />

5. When a component or assembly that has a DA Form 2408–5 is removed for any reason to include evacuation to<br />

depot, the form is attached to that item and the form placed in a protective cover.<br />

6. When a component or assembly that has a DA Form 2408–5 is installed, the form is added to the missile system<br />

log.<br />

5–6. Usage reporting through ULLS-G<br />

a. Units operating with ULLS-G SCP 05 or greater are required to submit ground usage to LOGSA monthly using<br />

the AMSS end of period report process.<br />

b. Usage is reported through ULLS-G on all vehicles with a “Type Report Code” of “Y” in the MMDF. Appendix E<br />

of this pamphlet also provides usage-reporting information. Usage is reported through ULLS-G for all equipment with<br />

an X in columns “2408–9 USAGE” or “DD 2026 USAGE” in appendix E. Units review the actual equipment mileage/<br />

hours against the ULLS-G equipment usage report to identify and correct usage data inaccuracies prior to submitting to<br />

higher headquarters.<br />

c. Units produce the AMSS end of period report (which includes the monthly usage report) on the 16 th of each<br />

month, or the closest working day after the 16 th . Active <strong>Army</strong> units, <strong>Army</strong> National Guard, and <strong>Army</strong> Reserve units:<br />

<strong>The</strong> AMSS data must be received by LOGSA no later than midnight central standard time on the 19th day of each<br />

month. Units perform quarterly reconciliations between equipment data shown in units’ ULLS-G equipment data file<br />

and equipment data shown on unit property books to ensure data synchronization and accuracy.<br />

d. ULLS-G units supported by SAMS sends the monthly AMSS end of period report (which includes the monthly<br />

usage report) to the battalion. <strong>The</strong> battalion sends the data to the supporting SAMS site, where the data is forwarded to<br />

LOGSA via telecommunications.<br />

e. Units with ULLS-G that are NOT supported by SAMS send their monthly AMSS end of period report (which<br />

includes the monthly usage report) to the battalion level via e-mail or floppy diskette for battalion roll-up. <strong>The</strong> battalion<br />

sends the data directly to LOGSA via e-mail . <strong>The</strong> e-mail address for AMSS data is ridbdata@logsa.redstone.army.mil.<br />

f. Separate units who are not aligned with a battalion forward the monthly AMSS end of period report (which<br />

includes the monthly usage report) directly to the supporting SAMS site, where the data are sent to LOGSA. In a<br />

situation where a unit is not aligned with a battalion and is not supported by a SAMS site, data is forwarded directly to<br />

LOGSA via e-mail. <strong>The</strong> e-mail address for AMSS data is ridbdata@logsa.redstone.army.mil.<br />

5–7. DA Form 2408–9<br />

a. Purpose. DA Form 2408–9 gives maintenance managers at all levels a record of equipment acceptance and other<br />

inventory and maintenance data. It also tracks ownership, location, usage, transfers, gains, losses, and overhauls/<br />

rebuilds/recapitalizations. AR 710–3, chapter 5, controls registration numbers on specified <strong>Army</strong> vehicular equipment<br />

in order to be used on public roads and highways. <strong>The</strong> registration numbers of equipment is recorded on DA Form<br />

2408–9.<br />

b. Use.<br />

(1) This form is used to track equipment: acceptance, gains, losses, transfers, usage, overhaul, rebuilds, recapitalization,<br />

NSN changes, and registration number.<br />

(2) Equipment requiring DA Forms 2408–9 are found in appendix E. Other equipment may need these forms when<br />

directed by HQDA or other command. AR 710–3, chapter 5, also has equipment requiring registration by equipment<br />

category. When both this pamphlet and AR 710–3 cover equipment, keep only one set of forms. Separate forms are not<br />

needed.<br />

(3) LOGSA at Redstone Arsenal, AL, is responsible for maintaining the <strong>TAMMS</strong> equipment database (TEDB). DA<br />

Form 2408–9 is the prime source of information in support of the database. <strong>The</strong> database supports the <strong>Army</strong>’s budget,<br />

equipment procurement, operating tempo determinations, and provides data output summaries and reports in support of<br />

equipment age, ownership, asset position, usage, overhaul/rebuilds/recapitalization, and safety recall requirements. <strong>The</strong><br />

TEDB is the repository of record for the <strong>Army</strong> Vehicle Registration Program. Requests for information are addressed<br />
