SECTION II—MAINTENANCE ACTIVITY DATA.<br />

3a. WORK ORDER NUMBER (WON). Enter WON (see paragraph 3–4 for assignment of WONs).<br />

3b. SHOP SECTION CODE. Enter shop section code. <strong>The</strong>se codes are assigned to uniquely identify a particular maintenance shop<br />

section. Each maintenance battalion operating SAMS assigns codes A through Z locally. Examples: A—Automotive Shop, B—Battery<br />

Shop, C—Commo Shop, and so on.<br />

3c. PHONE NO. Enter the phone number of the <strong>Maintenance</strong> Activity.<br />

SECTION III—EQUIPMENT DATA. Use as needed or as prescribed locally.<br />

SECTION IV—TASK REQUIREMENTS DATA. Blocks 27a–27i, self-explanatory.<br />

SECTION V—PART REQUIREMENTS. Blocks 28a–28o, self-explanatory<br />

1. NOMENCLATURE. Enter the noun of the item.<br />

2. MODEL. Enter the Model number.<br />

3. SERIAL NUMBER.<br />

a. Enter the serial or registration number.<br />

b. For watercraft, use the DA Hull number.<br />

STATUS SYMBOL a. Enter the status symbol that applies to the fault. Status symbol X faults do not go on this form.<br />

FAULT b. Enter the fault. Entries are transcribed from column c, DA Form 2404.<br />

REASON FOR DELAY c.<br />

1. Give the reason for the delay.<br />

2. If the reason is a part on order, print the document number and NSN or part number for each. For parts on order from QSS, print QSS<br />

and the date information was provided about the part not being on hand. For items on order from the Self-Service Supply Center<br />

(SSSC), print SSSC and the date information was provided about the item not being on hand.<br />

3. If the part is cancelled later, print “cancelled” and the date the part was cancelled. <strong>The</strong>n line through the entry from columns a through<br />

f. If the part is still needed, reorder it. Enter the fault, NSN or part number, and new document number on the next open line.<br />

4. If the delay is until the next scheduled service, print “Schedule for next PM service.” State which service and the date or miles/hours<br />

when it is due.<br />
