2. NOMENCLATURE AND MODEL.<br />

a. Enter the noun abbreviation and the model of the equipment.<br />

b. For watercraft, use the noun abbreviation and Hull Design Number.<br />

3. REGISTRATION/SERIAL/NSN. Enter the serial or registration number. Enter the NSN when no serial or registration number is<br />

available. For watercraft, enter the DA Hull Number.<br />

4a. MILES. Enter the miles or kilometers on the equipments odometer as of the date in block 5. Round to the nearest mile or kilometer.<br />

Enter the letter K before the number if the reading is kilometers. Leave blank if the item does not have an odometer.<br />

b. HOURS.<br />

1. Enter the hour meter reading in hours as of the date in block 5.<br />

2. Leave blank if hours do not apply to the equipment.<br />

c. ROUNDS FIRED. Leave blank.<br />

d. HOT STARTS. Leave blank.<br />

5. DATE. Enter the calendar date.<br />

6. TYPE INSPECTION. Enter the letters BDAR.<br />

7. APPLICABLE REFERENCE.<br />

TM NUMBER,TM DATE.<br />

a. Enter the number and date of the PMCS TM. When two TMs cover an item, enter the second TM number and date in the second TM<br />

number and date block.<br />

b. When the manual has changes, print W/C and the latest change number after the TM number. <strong>The</strong>n, enter the latest change date in<br />

the TM date block.<br />

8a. SIGNATURE (Person(s) performing inspection). Leave blank if the item does not have an odometer. When the repair or replacement<br />

has been accomplished, the person doing the job signs name and enters rank.<br />

8b. TIME. Leave blank or use as needed locally.<br />

9a. SIGNATURE (<strong>Maintenance</strong> Supervisor). <strong>The</strong> maintenance supervisor or the commander’s designated representative signs name and<br />

enters rank. This is to ensure that when corrective actions are taken, no safety faults still exist that would endanger the operator or cause<br />

further damage to the equipment.<br />

9b. TIME. Leave blank or use as needed locally.<br />

10. MANHOURS REQUIRED. Leave blank or use as needed locally.<br />

DEFICIENCIES AND SHORTCOMINGS c. Briefly describe the fault. If more than one deficiency or shortcoming is noted, leave enough<br />

room between entries to allow for corrective action taken to be annotated.<br />

CORRECTIVE ACTION d. Explain actions taken to correct or repair the fault. Note any parts replaced, parts ordered, and work done.<br />
