UNUSUAL NECK OF FEMUR FRACTURE AFTER TOTAL HIP REPLACEMENT- A<br />

CASE REPORT<br />

We present an unusual case of fracture at the neck of the implant following a total hip<br />

replacement. A 72 year old gentleman underwent cemented total hip replacement for<br />

primary osteoarthritis of the hip. The post operative period was uneventful and he<br />

was mobilising well without any post operative complications of infection or<br />

dislocation. He presented to accident and emergency following a mechanical fall from<br />

a standing height on to his hip and inability to mobilise two years following the hip<br />

replacement. Clinical examination revealed a tender hip, with the leg shortened,<br />

flexed, abducted and externally rotated. He had no distal neurovascular deficits. All<br />

movements of the hip were restricted with pain. Radiographs revealed a fracture<br />

through the neck of the femoral stem in a cemented THR. The components were well<br />

cemented with no evidence of loosening. He underwent a revision total hip<br />

replacement. We performed an extended trochanteric osteotomy to remove cement<br />

and extract the broken implant. He was revised to a longer stem with secondary<br />

stabilisation with cables. He recovered uneventfully following the operation and was<br />

mobilising independant when he was discharged from the hospital.This case<br />

highlights the need for awareness of the possibility of a broken implant following<br />

THR.<br />
