USE OF EXTENDED TROCHANTERIC OSTEOTOMY IN REVISION OF INFECTED<br />

HIP ARTHROPLASTY<br />

Extended trochanteric osteotomy (ETO) was known as a useful method to remove<br />

implants and cements during revision surgery. The purpose of this study was to<br />

evaluate the safety and efficacy of using ETO on the treatment of prosthetic hip<br />

infections. We reviewed 23 patients who had undergone two-stage exchange<br />

procedures using ETO for periprosthetic hip infection. Control group was 46 patients<br />

who had undergone revision hip arthroplasty using ETO for aseptic loosening. Study<br />

group consisted of 11 men and 12 women with a mean age of 59 years (33 to79).<br />

Mean follow-up was 49 months (24 to105). Control group consisted of 21 men and<br />

25 women with a mean age of 61 years (34 to88). Mean follow-up was 54 months<br />

(24 to104). Outcome measures were Harris hip score, time to healing of ETO,<br />

femoral component stability, eradication of infection, and postoperative<br />

complications. The mean Harris hip score was improved from 36 points<br />

preoperatively to 82 points postoperatively. Periprosthetic hip infection was<br />

eradicated in 22 (96%) of 23 patient. In all cases, ETO was healed at a mean of 11<br />

weeks. There was no aseptic loosening of femoral component. Complications<br />

included two postoperative femoral fractures, one transient peroneal nerve palsy, and<br />

one dislocation. Postoperative Harris hip scores, time to healing of ETO, femoral<br />

component stability, eradication of infection, and overall complication rates did not<br />

differ between the study and control groups. ETO can be used safely and effectively<br />

as part of two-stage revision procedures for periprosthetic hip infection.<br />
