WITH CONSTRAINED LINERS OR DUAL MOBILITY OUTCOMES OF TOTAL HIP<br />

ARTHROPLASTY ARE SIMILAR FOR PATIENTS WITH DISPLACED FEMORAL<br />

NECK FRACTURES AND OSTEOARTHRITIS<br />

This study attempted to evaluate whether total hip arthroplasty (THA) for displaced<br />

femoral neck fractures had significantly different outcomes when compared with THA<br />

for osteoarthritis. 1800 patients had THA between 1999 and 2005. Six hundred<br />

patients (mean age, 79.7 years) had THA for displaced femoral neck fractures; 1200<br />

patients (mean age, 76.9 years) were treated with total hip arthroplasties for<br />

osteoarthritis. 75 per cent with fractures and 60 per cent with OA had a preoperative<br />

medical comorbidity including neurologic and vascular diseases. All THAs were done<br />

using postero lateral approach and cemented implants. Standard liners were used for<br />

OA. Constrained liners or dual mobility were used to avoid dislocation in fractures.<br />

Patients had radiographic assessment, physical examination, and evaluation with the<br />

Harris hip score. The mean followup was 7 years. The mean Harris hip score for<br />

patients treated with a total hip arthroplasty for a femoral neck fracture was 85 points;<br />

the mean hip score for patients treated with a total hip arthroplasty for osteoarthritis<br />

was 87 points. Patients who were treated with a total hip arthroplasty for a femoral<br />

neck fracture did not have increased perioperative morbidity compared with patients<br />

who had a total hip arthroplasty for osteoarthritis. The rate of dislocation was 2 per<br />

cent and not significantly different between the two groups. This study suggests that<br />

with selected liners the outcomes for total hip arthroplasties in this consecutive series<br />

of patients treated for displaced femoral neck fractures and osteoarthritis are<br />

comparable.<br />
