COMPLICATIONS OF SURGICAL TREATMENT OF EXTREMITY AND TRUNK<br />

INTRODUCTIONThe management of Soft Tissue Sarcoma is often associated with<br />

extensive tissue resection to achieve a good margin. We reviewed significant<br />

complications following surgical treatment for soft tissue sarcoma of the extremity<br />

and superficial trunk. PATIENTS AND METHODThis is a 7 year retrospective study<br />

involving patients presenting with extremity and trunk soft tissue sarcoma from<br />

January 2002 till December 2008 at the Royal Liverpool and Broadgreen University<br />

Orthopaedic Oncology unit. The minimum follow up period was 12 months and all<br />

patients records were reviewed with emphasis on significant post operative<br />

complications.RESULTS126 patients were eligible for the study with age range<br />

between 16 years and 96 years. The histological profile included, fibrosarcoma in<br />

24% [n=30], liposarcoma in 23% [n= 29], leiomyosarcoma in 19% [n=24],<br />

undifferentiated sarcoma in 10% [n=13]. There were 41 % high grade, 13%<br />

intermediate and 32% low grade tumours. A significant proportion of the masses<br />

were located in the thigh and buttock [55%]. Wide local excision was possible in<br />

48%, and amputation was performed in 13% of the patients. The common<br />

complications include treatment failure in form of local recurrence in 17.5% [n=15]<br />

and distant metastases in 15.1% [n=13]. Surgical complications such as seroma<br />

formation [22%] and wound infection [11.6%] were also recordedCONCLUSIONThe<br />

management of soft tissue sarcoma is associated with significant morbidity.<br />

Adequate awareness about the common complications and appropriate preoperative<br />

treatment plan may improve the overall management of patients.<br />
