TWO-STAGE ARTHROPLASTY USING A FUNCTIONAL TEMPORARY<br />

PROSTHESIS TO TREAT INFECTED TOTAL HIP ARTHROPLASTY & SEPTIC<br />

ARTHRITIS OF THE HIP-A MODIFIED TECHNIQUE<br />

Introduction: Two-stage revision arthroplasty is the gold standard for treatment of<br />

infection at the site of a total hip Arthroplasty. The staged technique, as well, is a<br />

good approach in dealing with end stage septic arthritis of the hipMethods: 8 patients<br />

with infected total hip arthroplasty and 6 patients with adult native joint septic arthritis<br />

of the hip were treated with two-stage revision. We utilized an inexpensive modular<br />

femoral component coated with antibiotic-impregnated polymethylmethacrylate<br />

articulating with a polyethylene acetabular liner inserted using a modified technique.<br />

The construct functions, temporarily, as a conventional total hip prosthesis inbetween<br />

the two stagesResults: 12 patients were followed for an average of 18<br />

months. 2 patients were excluded (no second stage). No patient was lost to follow-up<br />

evaluation from those included. No patient had recurrence of infection. No clinical,<br />

laboratory, or radiographic evidence of infection at the final follow-up. 9 patients<br />

(75%) had an improvement of Harris hip score of at least 30 points. Conclusion:<br />

functional temporary construct formed of inexpensive femoral component articulating<br />

with polyethylene acetabular liner can be used safely and successfully allowing range<br />

of movement, weight bearing, with little cost as a joint-preserving spacer in the<br />

awaiting period between the two stages in management of infected total hip<br />

arthroplasty and septic arthritis of the hip<br />
