OSTEOSYNTHESIS OF LONG TUBULAR BONES FOR FRACTURE<br />

CONSEQUENCES WITH HYDRAULICALLY EXPANDABLE NAIL (HEN)<br />

Aim: To analyze the effectiveness of using HEN during fracture<br />

consequences.Material and methods: 37 patients underwent surgery with HEN from<br />

the age of 19 to 51 years. The following are the segments where we used HEN:<br />

Humerus 2, Femur 16, Tibia 19. HEN was used in 30 patients with old and nonunion<br />

fractures, in 5 patients after correcting osteotomy and in 3 patients with<br />

infected, non-union fractures. In 30% of patients osteosynthesis with HEN was<br />

performed at the ishtmal/osteotomy/fracture nonunion regions and the other 70% of<br />

osteosynthesis was performed at supra or sub-isthmal region of the long bones. Auto<br />

bone grafting was performed with HEN in 12 patients with hypotropic<br />

pseudoarthrosis.Results:Post operative follow up time was vaeried up to 2 yrs. In 23<br />

patients, the complete clinical and radiological consolidation was achieved. The other<br />

patients are now under follow-up with good radiological prognosis.Summary: The<br />

following advantages are we observed: It reduces operation time, intra-/post<br />

operative blood loss, radiation to the patient and the medical personals. We noticed<br />

that stability of fixation with HEN is identical or more than the fixation stability of<br />

conventional blocking nails during osteoporosis, supra and sub-isthmal fractures and<br />

non-union. The stable fixation allowed us to activate the patient in the early post<br />

operative period which is especially important to the patients with juxta-articular<br />

fractures.<br />
