SACRAL FRACTURE DISLOCATION OF SUICIDAL JUMPER<br />

Study design : A retrospective study Objectives : The author reviews 4 atypical cases<br />

of sacral fracture composed with bilateral vertical fracture line at sacral alar area and<br />

transverse fracture at upper sacrum with the relevant literature.Summary of Literature<br />

review : Suicidal jumpers fracture has similar fracture pattern and clinical<br />

manifestation. Material and Method : From August 2003 to March 2007, 4 patients.(<br />

aged 23, 18, 25, 25 ) were selected who could be followed up for more than 2<br />

years(24-48momths), they were managed surgically and conservatively.Result : The<br />

neurological function of by Gibbsons neurologic classification, the first patient<br />

managed surgically, had improved from grade 4 to 2 and walked without walking aid.<br />

The second patient managed surgically, had not improved neurologically and walked<br />

with one cane. The third patient who was managed with skeletal traction had<br />

improved from grade 2 to grade 1 by Gibbons neurological classification and walked<br />

with one cane. The last patient who was managed with bed rest only had not<br />

improved neurologically and walked with two canes. Conclusion : Fracture dislocation<br />

of upper sacrum is a very rare injury and has similar neurological symptom and<br />

fracture pattern with that of Suicidal Jumpers Fracture. The injury is almost always<br />

requires surgical intervention and is difficult to treat.<br />
