SUPERIOR LOCKING COMPRESSION PLATING OF CLAVICULA FRACTURE<br />

NON-UNIONS WITH OR WITHOUT BONE GRAFTING - OUTCOME IN 58 CASES<br />

A retrospective outcome-study with respect to the number of operations needed to<br />

reach union and the surgical technique. 58 patients treated from 2003 to 2009 were<br />

retrospectively analysed. Results: 15 patients were female. The average age was 46<br />

years. Indication: Symptomatic non-union with disability from pain and altered<br />

shoulder mechanics. 38 patients were initially treated conservatively, 12 had a plate<br />

fixation, 6 patients were treated with elastic nailing and two patients with tensionband<br />

wiring of lateral fractures. In 2 patients a postoperative septical non-union was<br />

treated. 86% showed a bone union after the first operation with LCP. In 31 patients<br />

bone grafting was performed. In 8 patients a revision surgery had to be performed<br />

because of persisting non- union. In one of those patients union cold not be obtained<br />

even in 3 further operations with additional application of BMP-7. In two of our cases<br />

a postoperative infection occurred with following removal of the plate and no further<br />

treatment in one of them.A toto of 95% union rate could be achieved. In 52% a<br />

removal of the plate was performed. Conclusion: Superior locking compression<br />

plating in combination with bone grafting is a reliable and reproducible treatment for<br />

non- unions of the clavicula. Because of cosmetic reasons but also skin irritation<br />

about half of the plates were removed. The anterior-inferior plating could have<br />

advantages in regard of the number of plate removals.<br />
