RETROGRADE INTRAMEDULLARY NAILING FOR SUPRACONDYLAR AND<br />

INTERCONDYLAR FRACTURES OF THE DISTAL PART OF THE FEMUR<br />

INTRODUCTION: Retrograde intramedullary nailing represents an established<br />

fixation method for fractures of the distal femur, offering an adequate alternative to<br />

other techniques due to soft tissue dissection is minimized. The purpose was to<br />

investigate in a retrospective analysis the results of retrograde nailing in distal<br />

femoral fractures. MATERIAL and METHOD: From January 2005 to December 2008,<br />

36 fractures of the distal femur were treated by retrograde femoral nailing. The mean<br />

age was 67,7 years (17-94) and 20% presented associated fractures. Follow up<br />

ranged between 14 and 25 weeks (19). Fractures were classified according to the<br />

AO/ASIFF classification. For fracture fixation the Distal Femoral Nail SCN (Stryker®)<br />

was used. All patients were assessed with regard to operative time, blood trasfusion,<br />

hospital stay, and postoperative complications. All patients underwent a clinical and<br />

radiological evaluation. RESULTS: Average time to surgery was 6 days (0-20). 51%<br />

cases were 33-A1 fracture type. The average operative time was 66 minutes. Mean<br />

hospital stay 19,96 days (3-80). 13 patients required blood transfusion during hospital<br />

stay. 32 fractures (88%) healed in an average of 3.9 months. There were no cases of<br />

angular malalignment, nail failure or knee sepsis. Four non-union, two cases of knee<br />

stiffness and an intraoperative fracture of the femur above the nail were the main<br />

complications in this serie. DISCUSSION: Retrograde nailing represents a reliable<br />

fixation method for extra-articular (33 - A1-3) and simple intra-articular (33 - C1-2)<br />

fractures of the supracondylar area, offering a valuable alternative with few<br />

complications.<br />
