LOW-ENERGY FRACTURES - A PREVENTIVE CHALLENGE FOR AN<br />

ORTHOPAEDIC SURGEON?<br />

An analysis of bibliographical data and every day practice demonstrate that the level<br />

of self-consciousness of the need of prevention of low-energy fractures in the<br />

orthopaedic community is relatively low. On the other hand proven pharmacological<br />

interventions exist that could decrease the risk of osteoporotic fracture by half in<br />

comparison with untreated subjects. The goal of this presentation is to alert vast and<br />

basic orthopaedic community about need of introduction a pharmacological treatment<br />

in patients with low-energy fracture. In spite of the fact that some primary actions<br />

confirming the diagnosis of osteoporosis (e.g. DEXA) should be introduced, they are<br />

not a prerequisite for introduction of the treatment even in bed-ridden subjects as<br />

parenteral forms of antiosteoporotic drugs are available.A concise review of the<br />

burden and impact of low-energy fractures on the health-care economies in welldeveloped<br />

countries will be discussed. The modalities of surgical interventions<br />

concerning different locations of fractures will be presented stressing the need for<br />

augmentation of bone stock obviously deficient in osteoporotic subjects. Postsurgical<br />

complications will be stressed with the emphasis on life- and function<br />

threatening conditions concerning predominantly vertebroplasty and kyphoplasty. A<br />

need for early pharmacological treatment including parenteral therapies with antiosteoporotic<br />

drugs will be demonstrated since well documented data exist showing<br />

that such measures could reverse the secular trends in osteoporotic fractures<br />

occurrence.A low-energy fracture in the adult should always alert of possible<br />

underlying osteoporosis and treating orthopaedic surgeon should be aware of a need<br />

for urgent apart from surgical concomitant pharmacological intervention.<br />
