ARTROSCOPIC TREATMENT OF ELBOW JOINT CONTRACTURE<br />

Efficiency evaluation of arthroscopic debridement of elbow joint for contracture of<br />

different genesis. The analysis of results on arthroscopic treatment of 18 cases of<br />

elbow joint contracture was conducted. The causes of contracture were: 9 - intraarticular<br />

fracture of distal part of humerus, 3 - ulnar process fracture, 3 - rheumatoid<br />

arthritis, 1 - septic arthritis, 1 - agnogenic chronic arthritis, 1 - heterotopic ossification<br />

resulted from neuro injury of central genesis. The average age of the patients was<br />

23±2.3 years old (15-38). Indications for arthroscopic joint debridement were<br />

limitation in the range of motion with functional curve no less than 100°. The results<br />

were evaluated within 1-3 years after operation. The operation included: joint<br />

examination, front debridement (resection of coronoid process, adhesions removal),<br />

proximal resection of joint capsule, debridement of olecranon fossa, resection of<br />

olecranon, debridement of medial and lateral introversion, olecranon fossa<br />

fenestration. In the case of rheumatoid arthritis with evident loss of rotational<br />

movements, resection of head of radius was additionally applied. For removal of<br />

large abarticular ossificate, an additional incision was applied. The clinical evaluation<br />

of the results showed in 16 (88.9%) cases excellent and good results, i.e. complete<br />

or partial recovery of joint function. On average, extension deficit reduced from 46 to<br />

12°, flexion range increased from 92 to 136°, rotat ional movements became full. In<br />

the cases of elbow joint contractures of different genesis, arthroscopic debridement is<br />

indicated with differentiated choice of approaches and surgical technique depending<br />

on the cause of contracture.<br />
