EVALUATION OF SENSITIVITY AND SPECIFICITY OF 99M-TECHNETIUM<br />

ISONIAZID SCAN FOR DIAGNOSIS OF SKELETAL TUBERCULOSIS.<br />

The aim of our study was to evaluate the sensitivity and specificity of 99m-<br />

Technetium Isoniazid scan for the diagnosis of skeletal tuberculosis. 20 patients with<br />

strong clinical, radiological and microbiological evidence were included in the study<br />

presenting in the time period of January 2007 to February 2009. Patients already on<br />

antitubercular treatment, pregnant females, those with hepatic and renal<br />

abnormalities, and those with hypersensitivity to Isoniazid are excluded from the<br />

study. All patients were initially subjected to 99m-Tc Bone Scan and those patients<br />

with positive result were then subjected to INH Scan and images were acquired at<br />

1hr, 4 hr and 24 hr. After the images are acquired, the samples were taken and sent<br />

for mycobacterial culture (taken as the gold standard). Of the culture positive cases,<br />

69.2 % of cases were positive and 30.8% of cases were negative for 99m-Tc- INH<br />

Scan. Sensitivity, Specificity, Positive Predictive value, Negative predictive value and<br />

diagnostic accuracy were found to be 69.2%, 71.4%, 81.8%, 55.5%, and 70%<br />

respectively. Hence we conclude that 99mTc-INH Scan has a good sensitivity and<br />

specificity for the diagnosis of skeletal tuberculosis. Furthermore it is noninvasive,<br />

nontraumatic and it can be done on outpatient basis to rule out skeletal tuberculosis.<br />
