BLOOD ORDERING SCHEDULE FOR ELECTIVE ORTHOPAEDIC SURGERIES<br />

Aim: To audit the blood utilization in elective Orthopaedic surgeries in our hospital<br />

over two years period and to recommend an optimal blood ordering schedule.<br />

Methods: Retrospectively, Data was collected included type of orthopaedic surgery,<br />

pre and post operative hemoglobin levels, number of units cross matched, number of<br />

units returned, number of units transfused, cross match to transfusion ratio (CT),<br />

transfusion probability (%T), transfusion index (TI), estimated blood loss for each<br />

surgery and predicted fall in hemoglobin. The number of red cell units required for<br />

each procedure was calculated using the equation proposed by Nutall et al,No. of<br />

red cell units required for procedure = Predicted Hb fall (Preoperative -<br />

Postoperative Hb) Results: A total of 472 patients with a median age of 31 yrs (9 to<br />

88 yrs) were evaluated. The mean preoperative hemoglobin was 10.51gm/dl. A total<br />

of 1369 units were cross matched and only 538 units were transfused to 247<br />

patients. 225 patients (47.67%) did not require transfusion during their stay in<br />

hospital. Six of the thirteen elective procedures had CT ratio higher than 2.5. Eight of<br />

the thirteen procedures (61.54%) had low transfusion index (TI
