THE EFFECT OF ACETABULAR CUP PLACEMENT ON HIP JOINT FUNCTION<br />

OF CROWE TYPE 3 AND 4 HIPS<br />

PURPOSE: Total hip replacement (THR) is more complicated with patients with<br />

coxarthrosis due to developmental displasia of the hip (DDH) than patients with<br />

primary coxarthrosis. We studied radiological and functional results of patients with<br />

crowe type 3 and 4 hips which received acetabular cup placement for THR.<br />

MATERIAL and METHOD: 10 hips (8 female, 2 bilateral, 5 right, 5 left) that received<br />

THR were evaluated. The average age was 52,4 (36,9 - 65). Preoperatively 4 hips<br />

were evaluated as type 3 and 6 hips type 4 according to the Crowe classification. In<br />

the final evaluation functional results were evaluated using Harris Hip Score (HHS),<br />

Oxford Hip Score (OHS) and Visual Analog Score (VAS). For 3 hips the acetabular<br />

component was placed in the high hip rotation center with extenstive soft tissue<br />

release and no femoral shortening. For 7 hips the component was placed in the<br />

normal rotation center and 4 of these patients received femoral shortening. The<br />

average follow up was 66.3 months (17 - 97). RESULT: In the final control OHS was<br />

an average of 17.5, and HHS was an average of 89.7. The preop average VAS was<br />

9.2, postop was 2. CONCLUSION: Femoral shortening for Crowe type 3 and 4<br />

patients may be necessary for acetabular placement in the normal hip rotation<br />

center. However, patients who received the acetabular component placement in the<br />

high hip rotation center compared to normal rotation placement patients showed no<br />

difference in their radiological and functional mid term results.<br />
