MONOARTICULAR PSEUDOGOUT OF HIP PRESENTING AS SEPTIC<br />

ARTHRITIS - A CASE REPORT<br />

Objectives: Calcium pyrophospate dihydrate (CPPD) disease is the second most<br />

common crystal-induced form of arthropathy. CPPD disease usually presents as<br />

polyarthritis with the most common joints involved being the knee, shoulder, wrist,<br />

elbow, and ankle. Rarely, the small joints of the hands and feet can be affected. The<br />

heterogenous presentation of CPPD disorders has led to its classification into seven<br />

subgroups. We present a case of monoarticular pseudogout of the hip joint<br />

presenting as septic arthritis which has not been previously reported in the English<br />

literature. Case report:An 89 year old gentleman presented with acute onset severe<br />

hip pain, inability to weight bear, raised body temperature and raised CRP of 315.<br />

Hip aspiration revealed calcium pyrophosphate crystals and cultures of the sample<br />

did no grow any organism. Patient started to mobilise gradually within seven days<br />

with analgesics and recovered completely. Conclusion: In case of acute presentation<br />

with painful limitation of all hip joint movements and local tenderness in elderly<br />

patients, septic arthritis should be excluded first. Aspiration of the hip should be<br />

attempted before any surgical intervention is undertaken to exclude other causes.<br />
