70 LOOSE BODIES IN BAKER'S CYST -CASE REPORT<br />

Synovial osteochondormatosis is a rare benign condition characterized by formation<br />

of one or more loose bodies by the synovial membrane in a joint, or occasionally in a<br />

bursa or tendon sheat. We report an interesting case of a patient with 70 loose<br />

bodies in large 7x2.5x2cm Baker’s cyst of the left knee. A 68 year old caucasian<br />

male presented to the outpatient department complaining of increasing pain and<br />

stiffness in his left knee over the last 3 years. Radiographs of his left knee showed<br />

moderate to severe degenerative changes involving the patello-femoral and medial<br />

joint compartment. In addition to osteoarthritic changes extensive calcific foci within<br />

the posterior aspect of the knee joint where noted. Ultrasonographic appearance was<br />

suggestive of synovial osteochondromatosis within a baker cyst and the mass<br />

appeared to originate form gastrocnemus, semimembranosus bursa. The patient<br />

underwent a routine total knee replacement and through a small posterio-medial<br />

incision the popliteal cyst was exposed and 70 loose bodies were removed. Our<br />

patient made an unremarkable recovery and was discharged on the 4th day post-op.<br />

To our knowledge such a high number of loose bodies confined to Bakers’s cyst has<br />

never previously been described.<br />
