RESULTS OF KNEE CARTILAGE INJURY TREATMENT BY ARTHROSCOPIC<br />

OSTEOCHONDRAL AUTOGRAFT USING PSEUDOSYNOVIAL LIQUID<br />

Study of chondro- and osteointegration of autografts in animal models and evaluation<br />

of remote results of osteochondral autograft in knee joint cartilage injury. We<br />

analyzed the results of osteochondral autograft surgeries in patients by OATS<br />

(osteochondral autograft transplantation system). Surgery was indicated in cases<br />

with full-thickness isolated cartilage defects in the stressed zones of femoral<br />

condyles of the knee joint with subchondral plate stripping and total area not<br />

exceeding 4 cm2. In 66,7% cases, cartilage damaged occurred as the result of acute<br />

or chronic joint trauma, in 33,3% cases it was caused by osteochondritis dissecans.<br />

Treatment results were evaluated 4-7 years after the surgery on Lysholm-Tegner<br />

Scale. Chondro- and osteointegration was studied in experimental dog models of fullthickness<br />

damage of knee joint cartilage. Pseudosynovial liquid containing the<br />

mixture of cholesteric-nematic liquid crystals in 2% Na-CMC water solution. In this<br />

animal study, chondro- and osteointegration of autograft with the surrounding hyaline<br />

cartilage and host bone bed was reported 2,5 months after osteochondral autograft<br />

transplantation. Pseudosynovial liquid stimulated the process and facilitated<br />

activation of chondrogenesis, improvement of reproductive functions of cartilage<br />

cells, formation of interfacial regenerated cartilage tissue from hyaline cartilage, and<br />

practical absence of dystrophic changes in cartilage matrix. Analysis of the remote<br />

results of treatment of knee cartilage defects by osteochondral autograft<br />

transplantation revealed excellent and good results in most cases (92,6%).<br />

Satisfactory cases were connected with the development of joint cartilage<br />

chondromalacia around the autograft and on the opposite joint surface of the tibial<br />

plateau.<br />
