CHECKLIST OF DOCUMENTS OF PASSPORT FOR MINOR (UNDER 15 YEARS
OLD) WITH 05 YEAR VALIDITY/RENEWAL
□ Passport Online Application form No.-1 to be filled in English.
□ Please fill the online application form completely, in particular, column as applicable. It is mandatory
that all signatures are within the boxes.
□ In case a questions is not applicable, mention N/A, do not leave anything blank.
□ Please fill in an Indian address (as mentioned in current passport) at part 3a and the Dutch address at
part 3b.
□ Address to be printed on passport should be the same address as in one of the parent’s Indian passport.
In case of change of address a proof of address change should be attached.
□ Name change, papers from notary and copy of the two newspapers confirming the name change and
an uittreksel from the Dutch town hall.
□ The requisite amount of Passport , ICWF Fee & VFS Service charges to be paid at India Visa
Application Centre.
□ Photographs - 4 Four recent color passport size (about 35 mm x 35mm) photograph, showing full
frontal view of face against a white background, 01 photograph to be pasted on the application form
& other 03 to be attached with the application form.
□ Existing Indian passport of minor in original, along with a photocopy.
□ Resident Permit of minor in Netherlands" or the receipt for the renewal of the resident permit along
with expired Resident Permit.
□ Birth certificate of minor along with original
□ No objection certificate from Parents (NOC from both the parents is mandatory)
□ Existing passport of the both parents in original, along with a photocopy.
□ Resident Permit of parents in Netherlands" or the receipt for the renewal of the resident permit along
with expired Resident Permit.
□ In case the parents are separated/divorced/deceased proof of the same should be submitted with the
application.
□ Applicants are required to personally come to the India Application Centre to apply for issue of a new
passport.
□ Minor who cannot sign, a thumb impression may be put in place of signatures in the application form.
(Left hand thumb impression in case of male child and right hand thumb impression in case of female
child).
Please Note: Above requirements are only indicative and attempt has been made to cover requirements
for a major section of applicants. However, the Consular Officer may ask for additional documents and
/or could call the applicant for an interview also after examining the application for issuance of
Passport.
